The Purrgil had been encountered by the Ghost Crew prior to Thrawn and likely was not reported to Command. It was that one unpredictable element (and Palpatine's order to capture Ezra alive and then release his bindings) that primarily led to Thrawn's defeat. It was a small misreading of Wookie hair that led to his defeat originally as well. Always one small unknown.

That is who he was originally modeled after, yes. Timothy Zahn wanted to create a character that would sufficiently challenge our heroes back in the 90s; someone who, rather than using special abilities or powers, was nothing more than someone who had copious amounts of information at his disposal, and an understanding of how to use it.
