He's a specialist. He probably doesn't sign any death certificates. The Attending Physician in the hospital they pass away in signs. Outside the hospital a medical examiner usually signs unless they are being followed by a hospice or palliative team. The only reason a bariatric surgeon would sign is if they died on his operating table

I was hoping for the one where Dr. Now says "So chocolate is more important than living?". I had to go on a special diet for cancer treatment and changed that quote to help me stick with it. Every time, I got tempted to cheat the diet Dr Now comes to mind "So potato chips are more important than living?" It helped me stick with it.

I like how his “losing his cool” is just him being brutally honest. People who refuse to help themselves and make excuses aren’t going to listen if you try to sugarcoat things, so he knows he has to be completely honest even if it comes off harsh sometimes, but he still doesn’t overdo it. I quit cold turkey and never picked up another one. 41 yrs later, and I’ve never had another cigarette and I think they’re disgusting. You either want to be healthy or you don’t. I absolutely LOVE his NO BULLSHIT approach. As a bariatric surgeon, he's likely heard every excuse imaginable, and his experience has taught him to recognize when patients are lying. They say, " I don't know why I haven't lost weight. I'm sticking to the diet and exercising ", but they are not just lying to him, they are lying to themselves. It's comical when he drops logic bombs like explaining a calorie deficit and how many calories they are consuming to either not lose any weight or gain 50+ lbs in a month - which means at the very least a person is consuming twice their daily caloric value in food. It astounds me. I completely sympathize with emotional eating, I've done it and still do occasionally; however, I still live in reality knowing I need to check myself if I don't want to end up like some of these patients. I feel bad for them, but not really because the onus is on them and those that enable them.

As someone who's has had a full RnY (2006) and lost 225 lbs, I'm wondering how these people are passing the psych testing. I worked on myself for months before I even started a program because I was so scared of failure and dying. Taking control of your mental health is essential to long term success. My doctor told me that, "It's a tool in your weight loss arsenal, not the magic solution." Words to live by.

There are times, even now, when I overdo (Christmas Day, for example), and I'll have to lay down until the nausea passes. Knowing what I know now, I'd still do it all again. I'm alive. I don't blame him. These people come from far away asking him for help. He cares about their health. Their not following his instructions to help save their lives. They think they can eat like they do at the same time while on his diets. He tells them its no game. Unfortunately, some of them have passed away due to underlying at risk health issues.

As a WLS surgery paitent that has been successful in my losing and maintaining journey; he is not at any point losing his cool here. He is concerned about his paitents faliures and responsible enough to care for them to the point he will tell them about their issues and be blunt with how they need to stay on track. All these people are struggling with an eating disorder and need a serious mental health hand and the reminder he gave them.

Honestly I lost 60 pounds before and was still in a xxl shirt (doctor was the one who told me I lost 60 pounds)..I still looked almost the same, my chin was a bit smaller but you could hardly tell I lost 60 pounds, but I could FEEL it, I could breath better, could do things longer, I'm losing weight again after depression from my moms passing got me off track (no pop, less fast food if at all, eating 2 meals a day)

I love doctors like this. We need many more just like him. Tell patients the truth, even if they don't want to hear it. Don't let patients dictate the terms of their care. If they aren't willing to do what is needed, then they aren't serious about saving their own lives. There are plenty of people out here who genuinely would appreciate and value the care Dr. Now and his staff offer. He's savage, but he's the best kind of savage.
