I dont think Livia would have agreed to go. In her book, Tony was flipping by seeing a therapist. In fact, this is what made her order the hit on Tony. Livia would have treated Melfi the way she treated that nurse who walked away yelling "thats it!"

The costuming: Tony and Melfi in darker colors that go with the room, Melfi in a mature looking (she dresses like a Mad Men character) suit, while Carmella is in a cream colored suit feeling like she has to defend herself against Tony and Melfi

The acting and writing on this show is just beyond brilliant. It’s so realistic, yet you’re aware that it’s a dramatic TV program. It’s not necessarily plot driven, yet there’s always something happening. It’s a case study in psychology. Primarily dysfunctional families. Dysfunctional, unhappy people. The acting from all three in this scene is particularly powerful.

Carmela and Tony both look very pissed off in this scene. I hear them bickering and both of them seem very serious and not very happy at all. They're both frustrated in this scene. I think what Tony was doing for a living was putting a great deal of strain on their relationship. Melfi is right they both are very angry people in this scene!
