Incredibly informative video. Thanks, Dr. Peng. The fact that you want to help your patients get maximum results and are setting them up for that is amazing! I have an appointment with a Dr. Mathew Wells in FL to review my MRI and discuss PRP injections for knee arthritis. I am excited to potentially take better control of this disease through PRP, physical therapy, diet, sleep, stress reduction, etc. There should be more doctors like you in all medical specialities. @@JeffreyPengMD thanks Dr. Peng. One follow-up if I may. I want to do some cardio ahead of my appointment (as you suggested). It’s an initial appointment so I probably won’t get the injection at this one. My cartilage damage is under my patella. In terms of biomechanics (I think that’s the right word), what cardio workouts are best for knee cap damage. Is the elliptical okay? I ask because I went on it a couple of times recently and did have some pain for a day or two afterword (the pain was bearable). I’m generally also wanting to stay in good cardiovascular health. There is so much conflicting info on PRP injections to the knee. Understanding every patient is different , I have 2 important questions: in general, 1. what does your experience show to be the best frequency of intervals of shots? and 2. what is the best time period to spin the blood for--or what is the most appropriate amount of the concentration of plasma? I am a 53 year old with severe OA in my subcalcaneal and knee joint. I had 2 PRP in the subcalcaneal joint, 1 PRP on the left knee, and 2 PRP on my right. I have been pain free for 5 years now. Just wish I could find a clinic to inject PRP into my hip. My current regenerative clinic does not have ultrasound or fluoro guidance imaging.

@@Cybernetic263 I noticed improvements about the 6 week mark. Ask your doctor what and when to start strengthening exercises. That part is super important. No squats or lunges. Buy a foam exercise roll, sit on the ground with the roll under the pocket of your back knee, isolating your thighs & knee, and just extend straightening your knee. No weights, just plain foot lower leg extensions. Eventually ankle weights or a large bag of ice in a pillow case if you don’t have weights, just for the mild resistance. Some people see results in 4 weeks, some 12. It just depends on the extent of knee damage. One knee I had a second injection after seeing no results after 3 months. That second one worked and has not given me problems since my 2019 procedures.

@@Cybernetic263 wait until you are cleared for exercises from your doctor. I think I was cleared after 6 weeks, but ask your doctor, especially if you don’t have a follow-up appointment scheduled.

I think the pain level while undergoing prp is very much based on technique and experience of provider…..having mine in late October and freaking out about the pain because i am so needle phobic. But i really need this; trying to prevent a fusion.

This video is very informative, I did 1 PRP injection on my left knee and it was very painful and my leg froze I couldn't walk it was horrible but after a few hours there was no pain, just discomfort for about 4days.and I have 2more to do, not looking forward to it but it's necessary.

This channel is great. Thanks for all you do. I know you can't give medical advice here. I have what I think is a rare tendinopathy, it's bilateral calcific insertional triceps tendinopathy. The calcifications are visible on x-ray and MRI confirms fluid in the tendons. The problem goes back several years, I had taken a long layoff from the gym to resolve the left side. When I finally got back into it I only had minor pain. I built up slowly only doing pushups. Eventually starting triceps extensions with only 3 lbs. After many months I graduated up to 10 lbs and out of nowhere was getting pain on my right side, near full extension of the elbow, accompanied by tenderness on the lateral side of the olecranon. I stopped doing the extensions but continued with pushups because there was no real pain. That was 10 months ago. In an attempt to finally resolve this, I laid off pressing exercises altogether for 4 weeks, then did some very light pressing I had no pain but the elbow was irritated a few hours later. So, I seem to be worse off for taking the layoff. I'm scheduled for a PRP shot as I've had 2 opinions that think that's a reasonable option. Neither opinion thinks the calcifications are the issue or should be dealt with directly. So my question is if PRP has any research for insertional tendinopathies? Also, should the calcifications be dealt with could they be the source of the pain?

Pretty much all the PRP research on tendons is insertional or origin. There are no (that I can think of) studies looking specifically at triceps tendinopathy, but anecdotally, yes it works. Good luck!

I had injections in rotator cuff tear .Extremely painfull and lasted for a good 48 hrs.Should of had a driver .Couldn’t move my arm for a few days as well .Praying it works.

I was disappointed with my PRP treatments, inflammation did dissipate but the OA pain in my knee just came back. 3 months later second treatment and knee pain was worse. I personally had more success with Haurlic Acid injections, specifically Monovisc. I was so optimistic about the about the procedure. Good luck.

Thanks for your comment and sharing your experience. There's a lot of research (will make a vide on this very soon) on the different types of PRP, specifically looking at platelet counts. It'd be interesting to learn how many platelets you had in your PRP injection (can probably get a rough estimate by how much blood they drew) and correlate that with outcomes. HA injections are still a good option!

@@JeffreyPengMD I am 54, very fit. I ran marathons, mud runs, and other competitive type races. I' have done weight training and calisthenics my entire life. I eat a healthy diet. My draw was about 5ml of blood both times. My platelets were over 350 based of my labs. I also was a PT patient for the same issue for over two yrs before trying the gels injections. I tried PRP because of the current information available, as I stated my results were not as positive. Thank you for the response.

Hi, I was just reading that you said they obtained about 5ml of your blood. I'm pretty sure that amount is no where near the required. 50cc is the goal! You need a platelet count of at least 10 billion or more.

celebrex doesn't affect thromboxane a2, resulting in sparing platelets.

My orthopedic specialist advised me that PRP may lend up having bacterial infection and that will worsen the matter as one will need operations to remove the bacteria infested knee. Moreover he said if this happens it will worsen the knee condition. He advised not to do PRP but have knee replacement. Please advise doctor

my ortho suggested prp for my shoulder, I have bursitis and 2 small tears. tried cortisone injection but did nothing other than ruin my diabetes control for a while

I had prp injection in January this year with 20ml Hyaluronic Acid 7 day's after the injection i ended up in A&E i was unable to move a leg for 6 day's the Hyaluronic acid caused me to have pseudogout which was very painful.

If you had a patient with both knees with arthritis/ started off as a meniscus injury (over use, jogging) would you encourage that patient, or discourage that patient from having both knees done at the same time? New subscriber, looking into this procedure. Thank you

Wow I wish I knew all the things I should’ve done before getting PRP. My doctor did not let me know of any of that. I had a Tylenol the day before for period cramps. 😪

In theory, Tylenol should be totally fine! You'd want to avoid NSAIDs.

Thank you. You said one might need 3 injections into a joint and 1 into tendons to feel results. What about injections into ligaments and/or muscles?

Thanks for your question, these are all "in general" and of course needs to be individualized. There are less studies for ligaments and muscles. We lump them into the tendon category of most of the time needing just 1.

Thanks for the content , after Prp injection in my knee , it becomes swollen , with pain 12 hours after injection which lasts for three days , also i felt sometimes locking in my knees don’t know why !! May be because of the volume !! Anyway after the fourth day my knee movement returns back to normal without locking . Any explanation for these symptoms !! Is it indication that the injection in right place ?? Can it help for meniscus tear ? Theoretically it has huge amount of growth factors , can this induce cartilage or chondrocyte to regenerate the cartilage ?? Thanks

Hello Mr Peng ,thanks for the informative video.I got a PRP treatment on my knee for osteoarthritis and a week after it swallowed and got very painful no major incident I just used a strong pain relief cream with camphor ,chondrotine and collagen is that could be the reason of the sudden swallow and strong pain ?I m 10 days from the shot now the swallowing is slowly less so is the pain but still very painful can t walk properly ...will this affect the platelets ? have a good day thank you .

This is the first video I've come across that really gets to the questions I have. Thank you for cutting the BS and just getting to the point. One question I do still have though, I have two completely torn ligaments in my left ankle (ATFL and CFL) so my ankle is pretty unstable. I'm running fine on it but I easily roll my ankle on trail runs even with a brace. This happened in Feb 2019. Does the 3.5 year gap matter or would I still be a candidate for this treatment? Thanks!

Unfortunately PRP has not been studied for ligament tears. We do it a lot for UCL problems in the elbow, but results have been mixed. Using PRP for ankle ligament issues would still be considered highly experimental, regardless of when the injury occurred. Best to discuss with your health care provider. Good luck!

@@JeffreyPengMD I'm having a consultation with a specialist regarding PRP or other injections for relief in my elbow where I had a UCL tear a year ago. After the initial tear, 3 months of physical therapy helped me regain use of my arm, and then a few months after that it felt pretty good. But its been degrading a bit the last few months. Are there alternatives to PRP for the UCL for pain relief and recovery?

This video is very helpful but the part about preparing for your prp begs a new question. If we rid ourselves of Tylenol, Motrin and eat tons of fruits and vegetables, would that help osteoarthritis in all our joints overall? Or are some joints just stubborn?

Lifestyle has a HUGE impact on osteoarthritis flare ups. But once you have arthritis, modifying your lifestyle will not regrow cartilage and get you a new knee. PRP can help treat the internal environment of the knee to protect the cartilage that is remaining.

Is arthrofibrosis/ capsulitis a contraindication to PRP? I have post op decreased ankle ROM after ankle surgery and some pinching symptoms. (Primary surgery was debribement of scar tissue and removal of bone spur)

Curcumin has anti-inflammatory properties, but (as far as I'm aware) they do not inhibit platelets and should be fine to take. Thanks for your comment!

After listening to your video Iam going to focus on healthy eating and drinking water plus cardio vascular exercises before I fix an appointment for stem cell therapy and prp. Being said that how long should we do this before having the prp/ stem cell therapy to get ideal or best results ??

There are no studies answering your question "how long to make changes before starting a biologic treatment". With that said, what is certain is that healthy lifestyle changes are things you'll want to continue for the rest of your life!

Thanks for this and other video. Dr Penguin, do you know why insurance is still no covering the cost of PRP injection if there are data showing benefit ? I have knee pain after ACL and menisus operations 25 and 5 years ago. So am scheduling to do PRP injection shortly. Thanks again.

Dr Peng Thank you for a very comprehensive video on PRP. I have my left knee lateral meniscus traumatically torn with a radial crack of 7mm(0.28in) wide and 14mm (0.51in) long. In your opinion, would PRP help regeneration of meniscus tissue?

Thanks for your comment. It's VERY important to understand that as of this point in time, we have no definitive data to support that PRP or stem cell therapy "regenerates" anything. Rather, we think we are changing the biochemistry of the environment and that's what leads to improvements in both pain and function.

I got PRP injected to my shoulder joint in colombia. They used a shorter needle 1/2"- 1" and in kept in smaller tubes... Like 6 total moving around my shoulder. Is this as effective as one large shot?

Is Longvida curcumin 400mg the same pathway as nsaids - will it inhibit platelet counts? Same question for Boswellia extract 350mg? Thanks for any help!

Thanks for all the information. I had my first treatment today, I am on baby aspirin due to a stent/pipeline in the brain I have to take it daily, did I mess up my entire treatment? This Something I totally didn’t think about

I just had this therapy in Jacksonville, FL by the wonderful Dr. John Anderson, a foot and ankle orthopedic doctor. Because UF does a large number of these treatments, the cost was only $500 vs $1500 at other facilities.

Excellent video Doc & you answered most of the questions that I had about PRP. However, I was wondering how exactly these injections are done. If you inject the knee for example, how do you avoid the needle from going into the meniscus rather than the joint space itself? It also seems like you would need a lot more plasma than just the amount that you described in this video. Is the injection limited to the area where the patient is experiencing pain, i.e., medial vs. lateral knee joint? Anyway, thanks in advance for answering these questions! P.S. I live in CT & was wondering how I would go about finding a doctor that does these injections? I doubt that my Orthopod does them as he's old school. Anyway, thanks!

All PRP injections should be done with ultrasound guidance. Don't let anyone do injections on you the old school way which is landmark based. TONS of doctors on the east coast. Usually your best bet is to start with your local academic medical center.

​​@@JeffreyPengMD Here in India, Bengaluru an ortho doctor in a hospital has put up a video advising people to take prp but I doubt if would use ultrasound which anyway he has not mentioned in the video. Is it better to not get PRP from him (and continue on supplements ) ? Or take the risk ? This is for my wife for knee arthitis. Thank you.

Hasn't been specifically studied, but probably! It works for other nerve entrapment problems (carpal tunnel syndrome)

Great question! I will be making a video on this in the future. However, it's important to point out that PRP and hyaluronic acid treats symptoms and does not regenerate anything. If you have severe arthritis, the definitive treatment is a knee replacement surgery. The goal of PRP and HA is to reduce symptoms sufficiently so that you may not need a knee replacement surgery for as long as possible!

I had a PRP injection today and what my orthopedic Dr is saying is contradictory with what is being said here (makes me nervous and worried). I am suppose to take 600mg ibuprofen every 6 hrs for 2 wk and ice for the first couple days and I have restrictions along with a split to be used only to take off 4-6 times to do stretch exercises. He cancelled PT, ultrasound TX and massage - only stretching. So many drs have different opinions and instructions it is worrisome….

Depends on the indication! I've heard of spine docs putting it intra-discal, facet joints, foramina, etc.

In regards to diet. Would a healthy keto diet be good for PRP? It's higher in fats but mostly healthy fats. Plus virtually no sugar.

Thanks for your comment. PRP has not been studied for labral injuries. In general, PRP has not been shown to be beneficial for structural problems (ie. labral tears)

Much more information than my doctor provided! I have shoulder impingement in both shoulders with a minor tear that doesn't require surgery. I have tried 1 cortisone shot in each shoulder which worked but I know they break down the tissue. Trying PT again through a different therapist but if it still doesn't work, I may try PRP. Doctor said I could be in more pain than ever a few weeks later.. What is your opinion re: PRP for impingement?
