Back in 2023 I could not lift my arm up unless I use my other good arm to assist it up. My shoulder was in constant pain and I could not sleep on it at all. I came across your channel about six months ago and started doing these simple exercises especially the first one. Slowly my shoulder got stronger and stronger. Now I can bench press light weights. I still do the first exercise everyday. Even when watching a movie I lay on the floor with a foam mat and do this exercise. I can feel my shoulder being kept in place my these rotator cuff muscles. I'm not sure what was wrong with my shoulder in the first place but the pain was unbearable. Today there is no pain but I do hear a popping sound and some kind of friction at that shoulder joint. I'm hoping that the friction sound will go away. Thank you very much for your teachings. Now I can do resistance weight training again at the age of 64. Sure beats being on the sideline and being inactive.

Amazing story brotha. Quick question...you mentioned light weights. I injured my shoulder by bar benching and it crunched...are you back doing heaving weights? Or are you still doing lights weights? My biggest fear is not benching 315lb ever again

@@winterramos4527 After I had supraspinatus repair surgery, my surgeon said: " you can do anything you want just ease into it". Take it for what it's worth. I got injured because I didn't listen to my body and tried to force it to do what it wasn't ready to do. How important is it to you to be able to bench 315lbs? How important is it to you to not have to go through shoulder surgery? Just some food for thought.

I did a real number on my right shoulder last August. I was in so much pain. Mostly from Deltoid nerve damage. I went for PRP privately. It was such a blessing. The pain level went from 8 to 4 after one treatment. I am now ready to exercise. Maybe swimming too.

That was my first inquiry into what to do about my tear. But, when the PRP doctor looked at my mri, she said it was too severe of a tear. My husband had his thumb done, help him a lot.

How long should you wait after the initial injury before starting these exercises? I appreciate your advice so much . I recommend your channel to friends as well.

A refreshing way to approach rotator cuff impingement. As a Physio I believed that early ROM shoulder hitching was a sign of disordered scapulohumeral rhythm and that we needed to teach proper scapula stabilization before allowing a client to elevate their arm. Having a shoulder injury myself now and experimenting with your method of encouraging shrugging I can see how much it is going to help. I’m feeling hopeful 😅

I just found your videos thank you. I have rotor cuff tear, tendinitis and arthritis in my left shoulder. I am in beginning stages of therapy. I was hurting so bad afterwards I was crying. I can’t sleep the pain so bad. I bought an arm sleeve did what you said it’s already helped. I am starting the pillow exercise and purchasing the pulley. I pray 🙏 I won’t need surgery. My orthopedic doctor appointment is 4/16/2024. I am trying everything I can to be pain free ❤

Katherine...I read your summary and I think we're in the same boat...maybe different scenarios. I injured my shoulder(right) by benching 300lb...I'm almost 3weeks in...I'm much better but it's going up by decimals...in a scale from 1-10...I'm going up by only decimals....so I already know I'm out for a few months .so do NOT rush anything...I've been doing these exercises and pool exercises for 3 days now and I can see a difference...but decimals......say about 16weeks...if you're consistently exercising your shoulder. Please, feel free to ask any questions and I can definitely assist you any way I can PS: I ordered a few things on Amazon that will assist me...

Great advice that makes all kinds of sense. My partial tear so far, produces minimal pain, so can I practice my shrugging with dumbbells? Also, I'm trying to train myself to practice shrugging while doing everyday things like reaching up to get something off the shelf, etc. I have watched the majority of your videos since I discovered your channel and am very grateful for the advice you have given - definitely different from my previous physical therapist from a few months back.

Dr David you most certainly know what you are talking about. In your instructions its like you know what is on a patients mind.Small things like saying it's ok if your range isn't great or saying to take your time to improve.

I am on week three and shoulder movements beginning to slowly come back. Started doing the first exercise. The key I believe is rest and patience in the early stages...super frustrating but should not rush the body's natural healing processes. As a functional movement coach, I am now starting to rehab my own shoulder and doing scapula retraining.

I have a complete tear and want to avoid surgery after reading people have re-injured their shoulder again after their long rehab and weeks of wearing a splint. I see the Doc on 4/4/24 for his input, but I ordered the splint you recommended to support the shoulder and will start doing the exercises you recommend. Your the first person who makes sense to me as I’ve gone for PT and they had me doing all the wrong exercises and then it irritated the shoulder even more. I went for PT for weeks and once I quit because of the pain it settled down again and felt better. I fell outside on 11/30 and the pain came back and I finally got the MRI which shows a complete supraspinatus tendon tear with tendon retraction to the medial numeral head. I am able to rotate my arm above and behind my back so I am lucky. Thanks very much for your videos and helping people. Time will tell, but if I can avoid surgery I will. Live watching the videos and info that you give.

Two months of rather bad pain in my shoulder, tried lots of ‘band’ type exercises which did not help! Used the first excise here and a suddenly no pain, AMAZING! Thank you so much! I will continue to build on my progress, so far!

I am so grateful for Dr.David. A knee caused me problems around a year and a half ago, which may have been tibial shift. I followed his advice and all is well now. 👍I have what is probably some kind of impingement, not necessarily a tear, but I am doing these and Dr.Jo's top ten as well. Just started today and it feels better already. Pretty sore, limited ROM the last couple of days, and hard to get comfortable in bed so I am motivated. Thanks!

I have had 2 rounds of PRP and 1 Stem cell injections. I have 2 tears in the Supra (85% & 15%), labrum and new tear in the infra. I also have impingement and bursitis. I didn't type them out all of the way, sorry. I have done 6 weeks of PT and I have improved slightly. I think I have 4 more weeks of rehab but I want to add your exercises. They tried to add bands and rotating my hand out to the side. That was a hot NO! I have saved your videos. Thank you

Could not continue with surgery for a reverse rotator cuff: since my 77 year old Heart (baby heart never grew, back side) rest normal; heart failure and surgery in 2022. But Wednesday 12-18-2024 surgery for reverse was cancelled. Because blood pressure reach 255/91 and they cancelled it. Said I probably would never have another surgery done because of stress on my Heart. To delicate to stress it out and all my medications and cleaner allergies. So need to learn to use arm without surgery to make life easier for me and my husband of 56 years. Thank you

Excellent, tried this protocol, except exercise number 4 with the pulley, don't have one. Beginning to believe that I had mostly some sort of overuse syndrome. This problem started while doing a variety of many things within a 2-3 day period that ended last Monday coupled with snow shoveling, weight training, home made bread kneading for the prior week leading to an inability to raise my right arm overhead without assistance. Now, I can raise it without a problem, still some discomfort though. Wondering if all that condensed amount of activity led a pinching of a nerve in posterior rotator cuff area.

CONCLUSION: 11/29/2023 1. Frayed deep near full thickness 1.6cm tear supraspinatus interstitium and bursal surface. Mild bursitis. 2. Chronic Hill-Sachs lesion. Scarred anterior inferior labroligament complex. No acute macroinstability. 3. AC arthropathy. Pseudocysts and marrow reaction distal clavicle and acromion related to overuse. Low lying acromion. Lateral arch narrowing. I have had 3 steroid injections in the last year that lasted about 3 months. About to get the fourth because the pain is coming back. I should have been doing these exercises before but am very serious now. I lift weights 4 times a week and I don't want to stop. I have full motion on my right shoulder and can perform all exercises without a problem. I have heard that the injections prevent the tendons from healing and I'm worried about too many steroids and not healing properly. Now after 4 months from the last shot, that dull pain is back, especially when sleeping and after working out. Any suggestions.

that sounds like a large tear. I got an MRI and have the same type large tear. i have only a few tendons left holding my cuff in place. i waited too long on my first shoulder problems and it got bad. had surgery and its working well. recovery was hell. I'm not waiting too long this time. getting surgery ASAP and will start these exercises on both shoulders and hope i never get large tears again. these exercises are already making my shoulder feel better but it could take years for cuff attaching tendons to regrow, if that even happens. i don't have years to wait for that possibility.

Unfortunately both of my shoulders are very painful and it makes it impossible to do these but they are the best I have seen for people with only one shoulder affected.

I have a unique problem--I have been bedridden due to severe osteoporosis for more than 3 years. During a diaper change several months ago, as I rolled to my left, I heard a "ripping sound" and felt intense pain. Being totally confined to bed, I can't see an orthopedist or get an MRI, but I know it's a RCT as I've had one a number of years ago. I also had a reverse shoulder replacement on the other side. I guess the only exercise I can do is your first one, but with difficulty as I'm in a hospital bed! Since the bars of the bed won't allow me to fully extend my arm, is it okay to either reach on top of the bar, or do the motion up and down on the mattress rather than to the side? Thanks so much--you're terrific and you explain things in a clear and soothing manner. 😊

I do indeed have issues with my rotator cuff and am excited to start these. I do lift weights and know that it will most likely take longer to work than normal. I just hope it doesn't get worse. I have a question. Below my left shoulder blade I get popping like bubble wrap when I do heavy curls or fly curls. There is no pain but I am worried. Is it possible that these exercises could help with the popping that I get when I do these exercises?

Hi doc . I’m 73 very healthy are was had a stint put in September. Right after that right shoulder went crazy with pain, mri showed complete tear supraspnatus. Excuse spelling. Can it heal 100% with therapy

Thank you so much man.. I was miserable for 2 months now.. This exercises seem to work.. I'll keep doing them. Only I wonder if I can do the first one standing up back against the wall so I can to it while at work. I can not lie down in my office due to circumstaces at work.

Problem is I can raise my arm but I can't lower it without pain. The first exercise is gold❣I only tried a few just to see if it would hurt. Afterwards I raised my arm and lowered it with less shoulder pain. It worked so i'll keep doing this one. I'll try the others too. And you're right, my pt said the exact same thing while using that pulley😆 Thank you Dr. David, your videos are a blessing because nothing seemed to help me.

There was an lvl beam. 14 1/2 inches by 28 feet and the beam was 4 members deep. There was two guys. I’m right handed and was able to get one side up in the pocket and I went to go help with the other side. It was hasty on account that the other guy was bringing up the other side and it was a lot of weight so we were being fast. I put the ladder as close to where the action was as possible (the other guy was there too) and went up the ladder, this time with my left side. We got the beam in and I noticed something different in my left shoulder. It felt “dropped” Now when I squeeze my inner elbow “ball” it clicks in my shoulder. I have spontaneous pain and loss of strength in my left arm.

I have been informed that I have 3 tears. I chose to have an injection 2 months ago which did help but the pain is starting to come back now. It’s my right shoulder and unfortunately I’m right handed. I really dread the idea of having surgery and possibly being in a sling. I’m also dealing with a bad left knee. Just had a different injection done instead of the cortisone injection in my knee yesterday. I’m getting frustrated 😩

Do you recommend similar exercises for a dislocated shoulder from an injury? I had a mountain bike crash a week ago. My friend tugged on my arm and it popped right back in at the time. It was out of position for about two minutes I guess. A week later I visited an orthopedic surgeon. He took x-rays. Nothing was broken, but he recommends an MRI and surgery if a tendon is torn. I’m tempted to just try the natural approach first rather than rush right into an expensive MRI and surgery. I can’t lift my arm above handshake level unless I use my other arm to do it. I have full range of motion though and no popping or clicking. It’s just too painful to raise my arm on its own.

I haven't been to the orthopod post MRI yet but highlights from the MRI report say: 1. Minimally impacted greater tuberosity fracture, 2. Small tear/strain involving the anterior supraspinatus footprint adjacent fracture 3. Inferior labral tear with minimal marrow edema. The shoulder may have been dislocated inferiorly. The first few exercises in this video do make it feel better. Thanks so much.

I have a partial tear but the pain is in my upper arm, no pain in the shoulder at all. I first felt a ripping pain in my arm, three times total. I thought it was a torn ligament but the MRI showed the rotator cuff was torn. Would surgery alleviate the pain my arm. I am unable to use my arm in certain direction without experiencing excruciating pain. Why does my arm hurt and not my shoulder?

I had the same thing with pain down the back of my arm. My tear was less than 1/2 inch. Any more then that and my specialist said surgery would be needed. A bit of bursitis showed up on my scan and that's what was causing the pain. I got a steriod (Cortisol) injection and week later the pain was gone, but still had to do the rotator cuff rehab post that.

Hi Michelle, It allows for easier mobility. Otherwise, it can create discomfort in the shoulder and neck area when performing the movement.

MRI states a full thickness rotator cuff tear. I can reach overhead but when I lower my arm it tends to flop. Does this mean I need surgery

Thank you for the helpful information! Just one day of doing the first exercise hourly has made a huge difference. I ordered an over-the-door pulley system to try tomorrow. The second exercise doesn't work for me due to balancing issues, but I did try the shrugging exercise from another video and felt muscles I haven't used in a while. This is great since I've had shoulder and neck problems.

Thanks very much. I have Rotator cuff tear. I can do first one exercise but second one not possible because the weight of my upper body when i do angry cat position hurts so much. And Back of my upper shoulder hurts so much also front of my chest right hand side is hurts as-well. Also when i try to stand up from seat or siting down hurts as well. But lifting my arms straight up is ok and not hurts. Any recommendation much appreciate. But i will try do exercise in raw.

If I dont have the pulley can I replace this with a light dumbbell shrug?

Hi Miguel, you can but it's not going to offer the same range of motion as the pulley. The opposite hand of your rotator cuff tear provides more help but assists the shoulder to get more movement. It will work as long as you are shrugging.

Ok so I started doing three yesterday. Today my arm is ON FIRE! can't say three won't work but my cuff tear is pretty severe. It happened on January 3rd from a fall it's taken all these months to calm down enough when I would even try these. Man, I had such high hopes😢

Injured myself doing "cable overhead press" (cable and handle were much further out than shoulder width) due to keeping my elbows flaired out (you could draw just 1 straight line from elbow to neck to the other elbow). Went through a few sets, no pain or discomfort, but later that day a bit of pain on my left arm only. Then the next day I woke up and excruciating sharp pain only on my left shoulder...rotator cuff. Could barely move in any direction. This happened May 2024 and the pain reduced alot by July and alot of movement came back(90%) but when lifting my arm a sharp pain occurs in that last 10%of upward movement if my arm is turned outward(outward meaning if you look down at your own arm like its a tube, you rotate it counterclockwise). Since July it has not improved at all, that sharp pain is still there and I have that 10% limitation still😢

Hi Doc, I hope you'd answer this please, I got a full thickness tear of the Subscapularis with RETRACTED fibers. could that ever heal without a surgery?? again the tendon is fully RETRACTED in the anterior aspect of the glenohumeral joint. which brings me to the wearing sling point that you talked about a lot where the tendon ages would reconnect .. in my case the tendon is fully retracted. so do you still think the sling could help healing that full tear case that im having??? I trully hope to hear your answers about this please. Thanks!

Hi ajsupleo, That click/pop sound is very common when you have a rotator cuff issue. It is called crepitus and It'should be ok but it's accompanied by pain, please stop the exercise.

I tried moving my shoulder lying down, shoulder on pillow and sitting with my elbow on arm rest, moving or rolling my arms forward to backwards, I feel a little pain in the joint. It didn’t feel smooth and ez to move. Don’t know what I’m doing wrong. Is this a wrong exercise for me?

My rotator cuff got injured 3years ago, and never got proper help for this. My shoulder is killing me. Would physiotherapy be of any help now? I'm seeing an orthopedic surgeon Tuesday.

I have dislocated my shoulder 2 time at a time I relocate it by my self,then the other day I tried it to relax and put my arm in a chair badly I dislocated my shoulder again and I relocate with myself again😢 I rest my in 1 week without sling. I can do some movement but limit it in the other move😢 do you think I have rotator cuff tear because of my shoulder dislocated?

I appreciate your excellent videos.. I am a little bit confused with the shoulder shrugging. I understand that you are saying that that puts the ball and socket joint of the shoulder in a good position but are we talked to drop our shoulders down and away from our ears to elongate that area what is the reasoning for doing the shrugging for shoulder pain thank you in advance for your kind answer
