Here is a demo of the issue on our trailer with the UFP Model A-160 Hydraulic Surge Brake Actuator/Coupler.
To really showcase the issue we didn't hook the lighting of the trailer up at all, but this problem frequently happens even with all the proper lighting connections. From what we've gathered it has something to do with the surge actuator not getting a 'all clear' signal from the vehicle to not activate the surge brake system.
If you have any bad connections, wrong lighting adapter, faulting solenoid, or anything else random then this will cause the brake lockout in reverse when you jolt the coupler.
On smaller trailers they have surge brake lockout keys but on this larger trailer coupler we basically had to invent one since the design is different.
