There must be strict policies to avoid such kind of incidents. The guard and LPs should be well connected through high performance comminating devices like satelite phones. This is clearly a communication gap between both of them where the LP didn't wait for the Guard's hand signal to proceed further.

Hand brake of SLR should apply first and there should communication train crew and start only after all right signal by guard
