I received Prolotherapy for my Sacroiliac Joints over ten years ago. I did those for a year to year and half. I cannot recall how many weeks between injections. That worked until about from ce years ago when I slipped and reinjured my SI joints. I have been having PRP sonce last year. I have one or two injections left. Those have been working, about 50% less pain and night pain is mostly gone. But my VA pain doctor said she cannot offer CMC joint PRP. My VA Ortho offered to remove my CMC joints. That's my only choice as a Veteran right now. However, if it was offered, I eould do it in a heartbeat. I would also have it for my large toe joints if I could. Nothing but orthopedic shoes are recommended. My pediatric doctor said people cannot walk without a big toe so they do not remove them.

PRP and prolotherapy are both great treatments for thumb CMC arthritis. They are an effective non-surgical alternative to CMC arthritis. PRP is also a very effective treatment for SI joint pain and dysfunction.

I have a question about prp for tfcc. Isn't it dangerous if a needle is inserted into the TFCC area (Triangular Fibrocartilage Complex), and a small hole is created in the TFCC? As far as I know, cartilage cannot regenerate, so if a hole💉 is created in the TFCC area, wouldn't that make it even worse?

I tried twice this but unfortunately it did not work for my basal joint arthritis and finally had surgery on both thumbs over the last year. It must work for some.

PRP is a great treatment for thumb CMC arthritis, but it is not perfect. Some patients do not respond. The quality of the PRP also makes a difference with response. We concentrate our PRP to 8-10X for the thumb joint injections. I am interested in the limits of the prp treatment for a very localized and superficial nerve issue on my upper back. A small nerve was damaged in melanoma surgery. The damaged nerve is not visible on ultrasound but the spot is very specific and identifiable and constantly painful. I have had hydrodissection of the area with saline with no results. Is it likely that doing it in the specific area with prp would be more productive. I live in Nevada but can come to Florida.

Nerve hydrodissection with PRP can be effective at treating all types of nerve injury. Even injecting it locally in the area of damage can provide benefit. Salilne hydrodissection would not work unless you see the nerve constriction and specifically dissect it in that area. Is it true that numbing the injection location will reduce the prp effectiveness?

PRP injection of the thumb joint is an effective treatment of arthritis and joint instability, and this does not have to be painful. There is some concern that lidocaine is toxic to stem cells, not so much to the platelets, but it is still best to numb with ropivacaine, as this is a good anesthetic and is not toxic to the cells nor the cartilage. The nerve block also helps decrease the pain and makes the procedure more effective. With that said, no, numbing will not reduce effectiveness of the PRP.
