Two Dogs Unable to Walk But Very Different Localizations || Lower vs. Upper Motor Neuron Lesions

Dr. Wong evaluated two dogs with the same presenting complaint: sudden inability to walk in all four limbs. While their presenting problems were the same, their examinations had subtle but very important differences.
Acute onset of inability to walk in all four limbs can be causes by a problem affecting the spinal cord of the neck (called the C1-5, or cervical spinal cord) or by a problem affecting the nerves and/or muscles. These are called upper motor neuron (UMN) tetraparesis and lower motor neuron (LMN) tetraparesis.
It is imperative to tell the difference early, as it changes the list of possible causes, the diagnostic and treatment plan and the prognosis.
The distinction is made via the neurological examination. Dogs with LMN/nerve and muscle disease have decreased to absent spinal reflexes (withdrawal and patellar reflexes), while dogs with UMN/spinal cord disease have intact to increased reflexes. Dogs with LMN disease have decreased tone while dogs with UMN disease have increased tone.
The possible causes of LMN tetraparesis include: tick paralysis, botulism, fulminant myasthenia gravis, polyradiculoneuritis (coonhound paralysis), polymyositis (inflammation of the muscles) or less common causes such as coral snake envenomation.
Causes of UMN tetraparesis include slipped disks (IVDD), spinal tumors, fibrocartilagenous embolism (FCE), acute non-compressive nucleus pulposus extrusion (ANNPE), infection or inflammation of the spinal cord.
Tests for LMN disease may include blood tests, tick search, chest and belly X-rays (radiographs) spinal tap, electrodiagnostics, and possible muscle and nerve biopsy.
Tests for UMN disease include radiographs of the neck region, MRI of the neck and possible tap.
Prognosis depends on the underlying cause.

My dog had a two ticks about 3 weeks before he started to show signs of MG. I didn’t think about it at the time but now after 2 months of dealing with the disease and secondary ones such as ME he is showing signs of improving from taking his PB. The strangest thing is that when he started to show signs, they were in the rear legs and were slow coming. He started to get a little lame in the rear when walking and not walking as fast then over the coming days this become more and more till his back Ned wouldn’t hold him up at all. His front end then followed. Could this still be due to the tick bites?

Been to 12 facilities three neurosurgeons one university And they had no answers other than vague neurological deficits. Having to go over the reports and imaging myself I came across much much more than what they would tell me. Luckily dog hand a major growth spurt along the way In neurological deficits dissolved but the inability to walk remained. Through self-discovery in the MRI I found a rarely acknowledged injury due to low survival rate and/or conservative recovery outcomes. I could not put him down as he doesn't show pain that calls for it. He's just a baby stunted at 4 months now 10 months old through a 3-month failure process and in $12,000 debt. Lateral recumbency is doing him in. Please if you could help me with some pointers on how to get him sternal? He has a left thyrohyoid horn fracture but I can't find a specialist. The time has passed for when he was healthy enough for surgery and now I have to get him a healthy enough. What are the requirements health-wise to deem surgery safe? They keep slapping me in the face saying he'll do the surgery if he can walk and I say he would be able to walk if he could hold his head due to the barb like highlight bone interacting with his neck. I've archived every single report, test result, and image into a convenient share folder. This this pup is one for the history books...

The first dog does not have a spinal cord problem. It's the lower motor neurons (all of them - to the front legs and the back legs) that are affected. The second dog has a cervical spinal cord problem (C1-5). A dog with a problem affecting the L4-S3 spinal cord would be normal in the front (thoracic) limbs and LMN to the pelvic (rear) limbs.

Treatment depends on the underlying cause. That is what the video is trying to show: that many different things can look similar. Possible causes for the dog on the left include botulism, tick paralysis, polyradiculoneuritis and others (which are all typically treatable). Possible causes for the dog on the right include a slipped disk, spinal cord stroke (both treatable), meningitis or myelitis (manageable but typically not cured), and tumors. Some tumors can be treated while others are more difficult to do much about.

@@SEVNeurology thnks alot for replying me so fast. My dog had head twiching since she was 6 month..after that she ws active nd perfectly fine. Bt from last few days her bck legs not properly working. After treatment she was improving bt 3 days bck she got high fever and after dat she is not walking cnt stand on her legs. Vet told me that neurological problem has spread in her body. Nd there is no treatment for that. Please help me
