Very helpful video - Thank you! I'm an active, otherwise healthy, 71 year old male who was diagnosed with prostate cancer 2 months ago. My robotic radical prostatectomy is scheduled next week and I'm hoping for the best. My PSA of 6.6 was identified in my annual physical, I was referred to a urologist/oncologist who did DRE and said he thought I was clear but he recommended a biopsy just in case. My biopsy came back with 4 cores showing cancer, some having Gleason scores of 7 and 8, and some perineural invasion. This was the "dark day" Garvey describes in his experience. I give these details to say that modest PSA elevation coupled with a clear DRE is not the time to stop checking. After bone, abdomen/pelvis, and chest scans, it looks like the cancer has not spread but I won't know until the pathology report after the prostate removal - I'm praying... I was verging toward being more transparent about my diagnosis as a way to encourage other men to pursue regular prevention strategies; now I'm totally committed.

I had no symptoms and was and still am fit. Had the test and had cancer . Had it removed an doing great. Saved my life

Here are my prostate tests. I’m now 67 years old. PSA Testing 2008------------2.1 2011------------4.1 2013------------7.1 2017------------13.1 In 2011 when it was 4.1 I was supposed to get a biopsy, however I decided not to go fir the test. It’s now 2019 and I still have not had a biopsy. I consider myself a pretty healthy person with no other medical issues. I play golf 4 to 5 times per week. I also play tennis sometimes. Up till now I have had no lower urinary tract problems that affect my quality of life. I realize that I could have PC but so far there is no sign or symptom of it. I know many would say I’m rolling the dice but that’s the decision I’ve made. I keep wondering what my quality of life would have been like had I gone in for that initial biopsy. With an increasing psa I’m pretty sure I would have had repeated biopsies with all its associated risks and several courses of prophylactic antibiotics. I am not suggesting that anyone should follow what I’m doing. I think that every person is entitled to make their own decision as to how to proceed. I may yet have PC and regret my decisions but as of now I do not plan on having any biopsies. My life and health is exactly where I want it to be at 67.

Joe J no I have not and do not plan any in the near or medium future. My health status remains unchanged Since my initial comments. I try to have a balanced diet and exercise regularly. At 5 10 my weight is 170 and has never fluctuated by more than 5 to 7 lbs. l make a smoothie daily comprised of spinach, broccoli, carrots, 100% almond milk, garlic, ginger and an assortment of berries. I'm 68 and my PC Phycisian is pressurring me to get a biopsy because my PSA is up to 6.6 from 4.0 at last years physical. This whole thing is mind blowing. I may tell him to give me 2 or 3 more PSA readings prior to agreeing to have a biopsy. Thanks again.

Hi Joe I like you don’t like the risks involved when having biopsy, bleeding weeks after etc.Iv just turned 60 so iv decided with my doctor to monitor the levels. It’s 8 at moment so just taking certain fruits, greens. Vitamins etc to keep it low. Apart from this I am very healthy thanks. So yer just keeping an eye on it.take care.

My PSA was 9:two months later it was 8. When I couldn’t pee I went to hospital and they relieved me with a catheter, they wanted to keep it in I said no and have been peeing ever since although they gave me Tamsulosin I stopped taking it two months later and no problems. The Urologist wanted to do a biopsy when my PSA was 9 I asked for another test and it was 8 so he said let’s monitor it and see if it falls . I’m 69 and have an enlarged prostate which I think is the reason why it was 9 I also have high blood pressure and take Perindopril

Dr. Litwin thank you for your presention. I am a 70 year old patient. Please speak to what a normal Gleason scale may be? Is a Gleason scale of 6 plus scale Cancer? Do you have any experience with Tomotherapy treatment?

Well I for one appreciate this video. I'm a 64 yr old white male PSA LEVEL 8.3 and my doctor is on top of it....have already had the culture, MRI, EKG, and am scheduled next week for the Biopsy. So I guess we will see. Truthfully I'm a bit nervous about everything. Thanks to both of you for this, I feel that I am DEFINITELY more educated about it now.

My husband is 66 and will be undergoing a biopsy in a weeks time. He first started of with having PSA levels of 14 and symptoms a year ago. He had a scan at that time that revealed an enlarged prostate and no cancer was seen on the scan. It was probably 2 or 3 months when he started having problems with waking at night to pee and was put on medication for this. We are now a year on and he started having other symptoms of erectile disorder and no ejaculate. The GP was not interested and just left it. Now his PSA levels are 28 and he had his first rectal examination. The Urologist has said the prostate feels very irregular and suspects cancer. He has a biopsy next week. I am terrified that he has cancer and that it has spread because of all of these other symptoms.

I’ve just noticed when I feel to urine there is a slight delay before it comes out. After it comes out it is not as forceful coming out as I’m used to it coming out. I don’t know how long this has been going but I’ve recently noticed it markedly as of late. My grandfather died of prostate cancer & my father is taking “flomax” now going on 12 years now. He is 74 now. I am 51 currently. I have never been tested or have had my prostate checked. Making an appointment to see a urologist this week. This video has been helpful.

I finally decided to get another psa about a month ago. I just turned 69 in June. I know people will freak out about this number but I’m still not getting a biopsy at this time. My psa is now 28.. From 4 in 2011 to 28 now. What has not changed is anything I can discern healthwise.. I still play golf and tennis 4 to 5 times per week. I have no difficult or painful urination or no obstructive symptoms that I can discern and no sensation of residual urine in my bladder. At 69 I do have some increased frequency which is not a big bother for me. I do have high blood pressure but it’s now down in the normal range with my medication. As I mentioned I don’t plan on getting a biopsy. All of my tests results are in the normal range except for the psa and I still weigh 170lbs. I know all about the increased risks for African Americans but apart from a slight increase in frequency I have no other signs or symptoms that’s affecting my quality of life. I continue to exercise regularly and eat a healthy diet.

Have you considered a 3T Multiparametric Prostate MRI? It's about 25 minutes and minus a contrast needle in the arm, it's not evasive. It is not as sensitive with lower grade detections of Gleason 6 cancers however if you did had low grade, it would be even less of a concern overall. I do understand for some not knowing is better than knowing.

Recently, I had a prostate needle biopsy done. My urologist will be meeting with me soon to discuss the results. There is no history of prostate cancer in my family, and I am not an African American. I would like to add that my P.S.A. prior to the prostate needle biopsy was 11.

My husband just went off his food, and did not want to work in his garden,very unusual as he spends 5 to 6 hours in his garden, went to the doctors 3 times in a week, then they gave him a blood test the next day the doctor phoned to say that his p s a was very high.had no trouble peeping, the oncologists said with the symptoms that he had he would have never give him a psa test. But thank God every day that he did my husband is 73

Not until the forty-first minute of this forty-seven minute video did either Garvey or Litwin address the issues of surgical side effects - even then, little information was provided - which leads me to wonder; was this video created as the public service they purport, or was it merely a somewhat informative infomercial for the UCLA Department of Urology?

a Person I love very much has a 62 PSA is 61.1 with a % PSA of 9.1 Three years ago his PSA was 2.4 Does this not seem high and within a short time? Trying to get insurance to approve his Fusion guided Biopsy. His Dr. did a DRE and said it was Hard. Not large but Hard Should I be very concerned about him?

You make the biopsy sound so grand! Could you have one and let us know your opinion on a deeper level? Are there risks? How do you navigate or is it just a random thing? Is a 'few' 12 hollow needle punctures through the penis, taint or anus? What is the failure rate? Are the odds greater than a coin toss?

I’m 77 and had my PSA test done yesterday and was informed that it was 20 (has my Prostate drained & again in 2 weeks. Does it mean I have Cancer or (hopefully) only a prostate infection?

Dr who analyzed the PSA is the only one to tell you. Prostate Cancer is closely related To enlarged prostate and prostatitis. Also can be elevated from sex and bicycle riding with those hard seats. Don't worry. Put it all in God's hands. Your strength and Salvation

So if you are getting your yearly PSA and one comes back elevated from the previous years, your primary will have you retake another PSA approximately eight weeks later. A PSA of 4.0 or lower is considered normal but it is age driven. Men under 60 are expected to have even lower PSA lower than 4.0. The doctor will also perform a Digital Recital Exam to feel for lumps on the prostate although many are moving away from this at it has poor ability for detecting early stages but it can detect lumps if on the outer part and is also useful in detecting prostate enlargement. So after two confirmed elevated PSA's, the mostly standard US medical protocol is to do a random ultrasound guided biopsy which usually consist of 12 cores give or take however In the past few years there has been a slight shift towards performing a 3T Multi Parametric Prostate MRI with contrast before a random biopsy so if anything is found, the MRI mapping can be placed on the ultrasound for targeted biopsy for certain areas however they will still include random cores. My Urologist initially wanted to go straight to biopsy due to insurances not paying for a MRI most times according to him. I said I wanted the MRI first and would pay for it out of my own pocket if insurance did not. I was hopping the 3T MP MRI would negate the need to next move to biopsy however I've come to learn MRI is not as sensitive with lower grade detections. My biopsy did find Gleason 6 in two out of 12 cores and I am a candidate for active surveillance but at 55 I am still trying to decide if I want to just get treatment over with or wait until a later time with AS.

Just an update. I did decide to go with a Robotic radical prostatectomy. My surgeon said I was a prime candidate for active surveillance but said he would support my decision what ever I decided. I had surgery on October 25th and the pathology report came back a little over 2 weeks later. My doctor said it was very low grade and is confident of full cure. I read the pathology report further and it said 1% of prostate which made me wonder if my decision to treat such a low grade low volume Gleason 6 was wise but then I remembered the prime focus on cancer cure and that's to catch and address it early.

Any side effects immediately (catheter) after the surgery or down the road (impotence, urination, etc)? My understanding is that radiation and Prostate removal have similarly excellent cure rates for the situation you describe. So many folks tend to go with side effects. Which is why I I asked.

If this guys is an expert then he should give you the correct stats. If you have a PSA of 4 or below your chance of not having prostate cancer is about 80%. However, that same score has a false negative value of 15% (in other words, you are led to believe you don't have prostate cancer but you actually do!). Likewise, if you have a PSA between 4 and 10 then you have a 75% chance of NOT having prostate cancer. That being said, if you are one of the unlucky 25% the fact is that you have a 97% chance of NOT having an aggressive cancer that will kill you. So the PSA test is meaningless for the average man. However, it has helped to pay for a doctor's new house or bass boat as it typically brings people into the urologists office for a biopsy and sometimes radical treatments which can ruin a man's life. In short, the PSA test for prostate cancer screening is a scam for most men other than perhaps African-Americans or those with a family history.

There's a lot of truth there , I have been getting psa tests since 2008 where there was a slight cancer reading from a biopsy . I've had psa tests every year since averaging 9 , now it's 11.5 , but nothing has shown up on scans ,and my uroligist wants me to get a biopsy, I'm 71 years of age and I'm more worried about overtreatment as much as anything else . It's as if he wants to find something , and if there's anything at all , he's not going to take a chance not because I need it , but because he can say he's done his job .

I am 73 years old. My MRI test reveals there are PIRADS -2 nodules in the transitional zone. Prostate is enlarged, Size 46x45x43 mm - volume 44 g. ph 6.0 value observed. PSA Total is 16.57* After one month test result shows - psa total 10.57* Free psa 1.900 Percentage 18% Is this suspect prostate cancer???

my name is Omar and I wanted to know um the real symptoms of prostate cancer.I been suffering from uncomfortable-ness down there below my testicles ,between the testicles and the sphincter.sensitive, Pee dribbles and very intense ejaculation.I got PSA done came back negative.had my prostate exam check twice again negative! and also got it early colonoscopy came back negative.I'm a little confused right now I'm going to see the urologist March 18 talking so I could get an ultrasound on me.constantly going to the bathroom dribbles uncomfortable just ruining my everyday shores.I feel miserable! missing the old me I'm only 37 going on thirty eight.my great grand father or grandfather died of prostate cancer so that's why it worries me.please help me.and thank you for your time God bless you.

If the Prostate produces PSA proteins.... and a patient has opted to have the prostate removed, why then, are PSA tests monitored AFTER the Prostate is gone? How could there be PSA proteins to measure, if the prostate is no longer there to secrete them into the blood stream?

Because malignant prostate cells sometimes get into the blood before or during surgery and spread around the body and make tumors later on. These cells produce PSA just like the prostate itself does.

The psa test is actually a test for fungus. Go on the Doug Kauffman phase 1 diet and I guarantee your psa if high will drop.

Does anyone who gets a prostatectomy (prostate removal) ever regain control of their bladder or is it a life sentence with a catheter?

Definitely no life sentence involving a catheter. Not saying that that can’t happen, but the “lifetime” part is rare. Typical for a prostate removal is a few weeks of catheter time.

Get checked right away. Unfortunately I didn't, now I'm Stage IV with Bone Mets. No cure. I tried ADT, it only worked for a short while. I tried chemo, but had a terrible reaction to it. Talking to my Oncologist this week about some new options.

How does taking 6 chunks out of my prostate.....do anything? Especially if the only reason is an elevated psa. I feel I’m going to be a lab rat for the VA. It does little except prove you have it. Cutting into cancer makes it spread. I have little faith in VA....gotta feeling there going to make this worse......not better. And drag this out over months of tests and appointments. All the while......I’m highly stressed because of a lack of information. There is no bedside manner,.......just platitudes! Well we don’t know for sure.....let’s run more tests. It’s one sided and you feel quite helpless. Oh and your doctor......he’s been replaced. Now I’m being treated by a women....I’ve seen twice....for about 15 minutes. I’m 54.

I think I have prostate cancer. Feel terrible..debilitating fatigue all the time...weak, back and hip pain, poor urinry function. Im in big trouble, and this was all caused by biking. Sitting on a bycicle seat..

A study should be made on that but my urologist told me that he rides a bike and he's not going to tell me not to ride one. It. can elevate your PSA. Prostatitis. can make you feel sick to. Get checked out is my advice

I recently had a psa level of 6 from a previous 3 n im 59. I’m scheduling a MRI over the dreaded needle biopsy. Simply not interested. Changed my entire diet - ZERO FKN SUGAR! I’m doing my best to mitigate cancer IF it’s there at all. Hoping it’s just BPH.

How are these guys not familiar with the work done by Dean Ornish in reversing indolent prostate cancer through diet? Prostate cancer is virtually non-existent in low meat/diary eating countries. South-central Asia has rates below 10 per 100,000.

HI, I am 55 and my engine needs re checking, After a complete prostate removal surgery in 2020, I was able to maintain a PSA of 0.1 but in 6 months it climbed up to 1.4 without symptoms, I am getting a PET scan at the end of the month to find out where it has grown. Looking for support groups (nonreligious if possible) to get and share infornation in recovery and survival tips. Thank you and a prosperous new year 2023

Waking up once or twice is common for men in their 60s and older. Waking up 4 or 5 times is cause for further investigation

That is it, I have other health issues and I am not interested in being screened for PC. I don't want to be caught in that web of trying to convince my doctor that I don't have PC. More than likely I have BPH. I have Inflammatory Bowel disease so needless to say I don't want to risk a biopsy. In people like me it could cause a fistula or sepsis due to being on immune suppressants. even though my doctor knows this he is insisting on PSA testing for me. I DONT WANT IT. I need to find a new doctor. Oh, I don't have any family members that have had PC, so I am in low risk. I am 48 years old and that is the only reason I can think of that my doctor wants to screen me.

A PSA is a simple blood test. If your PSA is elevated, a doctor may suggest taking antibiotics, if he feels it could be an infection or he could order a biopsy. Your biopsy will tell the whole story. Trust me, I had no signs or symptoms other than a PSA score of 6 and come to find out, 5 of the 12 biopsy's tested positive for cancer. Please, do not try and be your own doctor.

JH586 The VA try to allow a physician assistant to perform this procedure on me. Instead allowing this to happen, I did some more research and founded there's a test called 4K Score which helps to detect cancer in those with high PSA levels.

Jeff Ntambara ABOUT A YEAR AGO I DAMAGED MY JOINT IN MY RIGHT ELBOW ARM WRESTLING. THE PAIN FOR WEEKS WAS SO BAD I HAD THE TAKE ADVIL IN ORDER TO USE THE ARM, I THOUGHT THE JOINT WAS PERMANENTLY DAMAGED. Doctors are experts at seeming to care. But are more distant and less responsible then ever. 15 minutes of looking concerned......then it’s out the door! Rats! Never wrong! Never around! Overpaid and under worked. Pull any muscles? Is your back sore from your labor? Do you get dirty? No of course not.......just full of advice. At $300 an hour.

Yes and because of this through my research I placed a lot of emphases on 3T Multiparametric Prostate MRI. It's actually the "Gold Standard" in Europe for skipping a biopsy if nothing is detected but not in the US where random 12 core biopsy is the norm. My MRI report even has a small disclaimer stating "3T MP MRI is not sensitive in detecting low Gleason grades"

That's probably why it's a wait and see approach without significant changes in PSA. Many prostate problems that occur in older men like waking up to pee once or twice or needing Viagra. Any kind of burning urine can be cause for concern. Prostatitis is a common cause and effects younger guys as well. Older guys get that too but it sometimes just means enlarged prostate which although it's not fatal can have its own set of problems.

you have nerves in there that you do not want them to mess with. and they do not tell you they easily can destroy your life.remember god put that in a place so no one would mess with it.look at biopsies are dangerous on the web and you should look at all options
