@11:40 you mentioned urine chloride to be high in Conns and Cushing but @ 15:00 you say Urine chloride is low. Which one is right with explanation please. Thank you!

i think this is specific for metabolic alkalosis and saline responsivness, for example in the setting of Conns, You may have increased aldosterone secretion of H+ ions in the Tubules, this will result in more CL- used to buffer in the urine and the urinary cl > 20 will result in a nonresponse if you adminster NACL to correct the alkalosis. because the problem is not loss of urinary chloride or (Volume) it is excessive secretion of H+.

But in general, Urinary Chloride in Conns will be low becaue we are absorbing more salt and chloride follows salt. in this situation a different mechanism is responsible for chloride wasting which is buffer system and not salt following system if that makes sense.

@10:19 how is Na loss (diuretics) causes low urine chloride? Shouldnt it be the opposite... chloride follows Na and diuretics also block the na/k/2cl or Na/cl channel leading to high urine chloride

​​@@mariamaboutouk8849I think this is in a specific setting, where diuretic overuse leads to hypokalemic alkalosis with reduced tubular NH4- secretion where Cl- follows it in the urine. Otherwise there is little point of measuring urine electrolytes while taking diuretics.
