Please Dr. Rattan can you do a video on Xanthelasma & peop of color? I have one under my eye & another smaller one forming. A dermatologist took a slice off mine to biopsy, but can't guarantee if she cuts it off or burns it off, that it won't return. All I've found out is it's some sort of cholesterol deposit, but supposedly not from high cholesterol. Mine formed 2 years, after I stopped eating dairy, eggs & all meat, except seafood. I've been told diet is not a factor. Should I let the dermatologist cut it off or just use a wart remover to burn it off at home, like I've seen peop do?

Clean & Clear (Persa-Gel 10) Benzoyl peroxide 6. Cetaphil Moisturizing Lotion

Hi I'm a med student considering becoming a derm and this would be my criticism: 1) you really don't need to wash your face in the morning but if you absolutely must then use something a bit gentler (also don't spend too much money on cleansers, a cleanser with actives really won't change your skin. Vanicream, Cetaphil or CeraVe hydrating cleaner is great) 2) exfoliating in the morning (even when followed by sunscreen) isn't ideal since sunscreen isn't always a perfect barrier and you should always be on the safe side. plus you really shouldn't be exfoliating everyday because you're already using differin which should be normalising your cell turnover rate anyway. 3) 30% vitamin c is too too much and can't not irritate your skin. too risky and not worth its benefits. 4) the ordinary's niacinamide serum has too much niacinamide which could irritate your skin (especially since it's already prone to irritation due to the adapalene) 5) you seemingly don't use an actual moisturizer which is a huge no what I'd recommend is: 1) instead of using a spot treatment you can use a benzoyl peroxide wash (let it sit on your skin for 5 minutes) 2) exfoliate no more than twice a week and only in the evenings (you could also skip adapalene on exfoliation nights) 3) instead of layering an antioxidant serum with sunscreen, use a sunscreen that contains antioxidants. layering sunscreen is never good since it can drastically reduce its protection. 4-5) skip the niacinamide serum alltogether and just use a non comedogenic moisturizer that contains niacinamide (this way you'll have niacinamide and also an actual moisturizer) what my recommended routine would look like is: morning: 1) wash your face with lukewarm water ONLY and make sure it's dried well 2) apply sunscreen generously (a sunscreen with antioxidants could be cosrx's new sunscreen which you can find on their website but there really are tons of great sunscreens with antioxidants.) evening: 1) wash with a benzoyl peroxide wash 2) apply alpha arbutin and LET IT DRY (adapalene on wet skin equals irritation which we can't afford to have especially with skin of color) 3) apply differin and also let it soak in for 10 minutes 4) seal in with a moisturizer that has niacinamide (could also have several other ingredients for hyperpigmentation) 5) you can also apply the moisturizer first, let it dry and soak and then apply differin on top which will assure that you distribute differin with an even layer on the skin. this reduces irritation without decreasing the benefits of differin feel free to comment if you'd like product recommendations :)

Some of your comments though are contrary to what Doctor V has previously suggested. 1. Dr. V has never advocated for not cleansing your face in the morning 2. Dr V said her favorite vitamin c serum is Facetheory Regena C30 3. Dr. V suggested using Alpha Arbutin and Niacinamide together everyday (specifically the products from The Ordinary) Many of the products listed are suggested from Dr. V, except that Dr. Dray suggests Differin Adapalene in conjunction with a topical Benzoyl Peroxide. So my question more or less was…how do I combine Dr. Dray’s suggestion of using Adapalene & Benzoyl Peroxide together at night with Dr. V’s suggestion of using Alpha Arbutin and Niacinamide together at night? Would the ingredients deactivate each other? Would the combination be too harsh? Is it wise to split the night routine in two and apply on alternate nights?
