Lets put aside why Cletus is much more insane than Joker. Cletus did not went through any traumatic or depressing event that broke him enough to become insane or evil. Joker went from being a failed comedian to become the ruthless crime lord in gotham. Joker would even throw up or see carnage's kills as inhumane, as Joker prefers slow killing or torture, Joker also kills for a reason, to inflict pain (emotional/physical/mental) to someone or to prove something, while Cletus just kills just because it drives him pleasure
