Zuko's sickness can actually be contributed to a real disorder, called Conversion disorder. It's a somatic disorder caused by mental illness or distress, specifically in regards to how someone sees themselves vs their reactions to the world. E.g. a "pacifist" might experience this if they get angry enough to hit someone. Their arm could be temporarily paralyzed, because of the mental distress from the conflicting ideas of pacifism and the desire to harm someone. In the same way, Zuko sees himself as being against the avatar, wanting to capture him, as loyal to the fire nation. Then he lets Appa go free, which helps Aang and conflicts with his self image. The resulting sickness is a result of his mind trying to reconcile the 2 realities. He even follows the similar behaviour, like a lack of concern about his condition afterwards, and a psychological problem which is remedied after the sickness (his anger and depression). It's not to say that these are permanently fixed, but they are alleviated by the sickness.
