Altered is short for altered mental status. It is how we describe someone who has some neurological issue or mental health related issue. It isn't always due to drugs as said in this video. Could be traumatic brain injury or any of the other things he listed in the video. We don't just mean they are high.

Yeah... Got into a bad accident 2 weeks ago on my bike and the poor EMS team had to deal "with what day is it?" On repeat while I apparently very stubbornly refused to leave my son's side.. and got very belligerent about it when they insisted. I.. have no recollection of this event, but given their demeanor and my son's retelling of it I suspect it's quite accurate. Sorry guys >.

I prefer AEIOUTIPS: Alcohol/Acidosis/arrhythmia Endocrine/electrolytes Infection Oxygen/overdose Uremia (actue kidney issues) Trauma/Temperature Insulin(diabetic emergencies) Poisoning/psychosis Stroke/seizure I know its a bit longer but I feel like this mnemonic encapsulates most issues that EMS can identify, treat, delay death and help the ER come up with a quick treatment plan

It's really just if you're close. Today is Wednesday, April 24, 2024, and you can probably get +/-3 dates of and maybe the day of the week off by 1 on each side. If it's late enough or early enough in the month they'll give you the one before too.

If we're going with altered mental status my first biggest concern would be hypoxia. That's the biggest indicator. I would do an 02 sat reading and check the waveforms before i proceeded with treatment.

“Diabetes Solution” Book by Dr. Richard K. Bernstein, Diabetologist and T1D himself from age 12 to 90 (in two months). No diabetic complications. I follow his book because I don’t want the complications from the ADA dietary recommendations, or “eat whatever you want, then dose with insulin”… like that that ever works. Life with T1D is so much simpler without the 🎢🎢🎢🎢🎢 blood sugars.

The ER doctors kept asking if he had neurological problems or had a seizure disorder, and when he was discharged, they sent him home with papers saying,”you may have had a seizure because you didn’t take your seizure medication…” THEY HAD NO IDEA WHAT HYPOGLYCEMIA EVEN LOOKED LIKE!! It’s scary to think we know more about his diabetes than they do

Random, I always heard; sepsis, seizures, strokes, and sugar. When u said it fucked my brain for a second. It may be how I was taught the corresponding warning signs. Someone lame/down/slow Y4; yellow, yelp(little shaky dog), yaw( face like going through a turn), yum. You would be amazed how much u can SEE someone's health in the first few seconds if you like. By all means not everything. But 2 minutes gives so much. Bravo to you for learning to see and make momentous decisions in mere moments. P.s. absent seizures are ABSOLUTELY real. Only realy 2 signs, limp limb(s), surely seen. As in you KNOW they should hear/see you.

I understand that as an emt you're not primarily a psychiatric or crisis response, but you can be, so I'd throw 2 Ds in there (delusions and dissociation) or at least an M for mental health. That too can lead to someone not knowing the year, president, etc. I personally deal with bouts of amnesia, and I've bombed on the mental status exam a few times as a result of my mental illness

True, this acronym is more so to rule out the life threatening causes and the things that we can treat, or attempt to treat, in the field. Usually if we rule out these things then we assume mental illness or dementia/alzheimer's depending on the patient age. Those things don't impact what immediate treatment the patient needs from us specifically if that makes sense. There is also another acronym which is used more often and includes psychiatric which is aeiou-tips

I’ve been in septic shock and it happened fast 💨 😢 and by the time I got to the emergency room I didn’t know wtf was going on, I only had the presence of mind to give consent to be intubated and sedated and then I was awakened 13 days later to see my feet black with dry gangrene because the 3 different pressors used at once was causing tissue damage, to those who don’t take MRSA (methicillin resistant staphylococcus aureus) seriously, it can manifest in our lungs as MRSA-pneumonia and it has a high mortality rate, I consider myself extremely lucky to be alive and only missing a couple toes and half a thumb, it’s very hard to recover from and it’s not about “pushing yourself” especially if you’re still anemic from the illness, it’s been over a year for me now and I’m still not right, I just had to have an iron infusion because I’m not making enough hemoglobin 😢 I shouldn’t be anemic in any way.

We don't start with altered. That's not enough to start diagnosing. We always do basic vital signs first. So if there's no fever they're obviously not septic! Do a thorough exam and vital signs before you start any of your acronyms.
