While an ACL surgery is a short procedure performed in an outpatient setting, it requires a long recovery. In this Ask Dr. Geier video, I explain the rehab process and when you can walk and run after ACL surgery.

Jodi asks:
ACL repair and sutures in meniscus means no walking for me for 4-6 weeks. Ugh! I can’t imagine teaching on crutches and am worried about more swelling when I return. Thanks to Christmas break, I’ll be home 3 weeks total. With all the appointments, I feel like I need longer. The first week was all sleeping. Am I looking at a full year before I can run, jump, and play again?
In an ACL reconstruction surgery, orthopedic surgeons make a new ligament, basically replacing a torn ACL. Your body will turn that graft into a new ligament, but it takes months to get as strong as your original ACL.
The first part of your rehab involves regaining full knee range of motion and strength. As you progress, a physical therapist will guide you through returning to jog, regaining the ability to jump and land from jumps and more before starting sport-specific training.
In this video, I explain the rehab process and discuss why working with a physical therapist is so important. You will better understand when you can play sports and run after ACL surgery.

Hey, I had acl and surgery back in feb 2018. I completed the therapy but unfortunately I never got back to sports. Im having constant pop throughtout my work day nd my knee starts back swelling up. Do you think i tore something again? At this point i feel like all the money spent was a waste and temporary.

Hello, I just watched this video and I need some advice. I had my acl replaced and my meniscus sowed back together 10 months ago today, back on February 7th of this year. I still have not run or jumped once, but I do squats and leg exercises in the gym. I'm talking about repping 225 on squats, nothing too light. So I was wondering if. Do you think I could run and jump?

I’m 47 years old and had a fully torn ACL. Doctor did a full reconstruction using hamstring from the same leg. Tough start to my recovery for the first month. It took about 4 weeks to get off the crutches but I persevered with the direction of a physiotherapist. It’s been 4.5 months since my surgery and for the last 4 weeks I have been jogging with intermittent running every 2 minutes for a total of 20 minutes about three times per week. I could probably do more if I pushed it. Also jumping and doing ladder drills regularly as part of my training. I work hard at it everyday for at least an hour using a specific regimen that has been regularly modified and prepared by my physiotherapist who shows me what to do and makes sure I’m doing it correctly. Knee still doesn’t feel “normal” but it is getting better and I can see light at the end of the tunnel.

I had an ACL in my left knee 2years ago and I returned to sports after 11month and that due to less practice. Unfortunately had another ACL/Meniscus in my right knee 4month ago and I feel that I can return back to sports in 6 to 7month as I am following the right rehab process. It is all about practice and training.

Ive had acl allograft/meniscus surg 12/11 approx 5 weeks ago. Ortho surgeon told me yesterday i wont be able to fast walk till probably 4 months out.. also mentioned 18 months full recovery. Long journey but gonna do it!! Also i am doing p/t one time per week due to cost. But i also do those exercises at least 1-2 times daily.... sounds like the info you provided and my ortho surgeon provided arent much different. Im very hopeful thanks for your informative videos

I am 4 months post acl surgery on my right leg and about 7 to 8 months post acl surgery on my left leg. I am beginning to jog once again but jumping on both legs is good but jumping and landing on my right leg is so bad but my left leg is very good but it's just crazy. So I know I can do this and everyone else can do this!

I'm 5 months post surgery, had ACL and PCL donor grafts. Also I had MCL reconnected. Still have a knee brace on and not exercising on instructions from the surgeon here in the UK. I am hoping I can get back to normal activities eventually but I have been strongly advised not to do any sporting activities for another 13 months for fear of damaging the allografts.

I had to get my acl, mcl, and meniscus all fixed I had surgery in November and now I’m in the 8th month going on 9 months July 1st. I’ve been running for a month now I’m lifting I’m jump roping and doing things of that nature going to physical therapy things started off very slow but now things are picking up so quick and I’m able to do so much. When I first started running and working out harder, it was sore on my off days. But now the soreness is gone I only have to deal with swelling and I ice everyday so that I can get pass the knee swelling stage as quick as possible. This experience has made me mentally strong and also has made my legs a lot stronger. Overall I’m blessed and happy to know that I’ll be 100% by august. I’m on the right track and for those who are scared or going through a lot of different problems continue to stay positive and work hard, yes it’ll be painful but it’s all mental and if you are able too push through you’ll be fine but always remember if it hurts to much stop immediately but discomfort and a little pain will come with it. In closing everyone heals differently, so if you aren’t making quick results stay with the course and always check in with you’re doctor because sometimes it isn’t our fault I’ve been told that when the surgery isn’t successful you’ll have a lot more problems then others so always keep a sound mind and ask questions to better you’re future ! Hope this helps all love everybody !

I just want to get back to the field and play again as soon as possible.. could you tell me you exercise routines and strategies or anything that could be useful to me? Thanks in advance!

My knee is 100% back to normal now. I’m in the gym and doing a lot of strength exercises. I know the grinding can be irritating but you have to keep extending and bending it so that over time it starts feeling normal trust the process and push through the discomfort that’s the only way it’ll feel normal again it’s not easy at all but with repetition you’ll get it!

I’m three weeks post op. I had an ACL/PCL reconstruction w/ a meniscus resection and Baker cyst removal. I’ve never had a brace. I could fully weight bear immediately. I go to PT three times a week. It’s a struggle. But, don’t give up. I’m still dealing with lots of swelling & ICE is my boyfriend 🤣. I miss the gym. But, PT is definitely keeping me on my toes.

Currently I completed 8 weeks of operation of my ACL surgery and fracture in tibia , I can get back my range of motion by 120° in 21 days , because after surgery one month is total bed rest suggested to me and then my recovery is good now and I feel I can become normal sooner because my physiotherapist address that my recovery is too good. I hope whoever goes under any type of surgery don't loose hope nd do proper exercise to regain your strength and believe me , you will become normal as soon as possible.

Thanks for the video! I'm about 4 months post op, quad autograft with meniscus tear trim work only. Just started running, but only on treadmill, and only about 5 minutes/ half a mile every other day to start, as I'm not in a hurry to return to my adult rec soccer and basketball, but want to keep progressing. Have a very hyperextended healthy leg, so my ROM is not equivalent, but have at least some hyperextension and close to full flexion in injured leg. Quad strength and size still needs a good deal of work, but improving. Get stiffness and pain mainly when I'm not moving or working it enough. Quad muscle work helps. Helpful to read how everyone progresses differently, thank you!

Hi Doctor, Thanks for the awesome videos. I had ACL reconstruction 6 years ago. I stopped the exercising just 3 months after surgery. I had pain doing exercises so I stopped it and got back to normal routine. I still have lots of pain when I sit at some position bending my knees. Is it possible if I do the post ACL exercises will it still work to get back ligament strength ? If not are there any specific exercises to get rid of pain ? Thanks Nithish

I'm at the 1 year mark, and can finally start to walk (have normal gait) without awkwardness. I've been doing weight bearing exercise at the gym, and started to rock-climb (avoiding bouldering). My ultimate goal is to run many miles, but I haven't been able to run much at all. In terms of pain, I always have soreness and pain after exercising in the local knee area. And sometimes have swelling and stiffness. With rest, massage, and stretching, pain and stiffness usually improve. I keep a rolling pin at work, and when I have some extra time, I roll out my knee with the rolling pin and massage it. This helps bring good circulation to the knee and relieve some pain. I try to be mindful of proper form when walking around. Try to engage your quads when walking and not compensate with the other leg. The best thing is to listen your body. Everybody's knee is a little different.

Thought it would never get back to 100% , Bit you will......IF... you listen to your doctor and your PT,,,,Do keep up with the exercise,,, But ..DONT ..over do the exercise,,, I thought the harder I worked the faster I would get back in shape,,Wrong! I ended up setting myself back a few weeks because I over worked my knee.. Work smart,, and you will be fine,,BTW- I could walk with my full weight on my leg( with a brace) when I came home the day of my surgery. BTW- I had a ruptured ACL, a torn mcl, torn minicus ,,and a compression fracture ,,

I’m currently 9-weeks post-op with an ACL reconstruction using a Achilles’ tendon allograft. Recovery has been great, and I’m probably a couple of months “ahead” of where I should be. I am physically able to jog, jump, and do some cutting, although I know I’m not supposed to. The hard part for me is holding back, not returning to sports, and giving the graft time to heal. I think focusing on regaining muscle activation, walking, strength really early on, in the first week, has payed dividends in the recover. I also have been advancing the strength training portion as I’ve felt comfortable and haven’t felt pain or swelling by doing body weight squats progressing to barbell low bar back squats, RDL’s, Bulgarian split squats, steps ups, etc. All this said, I have been paying really close attention to body mechanics and form and haven’t been doing anything that overly stresses the graft, which most of the things I’ve mentioned shouldn’t if done properly. I have had a little bit of incision site pain and minor swelling occasionally, but all normal. My plan is to continue progressing the strength training gradually as long as there’s no pain or swelling while making sure focus on regaining muscle/strength balance between legs, and getting more into plyometrics in the next month or so, and heavier into return to sport drills (basketball for me) around month 4. But I feel like I am physically able to play at about 70-80% now (and have already dunked again off two feet) so I need to just be patient and not push it. I’m 32 and was pretty active with lifting weights and playing ball beforehand, and all I did was tear the ACL (no meniscus) so all that helps!

Hello Dr. David Geier, I had ACL reconstruction in France the 21st of June. So its been about 3.5 months. I did physical therapy for about two months after the surgery, then I had to move to Germany. For the moment, I do not have the ability to see a doctor or physical therapist here. I feel like I am moving backwards. I am a soccer player and really want to be able to play again soon. I have a bike, but my biggest problem is motivation. I feel like every time I try to do the exercises on my own and go biking, it hurts afterwards; whereas, with my PT, it felt fine. I would like to start running, but I am worried that I am not ready, since even when I do exercises like single leg squats, it really hurts. What do you think I should do?

why would a Doctorr tell the patient after ACL reconstruction that they will need to continue wearing a knee brace during any physical activity for the rest of their life? when my doctor said that, I just assumed it was standard for an ACL injury, I was pretty clueless at the time but he didn't mention any complications with the surgery and now I realize it's not normal to have to wear a brace forever so I wonder why he said that...? I got my flexibility and range of motion back just fine right away, and the physical therapy went just fine as far as I could tell. Although I was a bit frustrated with the fact that, for the strengthening exercises, the physical therapists seemed intent on doing them at an intensity that would seem more appropriate for an 80 year old woman! I was early 20's and male, I was obese and out of shape so I'm sure that was a big part of it but they had me doing exercises in sets of 10 reps at an intensity level that I could've done sets of 1000 reps all day long, and they didn't want to listen to my feedback about that.

Hi sir im 24 years old from Kosovo. I had ACL and meniscus repair 4 and a half month ago but I was able to jog after 2 month and a half and felt no pain or swelling. Can you tell us if recovery time depends in genetics or what is the reason for such a fast recovering. Btw I can play now whatever I want and still didnt do 5 months from surgery.

Hey doc, quick question. I am 1year out of acl/meniscus surgery and I decided to play baseball since I have been able to jump & run with a little soreness but after playing my knee has been swollen for a week no real pain unless I bend it all the way. Right where I have my scars are on the right side of my right knee have swelling and tightness almost like 2 balls.. did I possibly damage something or is this normal after a year post op ?

I started walking without walker within 2-2 and a half week or so, and after 1 month I'm capable of walking close no normal, but every now and then I get my hamstring accidentally pulled due to slipping on the floor or getting a jerk which causes alot of pain in the hamstring part, what can I do for that?

I'm about 4 months now, feeling great! Maybe i will start to jog now. But sadly my flexion[RoM] wasn't that normal like it used before due to my graft B-T-B , my knee had limitation for about 6 inches i guess.

I partially tore my ACL in February of 2018. They suggested I do physical therapy and maybe i could recover WITHOUT surgery. So I did that for 6 months thinking my knee was getting stronger. However, a week before tryouts I “retore” it and it felt like the way it did when I first injured it. Overall if you even have a partial tear, I suggest u getting surgery bc it’s not most likely not going to get better :’). I’m getting surgery this Friday hopefully all goes well

I'm in the UK. I had ACL and meniscus repair surgery on 30 September last year. The first few weeks were awful, I had a lot of pain in my calf and wasn't sleeping. It took me about a month to get off crutches. I think I'm getting there although I was discharged by physiotherapy pretty early and feel like I'm winging it a bit. I've been gradually building up strength and one thing I did as a test was put a load of bottles of beer in my backpack and did one-legged squats with the bag on my back. I read somewhere before you run you should be able to do 8-10 one-legged squats with 120% of your body weight. I'm planning to go for a short run today of maybe 500-600 metres, it'll be my first run in eight months but hopefully it'll go well.

@@mikeb9909 it's pretty good all things considered. I had a bit of pain first few runs and had to stop a few times annoyingly early but it's better to be extra careful and I was back running 10ks within a couple of months of my first run. I haven't played football since which I'm still hoping to do, partly scared but also when I got back running I stopped doing strength and conditioning exercises thinking the running was enough so now I've rejoined the gym and am working on more jumping and twisting exercises with some strength building. You'll get there. I think the most important thing is do the exercises and listen to your body. If you're feeling pain stop.

I had physio for 5 months after the surgery 3 times a week, stopped physio as the doctor said this is how good it will get for me ?? Im 29 male and was really active before the injury, is there anything I can do to get back to normal? I'm beginning to feel hopeless and its depressing.

I hope someone answered your question. I had acl surgery from a quadricep graft. I also fractured my tibia & tore multiple muscles (calf & foot) throughout my leg. At about 8 months I could run again. Actually it was more like 6 months, but I was running with a limp. The recovery time is different for everyone. I do not jog, just ride a bike swim & do aerobic exercises. I am waiting for a full year, as recommended by many physicians, until I jog again. That's not to say I don't run. I will run for very short distances. Usually on a track or flat surface, with no holes or bumps. I've noticed this is therapeutic for me. It's the only exercise that still fully works my weak leg. However, I noticed that this running aggravates my graft site & causes swelling. So, my knee seems ready, but my graft donor muscle is still to weak. I still have 2 months & then I'll start more run/light jogging exercises. Test your body & see how it responds.
