Although I agree with better recruitment practices (the anonymous idea), the main problem still stands. Notice how it is always stated, “how the candidate PERFORMED in a final interview”. That’s the problem, interviews are just performances. Whoever acts the best, wins in the end (not those that would actually excel in the position). Large amounts of pressure is placed on a candidate: don’t be nervous, answer the question exactly as they want to hear, don’t stutter, make sure everything flows, etc. The problem with the hiring process is that perfection is expected when no one is perfect.

Absolutely. And I think often the interviewer is more focused on finding reasons NOT to hire someone, until one candidate is left standing. But if that one isn't perfect, they won't hire - they'll re-post the job and start over.
