Might it be a good idea to provide examples of cases in the age cohorts that are most vulnerable to losing bone strength AND most vulnerable to falls and multiple fractures, the 70 and above age group? . Thank you for a most informative first video that I have ever listened to. I refused drugs and am working on building bone through exercise and diet. I don’t think the dexascan is accurate for long lean bones. I got hit by a car thrown up in the air if my bones are so fragile I should have broken my hips and spine not only my ankle and foot in 6 places That was 2 years ago I do Pilates and barre classes, strength and resistance training. play pickleball

I am 60 and was diagnosed with osteoporosis 4 years ago. T SCORE -2.5 I was also overweight. I started on a serious exercise regiment of walking, yoga, weights. And a calcium and vitamin supplement together with dietary improvements. As i got fitter, i started running and increased the weights. I lost weight, all my pains disappeared. My bones improved. My health improved and even my mood improved. I now love to exercise, its the best decision i ever took .

@@gailchypyha3587 I hope you get better. Check out a good age appropriate exercise program . I hired a personal trainer at first , as I did not know how to exercise without injury. It was the best investment , better than paying hospital bills. The bonus was on other aspects of my health as well. I was prediabetic, it disappeared. I had high LDL, it normalised. My stiff joints loosened , I became pain free. It's slow and one needs patience, but it works. I dont smoke or drink. God bless.

Helpful and practical video. When Fosamax first came out, it was even being prescribed for osteopenia. Now some doctors say it should not be prescribed for people who don't actually have osteoporosis. At first, the drug was considered a lifetime drug. Since some serious side effects have come to light, they have backed off on long-term use, using such terms as "drug holiday." The bottom line is that some of the earlier guidelines for this drug are being walked back. Despite that, many doctors are still promoting it without taking these patient differences into consideration. Let's all hope and pray a better, less dangerous drug comes along.

My aunt was prescribed a bisphosphate (Fosamax I think) for her osteoporosis. She's also previously had oral cancer and radiation treatments for that cancer. She should never have been prescribed that medicine given her medical history. She died of necrosis of the jawbone (complications due to...?, but yes, we were told it was 'necrosis of the jawbone' that killed her). I feel like too many Drs prescribe patients whatever is the common drug for whatever issue without considering a person's total health picture, medical history, or even what other drugs they're currently on. And then they get angry with you if you don't want to take it or express concern or reluctance - some are even horrible bullies about it. People are losing faith in doctors, and its not surprising.

I'm sorry to hear of her experience. You are correct that prescriptions are often written without an understanding of the full picture. There are a lot of reasons for this but no excuses. The bullying part gets under my skin and is related to hubris and pride in my experience. Thank you for sharing your tragic story.

My dr started me on alendronate 70 mg, for last 2 months, extreme pain on bottom of both feet, got steroid injections on both feet, fixing to get steroid injection on L-4, L-5, bc of recurring sciatica. I can’t even go exercise anymore & it all started after taking this drug, I AM NOT taking this crap anymore, I’d rather die exercising sporadically than nothing at all. Never had prob’s w/plantar fasciitis before! Yes I have osteoporosis & osteopenia but I was happy before, I was exercising, now nada! No more Alendronate for me, I’m 75 & miserable w/this drug. No mas!!

i was told I had osteoporosis by a PA after a routine bone scan. I didn't see the report but agreed to take Prolia because I didn't want to take pills weekly. Big mistake! Pain from the first week. Feet, hips, legs, back, ribs, knees, hands, fingers, jaw! OMG! I did research (which I should have done BEFORE but I trusted the doc)...anyway all the POSSIBLE side effects listed were what I had. Kicker was once I saw the bone scan it only showed my arm had the bad score. I had a spine fusion and they couldn't do the complete scan, but the other areas were fine!!!! Later I was told perhaps I had fallen and hurt that arm at one time. Anyway, I also was not told that when I stopped take Prolia, I HAD to have a relay drug or I could get vertebral spontaneous fractures! WTH! It has been almost two years since my one and only shot. Still have pains and I have been to six different doctors and they have no idea what I'm talking about!!! Apparently, after going off Prolia the bone is basically goes into the loss of bone unless stopped. It is almost impossible for me to exercise now. I was told I need spine surgery (again) and a complete knee replacement! So frustrated! I will not take any meds for this EVER as I tried Actonel as the relay drug and hurt just as bad!

I have taken them all. Most made me sick after using for a period of time. I rather risk fractures than ever be on these drugs again.

I lift free weights and walk daily, do chair excercises daily...hoping that helps my bone strength too.

@@lindathornton3626I hope it helps u allot my problem is I have stage 4 very severe emphysema so get extremely breathless Last yr I had two severe wedge fractures at T6/7 and it given me kyphosis and heigh loss Im n terrible pain Not only with my back but I feel like I’m being crushed around my ribs and my abdomen feels so uncomfortable I’m struggling to breathe eat It took doctors 5 months to heal bfirm me of the fractures because emergency doctors failed to pick them up on X-ray it was having a scan on my gallbladder that revealed the fractures/ kyphosis the report also states were present on X-ray 5 months prior I was told it’s all muscular by two doctors after they were poking there fingers in my back and spine 😡 for all I know they could of made it allot worse because believe me I nearly shot through the roof when they were poking around So basically I was unaware still bending twisting and the respiratory physio had me doing wall press ups and I excersizes that I’m not aloud to do I now have a physio after 12 months and they gave me a brace To little to late the brace really hurts it’s so hard on me spine and digs in like a hard board Trying to sit in it is dreadful Iv also got dengerative disk disease and my lower box burns like mad I do heel drops Small squats but just 5 Stand up from sitting 5 times And a band that I put under my foot and pull but again only five Theses are to strengthen my back muscles I feel as though I’m getting no where apart from my calf muscles hav I’m proved and strengthened but I do Moore than the 5 that I was told to dob Every time I go in my kitchen I stand and do 5 that can be a few times a days so prob doing 50 throughout the day If I’m told to do stomping also just 5 of those and not to hard I’m in alendrate acid calci-d 1000 and now they want me to have zoldronate infusion 1 every yr for 3 yrs I keep backing out I’m wondering if the alendrate is making my pain worse and is so what is the infusion going to do?

Hi Linda, we use younger patients because the contrast between scenarios is more clear. The older people get the more we have to ask ourselves about other limitations from aging. We certainly work with people in their 70s but the conversation changes as people age.

Thank you for explaining the variables. I’m 69 and have been researching this heavily for the past year. This is the first conversation I’ve seen that actually offers a considered approach vs a blanket ‘don’t take the meds’ approach. I am on hormone therapy after breast surgery I have been on Arimidex tabs plus calvium daily for 7 years. My last Bone density test was poor so Dr says infusion of Zometa 1infusion 3 yrs. Bones joints are painful

I so much appreciate this video!! What a clear and concise explanation of bisphosphonates. I was prescribed them at age 40 because of osteopenia and had horrible side effects. Doctors have continued to push me to take these drugs even though they made my quality of life so bad. I am now in my late 60's and probably have osteoporosis. I had a DEXA scan 6 years ago that still showed osteopenia and have now decided that there is no point in getting another one because I cannot tolerate the drugs. I have made every lifestyle change possible and that will have to be good enough.

My neighbor and my sister both took bisphosphonates; one weekly, and the other was a monthly prescription. They both had extreme pain immediately in their bones. My neighbor, 82 yrs old, went to the hospital for help, screaming in pain and they told her there was no antidote for that and she'd have to just put up with it and it would probably last a month. They gave her painkillers, which she eventually got hooked on, and no resolution for what she took. That's why I will never, ever take bisphosphonate's.

Wow, I'm sorry to hear of their experience. Unfortunately, side effects are often under reported and what we hear on the clinical side seems like higher rates than what we see in studies.

@@Dr_DougLucas That's how it always is. I think it's the typical shenanigans used in so-called clinical trials and "studies" that have an end goal in mind. They ignore or downplay side effects, making the patients the actual lab rats later. I'm a retired RN, and hubby is a retired pharmaceutical chemist, and what I've seen since COVID, when I finally started paying attention, is that the whole system is set up not with the patient in mind, but to support Pharma, especially the big names.

Same happened to me, minus the pain killers! I will NEVER take 1 of these drugs again! I have cancer, have had several intensive surgeries, nothing caused me more pain than Zometa. Another doc prescribed XGeva, I had to look it up myself to see that it was in the same family of drugs! Never, ever, would I take these.

I am 54 Breast cancer survivor in AI anastrozole and following two Deca scans I have just reduced my first yearly IV of eclastra . Really suffering with the side effects . Bit I have just had a really hard 24 hours !! No one mentioned the side effects to me !!! But thanks for the great explanation of why I am taking this drug !! Helping me get through the side effects !!!

This is one of the best explanations on osteoporosis treatments I have come across. Thank you ! I have osteoporosis and I don’t want to take the drugs but my recent bone scan report showed significant deterioration in my spine . Still don’t want to take drugs. Daily walks and weight lifting a few times a week haven’t helped. 😢

I won't take drugs either....especially Fosomax....NEVER!!! I am really disappointed that I went to the Osteopathic clinic at the University and the only thing he told me was to take Fosomax. Really????? Is that all you can tell me to do with your "speciality"???? The only test I've had is DEXA scan and not offered any other tests. You don't have to tell me to take the Masterclass because that is your reply to everything on here.

What pisses me off while going through menopause..not one doctor mentioned calcium supplements or any other type of prevention. But, OMG you have osteoporosis so you need to take these toxic drugs🤨

But calcium alone is not the answer. They were overdosing everyone with calcium. Remember they were even marketing Tums for their calcium, and even putting it in some chewing gums. All that calcium was not good. I believe it contributed to dementia and heart issues. I have lost faith in the mainstream medical community. My grandmother wasn't popping calcium all day long and she never fell...also never had a dexa scan. I put this in the same place as mammograms. More harm than good.

My experience with Fosamax: Took one pill. Starting hurting all over and felt pain and weakness in previously healthy joints (3). Never took another pill. Those joints have never been the same since taking that pill.

That's why I don't feel comfortable taking those pills because of the side effects of what they can do To take the pills and then feel sick and horrible from the side effects and how would we be able to work out and straight train and get through the day if you're feeling awful from the side effects? I made this mistake taking birth control for many years I don't want to make this mistake anymore

@@Dr_DougLucas the reason I was on birth control for so many years it wasn't to prevent pregnancy I wasn't even active , my doctor and other doctors I would go see recommended birth control because I wasn't menstrating , even though birth control pill did help with my severe severe awful cystic acne and gave me a menstration I know now it wasn't a real menstrual cycle , doctors should've helped me find out why I wasn't menstrating and help me get back my real menstrual cycle But a lot of doctors have too much faith on the pill , so all those 11 years that I Didn't menstruate is one of the reasons why I have osteoporosis today , I do feel misled by my doctors of those days I think it was lazy on their part for not trying to figure out why a female in her 20s wasn't having a menstrual cycle instead of putting me on a pill to have a fake menstrual cycle like I mentioned earlier I wasn't even active I didn't need to prevent pregnancy ,, to this day too many doctors still have too much faith on the pill for acne and to help women have a fake menstrual cycle I regret every year I was on that pill but it wasn't my fault I didn't know any better back then I didn't know to question doctors in those days I was just a kid doing what the doctor told me to do

Thank you for this information. It has been very helpful for me to hear. I took a 30 day Boniva pill in my early 50’s. I started breaking out in hives within 15 minutes and my throat was swelling shut. So I took a benydryl capsule and went immediately to the close hospital ER for help. I had to take Benadryl for 30 days every four hours. Also, my jaw started hurting. It was not a good medication for me. A very scary & awful experience. My physician advised that I never take another bone building drug because they are all cousins. Currently, I have been able to increase my bone health with diet, vitamins and exercises.

Thank you! We appreciate Dr. Lucas for sharing such valuable and important information. One terrible side effect not mentioned (unless we missed it) about Reclast is that Reclast may also cause dirilium as side-effect especially with elders. This has been documented on a medical research report we found. Our grandma had a Reclast infusion, which her Endocrinologist pushed hard for solely based on a bone density test (DEXA). The test result showed she had borderline osteoporosis. The endocrinologist tricked her into having the infusion in a deceiving way! After the infusion, she developed hypothyroidism and gradually showed symptoms of dirilium! It scared us! Not all doctors are caring, compassionate, honest, and are willing to spare time to keep up with up-to-date medical findings in their practices. So, we all need to learn to be our own and our loved ones' health advocate.

It seems to me that the relative reduction risk should be considered fraudulent advertising! Thank you so much for explaining this! My Dr. didn't mention any of this when he prescribed me Fosamax! This has clearly explained to me all the concerns I’ve had around taking a drug. Unfortunately here in UK there is no help apart from take a drug. I’ve seen a Consultant and that was his answer. It’s so frustrating because as you say I ended up looking at endless FB pages and that’s not helped either.

Thank you so much for this valuable information. I am 58 and had a hip replacement last year following a nasty fall. Since learning I have osteoporosis, I have been confused by all the treatments available. I had already made up my mind the meds weren’t for me and have been considering HRT. I’m putting it off time and time again but as Dr Doug has explained, I am one of the women who has been anxious about all risks involved. I have watched the relevant talks to me and now feel confident to ask for HRT. I have a plan in place and feel so much better. Thank you so much for these talks. I only wish we had somebody in the UK who had this approach. I haven’t yet found the help suitable for me like this.

@@jayneread2178 The flawed WHI study greatly extrapolated the breast cancer risk. In fact studies show it is protective of breast cancer, heart disease, alzheimers.,osteoporosis and many more. My fiancees Aunt is still on her hormone use of 40 years due to hysterectomy.She has no health issues. FDA approved bioidentical hormones, the same molecular structured hormones we made before menopause took them away are a great option for many women. Thank you for the great information. I was just diagnosed with osteoporosis this week. I am a 57 year old female and am not on any medications. My doctor is prescribing the drug and I am considering not taking it but trying supplements, weight training and diet. I am very scared and am glad that I found your channel. I am already vegan so eat mostly fruits and vegetables.

I just had my 90th birthday. Have always looked years younger and still do. Now 5 ft. only due to bone loss but keep my weight around 120 lbs., slim and trim still while being conscious of extra calcium plus D-3 and the K's for better absorption. Still confused about the ideal amount to take. I walk daily - again. Had a fall and fractured my wrist which was successfully operated on and healed well. But it had an impact on my body and I never felt the same after that. I changed and was unable to walk the way I always had. Then suddenly muscle pains in one of my legs (sciatica), horrible experience. The other leg has poor blood circulation possibly from an ankle fracture during my years living in Mexico nearly 15 years ago. (I had an oldfashioned cast to the knee for weeks, walked for miles daily in tropical heat and could have overdone?)Now back to walking routine (w. walker) and feel stronger and somewhat improved. Still DEXA shows serious decline. I still refuse meds. Diagnosed with Dysplacia under my tongue since 50 yrs. with many biopsies. So not worth taking the chance on my jaw bone, but probably would decide against any drugs with known side effects regardless. I just don't want to take it, period. Swedish gymnast as a child and, like my mother, never sick in bed for one day in my life.Three healthy sons. Came over on the boat to the U.S. a free spirited immigrant. How long can you live anyway? I prefer to end my days without suspicious liquids in my bod. Did my last show on stage just before COVID. Stumbled on a kitchen mat at home during the run and broke three ribs but never saw a doctor because: "The show must go on." X-ray after final curtain call (Threepenny Opera). By my own final curtain call hopefully still free of Fosamax, Prolia, Boniva, Evita, et al. Better safe than sorry.

The guidelines may be wrong too. Doctors don't have time to have in depth discussions like this one. That's why having a YT doctor is important. I don't trust doctors who are part of the medical and pharmaceutical industrial complex.

I've been on alendronate for over 20 years, along with Vit D3 and Vit K2. My Dexa scores have gotten worse over the years. No mention of taking a drug holiday from my doctor.

My poor mother took an oral bisphosphonate drug for 20 years, and her lower spine collapsed into itself. A surgeon tried to repair it, but she developed an infection and died of sepsis. Never never never would I take one of these drugs that I consider dangerous.

Hello, I'm sorry to hear of your loss. Surgical complications can be devastating and surgery on bone that has been severely impacted by bisphosphonates is challenging. I've done it myself. The bone has limited capacity to heal and wound complications, infections and further complications are common. Thank you for sharing your story.

Thank you Dr. Doug. I am 64 y.o and had the courage to refuse the Bisphosphonate drug that was prescribed by my doctor (gynecologist). I am currently followed by a bone specialist here in Geneva Switzerland, who also teaches at the University. I am taking the Algae Cal + Strontium Boost supplements daily, I eat well (vegetarian), exercise daily alternating with weights, running and yoga. Also take daily collagen peptide powder. Will have a dexa scan in about 5 months from now. I feel great and have to say, I'm in pretty good shape for my age! In general, I find that doctors here in Geneva prescribe way too much medication and I wish that you could come over and give them a training. Love that pyramid that you use with your patients!!

@@Dr_DougLucas Just got my Dexa scan results and I am out of the osteoporosis phase and into osteopenia (where I was 12 years ago!). This is encouraging and I am not taking any biphosphates or any other hard drugs. When I was 58, the gynaecologist put me on Prolia injections. As soon as I got off (after 4 injections), I was in worse condition. It's been quite the roller coaster ride. I am now thrilled with my results and I feel great. I will be seeing my bone specialist soon to discuss my Dexa and blood results (cross laps etc...). I continue to take Algae Cal + Strontium boost and daily collagen powder. Basically, I am on natural supplements, I exercise regularly and I eat really healthy. It's the way to go, at least in my case.

This was awesome, new subbie. I was on Fosamex for 12+ yrs without any followup and stopped (5/2022) due to muscle pain. After my last Dexa in July 2023 they want to put me on it again without consult . This answered a lot of questions and through my Dr I am scheduled for bone health program, but I do not plan on taking script for a LONG TIME. Thank you!

Watching your videos for the past month has been tremendously helpful. I was able to formulate a bone health plan, and my doc is okay with my proposed diet (nutrition-wise), exercise regimen, and supplements. This video on the drugs has helped ease my mind, as I am one of those people who would not take a drug if there were a 1 in a _million_ chance of developing osteonecrosis.

I don't have a diagnosis of osteoporosis, but I haven't had a bone density test either. At 71, having just sustained a stress fracture in my foot, I think the fragility of my bones is something to look into. Thanks for your excellent presentation.

I am a 76 years old female . Diagnosed as a case of severe Osteoporosis with T score --4.8 spine on 12 December 2022 and on inj. Terparatide 20ug s/c daily from 22 December 2022 onwards as prescribed by Endocrinogist. My dexa scan spine of 16 April 2024 showed T score --4.2 . I will be completing my 2 years course of Teriparatide 27:14 on 22 December 2024. Please let me know further course of treatment. Regards

Appreciated this video…. I am pretty much the first woman of the two you compared. I’m 53 and recently had my first dexascan and have osteoporosis in my spine and hip. Was prescribed Fosomax without a phone call to even discuss the drug or the diagnosis. I went to my health provider that prescribes my HRT and made a plan just like the pyramid you described and I’m feeling empowered to do all I can before resorting to medications. Ater taking the Fosomax for one month I just felt like there was a better way at my age, I have time on my hands to try the first four steps before feeling like I have to take Fosomax. Getting educated and taking charge of my health has really been empowering. I will continue to watch more of your videos but this one has been extremely helpful and I appreciate the information. I really appreciate your education on relative vs absolute fracture risk. I had to work that out for myself, after the locum dr at my GP clinic had terrified me with the idea that I could break a hip and die at any moment if I didn't immediately start on Prolia (some poetic exaggeration there, but it was really upsetting). Luckily I called pause on the drugs and did some investigation, where I found that a 58 year old was extremely unlikely to fracture a hip in any case, and similar to the examples in this video, the 75% risk reduction meant that for my age group, the risk of a hip fracture was diminished from 1.25% to 0.5%. I also discovered that the risks of dying from a hip fracture, that had been quoted to me, were severely skewed by the high numbers of very elderly people who die subsequent to a hip fracture, and if you dug down into demographics, I actually had very little to worry about. Which is just to say, numbers never lie, but they can be manipulated to imply untruths.

I am pretty much woman number one in your example. I’m 51 with a t-score of -2.5 in my lumbar spine. I’m in the UK and had to push for this dexa scan after being on PPI’s for six years. My hip t-scores have improved from -2.0 to -1.2 since my last scan so I’m doing something right. Sadly they refuse to check my vitamin D levels or any other blood markers! So I ran off to Google to do my own research and found this video. So glad I refused all the meds offered. I’m now off to check out your channel and free course. Thank you for this video.

Very good explanation of absolute and relative risk, and bisphosphonates. Would really like more info on cases 70+ who have been on bisphosphonates and are high fracture risk despite no fractures. Feels pretty hopeless being between a rock and a hard place.

Thank you, like I said, there is certainly a role for drug therapy especially as patients get older, and our options become less in both number and impact. We work with patients in her 70s and mini do not need drug therapy but this is a case by case discussion and situation.

I was on bisphonates and it caused a fracture in both my femur's. I would not recommend. My doctor never told me of the risk of fractures.

I was on Fosamax for a year when I developed osteonecrosis of the lower jaw. Thought what was visible was an impacted wisdom tooth coming through the gum, but oral surgeon said it was bone! Rheumatologist now has me on Forteo and I've had no issues. I'm 76.

My dentist explained the "clasts and blasts " when slow healing gum jaw produced problems. I found this information extremely valuable for my myself, a multiple myeloma patient once on Zometa. Hmmm. Living with a necrotic jaw. Thank you

You have some great informative videos. But honestly I'm overwhelmed and now my anxiety is through the roof. I had an appointment today with an osteoporosis specialist. I waited 8 months for the appointment. Very little was discussed. I asked numerous questions, but i felt like the doctor was annoyed with me. Diet, exercise, lifestyle, none was discussed. Just take calcium and consider meds. There are no providers like you anywhere. Guess I need to figure it out on my own...

This was an excellent video. I had a lot of confusion and yes, fear, regarding drugs. You explained the risks, what the data means etc. The jaw necrosis thing was my biggest fear as I usually have a lot of dental work. I appreciate you stating, yes it CAN happen, but it’s rare. I would like information as to how you take on patients from out of state? Also I would appreciate a quick video educating us on what to ask and look for when we meet with an endocrinologist for the first time. How do we advocate and educate ourself for this critical first meeting. Thank you again for a fabulous video!

My hip T score 2015-2017 -2.6 jumped in 2022 to hip -3.2. I believe the pandemic trapped me inside and caused the decline of my T score. I started fosamax in Nov 2022 and could not suffer the hives over upper half of my body starting in Jan 2023 and stopped. Further tests, consults w/ endocrinologist, allergist suggested I try fosamax again. Instead, I decided to start X3 training based on your initial presentation mid 2023 after returning from a month in France dancing at tango festivals or walking about 30k steps a day. The resistance training is much more rewarding than stomping up & down 6 flights of stairs and jumping rope. Watching this video has reaffirmed my commitment to continue without drugs, at least until my next bone scan in a few months. I am 73, eat animal proteins, fruit/veggies, and sourdough breads (that I make w/ Italian flour), and dance tango on 3" stilettos. I finally registere for your Master Class next week! Thank you!!!

I am finishing my course of Actonel. It has helped me. Very important to eat well, avoid junk food, exercise,walk and try to stay flexible. Try to stay within a good weight. I get DEXA done every 2 years. I am followed by an endocrinologist who specializes in bone health in NYC. If you have not have a DEXA test ask md to have it done at least as a baseline. I have osteopenia. Lastly no side effects as long as you drink about 12 oz water and stay upright after taking Actonel. I drink some water then take pill followed by at least 12oz water- stay upright and do not bend over for an hour. My situation complicated by idiopathic scoliosis with spinal fusion at age 15- does not interfere with my life. With hypothyroidism on synthroid. Good weight BMI 21. Have good teeth per dentist. I am surprised so many women have never had a DEXA. Everybody is different. I take vitamin d it helps with absorption of calcium. Try to eat green leafy vegetables I rarely use sugar or eat junk food.

At 74 I was just prescribed Actonel once a week for t score of - 2.8 in spine, all other scores are osteopenia. Were you on Actonel for 2 years? Did your t-scores remain the same or improve?

Very important Do Not lie down after taking you will feel sick You must stay sitting up standing I stayed up longer about an hour abd must take on an empty stomach with water. I’m 66 years old with osteoporosis. I did reclast twice my Dr. said it didn’t work. I have had so much dental work since. My periodontist told me reclast etc. does not like the bones in the mouth. I have a lot of bone loss in my mouth. Now she wants me to take Evenity. After reading the side effects I decided not to do it. I have a T score of -2.5 I’m glad I found your channel. Thank you.

Thank you for going over the two scenarios (postmenopausal w/out cancer, and postmenopausal with cancer treatment). I take letrozole and have lost significant BMD in only 1 year. I'm supposed to take letrozole for 5-10 years. I've been trying to increase BMD naturally and have been delaying the Reclast infusion because I thought I could improve my bone density all on my own. I will continue to do all of the things I'm doing concerning strength training, nutrients, etc. I will also let my doctors know that I am now on board with the Reclast recommendation. Thank you for this important distinction. Breast cancer is very common with 1 in 8 women being affected in their lifetime. Thank you for this important information.

My doctor just prescribed Fosamax for osteoporosis. My hip is a -2.5 and he said my lumbar spine is also low and I just turned 73. I up until three years ago have been very physical working as a paramedic in the field and then in an emergency room. I’ve never had a fracture. I had a knee surgery and a knee replacement three years ago, but unfortunately I had hyper parathyroidism that was not diagnosed for over four years of high calcium levels in which I was symptomatic with bone pain, hair loss, and all the classic symptoms. I ended up having two large tumors and two of my four parathyroid’s removed and cysts on my thyroid. I’m extremely nervous to take the Fosamax especially since I’ve had a lot of dental surgery ongoing. So I’m opting to go with the natural approach that I can get from diet and exercise. One of the reasons I decided not to take this medication was I was reading that while taking it in follow-up DEXA scans the bone can appear to have grown in density but in actuality, it’s very brittle! I’ve watched several of your videos on this topic. So is there there any reason my age would prevent me from increasing bone density, by going through the more natural nonpharmaceutical route. Also, what kind of physician should I go to regarding my bone health and diet and exercise outline program to help with my bone loss?

I’m binging all of your videos today because I was diagnosed with osteopenia years ago (like 12 years) and now full blown osteoporosis. Very freaked out by the pharmaceuticals that are planned for me over the next three years. I’ve been active my whole life; runner, cyclist, hiker. This is such a frustrating disease. Age 70 Started with hip pain - led to hip MRI - Led to Dexa scan - to I have a severe case of Osteoporosis. Hmmm compared to a 35 year old person? I am severe? Dr. called once to say I have a severe case. Told her we would talk about it on my next appointment. I've been back to same Dr. twice. She has forgotten all about it. I'm glad because I would have refused her drug. I wish Dr.s would study and research rather than - Here is your drug, you are cured. Thank you for helping me with my research. The patient should also know the duration of the study that risk change. For example, were the differences seen after one, two or three years of treatment? For most of these drugs we see a 1 or 2% absolute reduction in fracture risk after 3 or more years of treatment. The difference is real but not nearly so impressive or persuasive as the relative risk reduction. Whenever relative risk reduction is quoted (in a presentation or in any medical literature) it is usually a marketing ploy. Buyer (user) beware! By the way, excellent video, thank you. I am a Rheumatologist in the UK and will direct my patients to your channel in the future, if I may.

After 3 years, it started affecting my stomach acid. Two endoscopies showed inflammation and had a biopsy on my vocal chords. Stomach all messed up now. Occasional nausea, sore throat when I wake up and burning gums when I wake up from stomach acid. Going off helped about 50 percent but stuck with this for 2 years still.

Prolia caused jaw pain, tingling on my lips, hands,feet and rashes on my forehead. Actonel caused uveitis. Some people certainly do get very rare side effects.

I would appreciate an investigation and information on the difference between bone strength and bone density. I have read that in Japan it has been found may years ago that postmenopausal women who have diets high in natto which is very high in KM7 have been found to have substantially improved bone strength while showing not significant increase in bone density.

I was on a statin for three weeks and in that time I developed bruising and strange little blood spots all over my skin and my liver enzymes went up. I got off of the pills right away and it took a few weeks for all the blood spots and bruises to go away, but it took two years for my liver enzymes to go back to normal. I’ll never touch them as long as I live I take vitamin D and magnesium.

I’ve been diagnosed with hyperparathyroidism at age 68. I had papillary breast cancer 2 years prior and was given Letrozole. I went from normal Dexa to osteopenia T-1.8 within a year. My Crosslaps were 4 x the upper limit. I have broken both lower back molar teeth and had repairs. I’m taking oral alendronate and worry about the risks.

Dr. Lucas, thanks for explaining why the individualization of care regarding treatment is important. So sad most doctors especially in my 🧣 of the woods don't do so. I have seen a number of endocrinologists throughout the years, all recommending different solutions. The last two couldn't be more different in treatment for my osteopenia, now osteoporosis. One classified me as an accident waiting to happen and prescribed Evenity, the current one said no, ordered Fosamax. The current endocrinologist refused testing CTX P1NP, said insurance wouldn't pay, I offered to pay, still refused. So my dilemma is how to know if fosamax is working. Was on Actonel years ago and gained 5% in spine. Im so disappointed.

I really appreciate all this information. I was diagnosed with osteopenia 20 years ago and have been doing weight bearing exercise ever since. Now I have osteoporosis but think I might have had it for some time. I need to go back to my General Practitioner soon with a decision on which way to go and so your post is very helpful!

My spine is in terrible condition from DDD , entire lumbar fused now I’ve developed Scoliosis and osteoporosis of the spine the hardware in my spine has become loose and broken screws. I have been put on Forteo daily injections. What are your thoughts on this drug?

I have both osteoporosis and RA and I’m afraid to take big pharmaceutical drugs. I had a hip replacement in 2020,as well as a compound fractured tribulation/fibula a month later. Now I have a titanium rod from knee to ankle holding these bones together. My neuropathy never healed…and I’m in chronic pain. I am trying Mediterranean diet,exercising is hard…I’ve gained 30 pounds…and I’m a hot mess. Supposedly you can see through my forearm bone,but hip density has improved. What vitamins absorb the best and can be taken for both osteoporosis and R/A. I take fish oil,D3 and K. Thank you for your channel.

Hello! I am sorry for your painful struggle... And I am not a doctor or medical professional.... But eating whole, natural, organic foods are healthier for everyone. But the drug companies don't like that.... That won't make them money. Nor do the food companies. They all have been pushing a false food pyramid since the late 1970s. Look at pictures of people in the 1950s, 60s, and 70s.... NOT obese. Look at pictures of Americans from 1990, 2000, 2010, and NOW. SO MANY unhealthy and overweight. And I am not judging as I have had my own struggles with weight, even as a dancer and dance teacher.... But! Eating grass fed beef, REAL butter, REAL cheese, yogurt without added sugar, fresh organic fruits & vegetables, fish, chicken (NO hormones etc) nuts... THIS is what will help us all to be healthy. I have never done "keto" but I lost my dance studio to covid in 2020 and I was kind of depressed and I ate whatever I wanted for a couple years... Including desserts (because I have always loved caramel, chocolate, treats etc). Well I decided to eat whole foods and not snack in between and to try to eat less carbs to not spike insulin etc. NO PROCESSED FOOD. I lost 30 pounds. I feel great. Sugar is very bad for us all (even though I have always enjoyed a treat). Now I'm pretty much not eating any sugar anymore. Watch Dr Robert Lustig on yt. He had many videos about sugar and how it causes inflammation to our joints and how dangerous it is to our bodies. ALSO, Dr Jason Fung (a nephrologist) is excellent on yt explaining what is healthy for our bodies. (I wrote a little bit of my story elsewhere on this comment thread if you want to hear more about me... But I have had both my hips replaced...R in 2006, L in 2016....ACL replacement in 1995. Torn R labrum surgery (in R hip BEFORE hip replacement). And 3 R knee surgeries... Lastly yikes...I broke my left femur in Dec....oof!) So I have done a lot of research on the body and bones etc. (Altho my R hip was caused by an injury and I believe my left hip was a slow progression from the R hip and my R knee affecting it over time) Anyway! I wish my doctors knew this!! I was put on every bisphosphonate at the time with no results. Then a femur fracture. My bones and teeth are ruined!!! I'm 75 and have been on pain pills for 11 years. Have had 9 kyphoplasties and need more. Thank you for this!!! Maybe it will help others!

My T score for my hip has increased since being on Prolia. The vertialible area has supposedly improved so i was put on I V zolendronate. I am 80 so i have to go with this drug. Thankyou for your information. I have had to try and find details for myself over the past four years

Thx for your work here ! So, I am a 62, female, 5'0", under 100 lbs my whole life. No dr. Will talk to me about how dexa might be flawed for extreme height. I got a t -2.8 hip and t -4.2 spine. No issues yet. No height loss yet. 2 drs. want fosamax ..another Tymlos. No celiac or heavy urine calcium. Wish i knew my bones scan at 35 ! Why can't we compare ourselves to.."ourselves"...dexas are cheap ! One retired endoc. Said i would have never been non-osteoporatic with dexa earlier... just don't weigh enough...but yet small caucasians have poor statistics...but truly, do they ? Are we sitting ducks for it ...or will we always fail the dexa and can be bone healthy...? I am traumatized. Was a gymnast even. Trying to lift heavy weights now. Thx....small people need better answers !!! Lol 😢

I agree. I am 69, 5 ft. 4 in. and weigh 113. I was told the same. I was a runner for almost a decade and then got nationally certified in yoga and taught 21 classes rach week for a decade. Then osteoporosis???? I have been frightened since I was told.

@@maryweddle4150 fear is common! We find a comprehensive plan really helps to eliminate or at least put that fear into a smaller container. A REMS can also show good bone quality despite less than ideal density. I see this often in my athletic, lean OP patients.

Did you ask for bone marker blood testing (eg., C-Pelopetide/CTX)? That might at least tell you if you are losing bone. And/or P1NP bone marker to tell if building bone.

I'm 77, osteoporosis with risk of spontaneous fractures. Alendronate, taken 3 times, gave me pain in a jaw. It was not the fault of my imagination : the doctor didn't inform me about the risk. Fortunately, it was the time for my annual visit to my dentist. He made a radio, didn't see anything wrong with my teeth. Then, I told him I took biphosphonate and he explained me the risks. I stopped taking this drug. Now, I feel I'm trapped between pest and cholera.

Hi Marie, The bisphosphonate drugs are not the only option. We recommend a comprehensive solution that may include drugs in rare circumstances but most don't need them.

@@Dr_DougLucas I’ve also had a Bone Marrow Transplant & subsequently 4 spinal fusions from that -that make it very difficult to do weight bearing exercise. My Breast Cancer was Stage 4. Thank You for your reply.

Thank you so very much for being articulated while explaining osteoporosis... Like many people, I found out that I have osteoporosis when I had a fall. While my shoulder shattered, two vertebrae fractured. Had to get a total shoulder replacement, with kyphoplasty for the vertebrae. Then the worst happened. More vertebrae fractured. One after the other. Presently, 10 fractured and 9 kyphoplasty s. Though I feel more have fractured. On the 15th, I'll be consulting a more experienced Neurosurgeon, to repair these newly fractured vertebrae. I've been researching what I need to do or take to quickly repair my bones. I'm 63 with several autoimmune diseases. I have been on low dose prednisone for decades, that helps me get out of bed. The other meds for rheumatoid arthritis just were'nt helping. So, I stopped taking them and was feeling fine with changing my diet, focusing on nutrition, vitamins and supplements. Do you know if any of the meds for RA, besides prednisone, contribute to osteoporosis?

Very important and helpful video! Thank you so much! ❤ When it comes to trust there is one problem for me, since the covid pandemic. Professionals who write all those guidelines are payed by pharma-industry. Think their decisions are hugely influenced by this. Doctors became more and more reserved or even scared to recommend a treatment which is not in line with the giudelines. And not everyone goes searching through the whole internet to find differenciating videos like your. I will. Because my trust in the medical system is gone.

I'm happy to have found your videos. I'm 74, formerly very active (walking, Pilates, taught ballet previously), but currently dealing with an atypical femur stress fx, and hoping for healing with a bone stimulator which I wear 24/7 (fingers crossed), dietary and supplement changes as well. Will stop Prolia after 5.5 yrs and go back to a bisphosphonate to combat that rebound. I wish I'd known then what I am learning now. I look forward to watching more of your videos. Thank you.

Thank you for this video, and for explaining relative vs absolute risk. No mention is made of hair loss as a side effect. I was put on oral doses of Fosomax. After 3 months, I was losing hair precipitously. If the loss continued, I would gave gone totally bald, no exaggeration. I put 2 and 2 together and just stopped taking the Fosomax. Told my doctor; she did not protest or object. I am dealing with it with calcium-vitamin D supplements, diet, weight-bearing exercise, and keeping fit, especially balance. One must fall first before bones get fractured or broken.

Thank you very much for all these interesting information. I am 49 years old and have osteoporosis. I have always regularly done sports, healthy nutrition, no smoking and no drinking. I am currently on HRT, calcium and D3. My doctor wants me to start taking raloxifen or biophosphates, I am too afraid.

The logical way to combat osteoporosis is 1. PTH firsth 2 control biologic parameters of bone metabolism. 3.If bone density has increased give biphosphonates to maintain the osseous gain.

That is logical but not globally applicable. What's the long term plan? Bisphosphonate therapy can only last 3-5 years safely (if that). This logical plan doesn't make sense for everyone and not for most of my patients.
