So.... this happened in the weekend - at my brother-in-law’s on a Sunday (physio never stops!) 🤣, but unfortunately it’s to diagnose an acute Achilles’ tendon rupture 😱
He was playing soccer yesterday and turned, felt (and heard) a pop in his left Achilles. It was “kind” of painful and he knew he had done something and he couldn’t walk properly on it.
ASSESSMENT
1️⃣ Visual loss of plantar flexion
Firstly to help with an initial provisional diagnosis without imaging you look for a loss of plantar flexion in prone. As you can see on his left his foot is resting in neutral, NOT plantar flexion meaning the calf tension on the heel is not there (the town of the calf doesn’t pull on the heel as the tendon is disconnected), compared to his right which is in plantar flexion.
2️⃣ Palpable thickening, swelling, bruising band tenderness
The Achilles is significantly thicker and grossly swollen compared to the other side. There is also a thicker section which is tender and sometimes you can get a feeling of a “gap” (the rupture site).
3️⃣ Calf Squeeze Test
To see the active connection integrity of the tendon, in prone slowly squeeze the upper calf and see if the foot naturally plantar flexes. As you can see on the left it doesn’t move at all, indicative of a complete rupture.
So, it’s rest, compression and ice if needed for pain - but he doesn’t have any of that really which is mostly due to the tendon fibres not being connected. So no pain it’s not really a good sign!
He is booked in for an Ultrasound tomorrow, and then into the foot and ankle surgeon for an assessment and a MRI after. To help him through that waiting period a long walker boot is perfect to stabilise him so he can get around.

Thanks for the demonstration. I had slight pain for a couple months and during a recent training it's got worse, I also got kind of a strange click in one leg when I step on it and when I pull my foot up like for stretching, feels something is not in the right place there :(

Subscribed. I did mine 2 days ago, waiting for a scan and full diagnosis (praying it's not a full rupture, but the pop sound was so loud people heard it 10 feet away...).

Did mine 5 weeks ago playing rugby. Honestly thought I was kicked. Felt like it, sounded like it but no one near me. No pain at all which I now realise is a bad thing. Was in a cast for two weeks now a walker boot. I have Ultrasound results on Monday and having seen this I'm expecting it to require surgery. This is the best video I've seen explaining it and I hope this was followed up cause I'll be watching!

Just happened to me last night in my mens soccer league. Thought I got clipped from behind running at a defender with the ball, but there was no one behind me within a 10-15 yard radius. Felt like a shotgun went off inside my leg and I got sniped while running. Long road to recovery begins today. :/

Sucks I just ruptured mine 4 days ago, left foot playing basketball, everything you said is making my body twitchy cause that's what it felt like and sounded like, I'm to scared of surgery so I'm in a cast and praying for a recovery, I'm hoping by now dudes ankle is better, I'm 6ft so I can only imagine his pain and shock if he felt any, thanks for the video

@@Guest-ih7ig just trying to keep off it, I get the cast taken off Tuesday and see what the Doc says, I can still feel the spot of the injury but is not as sharp anymore, but just feeling it makes me think about that moment and that makes me nervous a bit to start rehab, but I'm also looking forward to rehab cause my body is so tired from doing nothing, "I'm in a glass case of emotions", but I'm so lucky my wife and 5yr old are being so helpful its gonna be hard to go back to being the work horse lol, thanks for asking btw

@@Alex-oj9xp I start rehab in January, I have most of my mobility back but I can't do any single toe ups with the injured foot, I'm out of all boots and casts thankfully, using heel lifts and trying to walk as much as possible while doing my exercises I've learned from helpful videos, I have a slight limp from not being able to lift with the ball as high, really hoping rehab gets me back to being physical cause it's a tiny,...tiny tiny bummer where I'm at I feel with the process, but it's a long process and I just gotta keep remembering that, I do appreciate you checking up like this, I try not to complain cause life could be so much worse so again many thanks for attention and concern, I hope all is well with you in yours.

I did mine playing football (what we call soccer in Scotland) yesterday. I too thought someone had kicked me, but no one was near me. I collapsed in front of a 16 stone teammate who then kneed me painfully in the back - that hurt more than my achilles! No surgery, but moon boot for 6 to 8 weeks! Good luck to your brother in law.

I we wonder why some people have to get surgery while others don’t, I assume getting surgery is worst and longer healing time, this happen to my husband a few days ago and was told he need surgery, I think he should try to hear without surgery 😒

@@physiorehab Yes it's going on very well, walking around in a VACOped that's been given to me by the hospital and gonna have incremental changes to the angle I'm walking on. Dreading the lengthy recovery, but I'm happy after 2.5 weeks since my injury that I am mobile again without crutches

I’m on day 17 haha. Just got a call from my ortho to schedule my surgery. I’ve been walking on it/using a cane and a knee rover for work. Brutal stuff but I didn’t know it was this bad

@@LouisianaPwnsUrMom Are you in a cast? The way the doctor put my cast on in the hospital makes it super hard to walk on it. I was walking on it on day one. Going to find out if I need surgery come Friday. I took the week off from work but would like to go back in and finish the school year off since I’m a teacher. Brutal stuff, but if Kobe can do it and bounce back, we will too. I suffered it playing football, right after I scored a touchdown too. The worst luck!

@@AceGrahamJB5 sorry for the late reply. I wasn’t in a cast when I was walking on it. My ortho wanted me in a boot but I had some busy week ahead at work so I decided to skip the boot and just schedule surgery. Surgery went well, I was in a split/cast for a week and a half or so and this Tuesday I got put into the boot with heel lifts. Im doing half weight now and hopefully will recover quicker than 10-12 weeks. It’s really a pain but just as you said, Kobe did it, we can too.

@@AceGrahamJB5 hope all’s well with your Achilles bud. I’m two weeks into physical therapy now and the difference is outstanding. I have decent motion in my foot now, but I can only walk at about 1 mph at 50% weight barring on a weight regulating treadmill. I’m still sleeping with the boot but mostly keep it off through the day when I’m not doing anything and my PT put heel lifts in one of my shoes so I use that if I’m just going to the store or something while using crutches. Still a long way to go to walk normally but when the progress starts it’s unreal. After surgery and first time I actually saw my ankle I didn’t know how my foot was so stiff and if I’d even be able to move it properly again but it comes step by step. Pun intended

1.5 Years after my rupture I am pretty much healed. Still missing some musclestrength in that calf but I am back to sports and working on it. I did the non-surgery route and I am very happy with it. 8 weeks in the plastic boot (started rehab every day since week 4, with phisioprogram), even slept in boot. 4 weeks with 24/7 shoes with heel lift and from there on slowly getting in to normal shoes and recovering life.

Happened to me a few days ago playing football, already went through surgery and currently recovering at home (still trying to figure out who was the bastard that shoot me in the leg 😅). They did the same test to me, squeezing the leg, as a first diagnose. Thanks to portuguese NHS it costed me zero euros. Thanks for the explanation, it’s really enlightening. Cheers!

@@brax5921 I'm doing great, thanks for asking! Apart from a really small scar, I'm feeling the same as before the injury. I never played football again, but I do functional training twice a week (cardio and body strength) without limitations. I lost some muscle in the left calf that I was unable to get back 100%, but it's only noticeable with palpation, in terms of feeling and strength, it's almost similar to the right one. Are you facing the same recovery now?

@@TheMacBallister ​​⁠hi, that’s great to hear, I think I might have partially torn or fully done mine on Sunday whilst playing I didn’t hear a pop I just suddenly fell . When it got checked out the doctor was suprised I was able to move my foot she said normally you shouldn’t be able to. Regardless I’ve been put in a cast and currently being referred to a clinic whilst I’m waiting for a scan to determine hat really happened. They’re saying that surgery is not needed so I’m going the conservstive way. How long did it take you to fully recover or to get walking

Hey, ruptured my right achilles two weeks ago. The boot was causing a lot of swelling, pain and discomfort from the heat. Found that removing the boot and resting the foot angled down and sideways on a pillow when working during the day eliminated all these issues. However, slightly unsure if this is okej or if it may affect the healing process, could not find anything on it online. So, my question is, is this a sensible thing to do. I'm still using the boot when sleeping and when moving around. Thanks

When I start physical therapy on my Achilles rupture, is it now time to ease off the boot a little bit? My boot has been causing me more pain in my calf. I was warned about the blood clot situation. I feel like the boot is making me immobile and stiff every time I take it out to work out.

Tore my Achilles back in 2017 playing ball when I was 18.I went the non surgical way which took a little longer to heal but now I’m back to normal.I didn’t do the surgery way because risk of infection

@@cs72799 it took me 8-9 months completely heal.after getting of the boot for a month and a half I had a small limp walking. Wish u a speedy recovery 🙏

@@youngcruz2709 question on what an achilles injury feels like because I think I may have injured mine....First... 1. When I put my leg out completely straight and try to bend my toes back towards my shin, I get excruciating pain in the bottom of my heel like it's knives or needles in there shredding my ligaments and tissues. 2. When I use my fingers to pinch the back of my Achilles Tendon on a certain spot, I feel the pain 2 inches down at the bottom of my heel (it's on the outer edge of my achilles tendon of left leg and pain travels down to outer edge of left heel 2 inches). Is this an achilles tear?

What would a work out routine look like for upper body while dealing with Achilles Tendon rupture? Ruptured mine last Thursday and I can still walk (I do have boot) and MRI/Podiatrist confirmed need for surgery. I have bench, powerblocks, bands, etc. at my house and 24HR fitness membership...just really don't want to lose everything for 6 mos while recovering. I get that lower body is basically but any thoughts on upperbody routine? Thanks.

I was only able to see a PA for my injury for the moment. She insists that mine is only a sprain, but she can't explain the popping noise I heard. Also, I'm a former athlete and current coach, so I'm no stranger to injuries. Honestly, my heel and achilles looks just like your brother in law 8 days after my injury took place. Really considering a second opinion. The pain is waking me up in the night.

My Achilles is so bad that I can’t walk when my body has rested. When I’m walking it running, it still hurts but it’s manageable ( some what). It’s one of the worst pains ever!

I just discovered this video. I was curious to know what would happen if he was to contract the calf muscles (so as to try and move the foot in a "tiptoe" position), the foot would stay in place while contracting the calf, right?

Hey Tim, im pretty much in the same boat as your brother, 2cm gap, and want to go conservative approach. Had boot with wedges on within 30min of injury. I had left Achilles ruptured in 2017 and that was extreme, so needed surgery. Question is, can i continue to put weight on foot in the boot with wedges? Or non weight?

I partially tore mine in ballet. It sounded like jeans tearing or a huge zipper zipping. I didn’t feel pain, I actually checked my pants to see if I tore something. I have to be in a boot for a month.

I totally ruptured mine 6wks ago, was demonstrating a triple jump, I heard no pop, I dont really know what happened, weird I simply have no memory other than I hit the deck, grabbed my leg and saw the deformity 🤯 big cast on that night then into fibre glass 3 days later. Of all my sporting rehabs....this so far has been the toughest in terms of time without independence, I have a manual car and a left leg rupture...its alot.

Had Achilles tendon surgery full rupture pain after surgery is so bad. Now I am in PT it unbelievable pain.... I am 12 weeks post surgery. Been very rough

Im on day 2 and I hate this soooo much as this is my second time having this injury but on the opposite leg. I was playing basketball and went to run and I heard it and felt it and I knew right away what happened. Makes me so angry because this is gonna be a long process again. Probably won’t play any sports anymore but will try to stay active in the gym.

I have a slight indentation/gap in my left leg/calf insertion point but still can point my toes and stand on my tippy toes? I rolled my ankle laterally and it hurt but I felt no calf pain and still no heel pain to this day and it’s been about a month. I heard a snap but it was more like an ankle roll. Had some swelling for about a week but I was back to playing basketball after that week with no problem. I pass the Thompson test, and have still been able to run and jump fine with minimal soreness after. Why is there a small gap when I point my toes? what’s going on?

Mines been hurting for weeks. When I go to step up on something it has a really weird feeling. I can walk, but limp sometimes. Wondering if my hamstring stretches could of hurt me? It’s non unbearable, but it’s not good either

The test is for a rupture. If it’s been repaired you don’t need to do it

Just tore mine playing basketball. Felt a loud pop and it also felt like I got kicked. 3 weeks in a splint, non weight bearing, 8 weeks walking boot. 6 months before I can think about being active again doc says.

Just ruptured mine on Saturday playing football, sprinting then thought I'd be kicked but no one near me.. back to hospital yesterday as they put an ankle boot on my by mistake. Confirmed 5cm gap between tendon so moonboot but no surgery. I'm away on holiday in 6 weeks to America and usually drive but its my right leg so can't... how long as others took to be walking without moon boot as I'm going to miss out big style.

5 cm is a big gap I suggest you get that reassessed in four weeks to make sure that it is going to heal well. if you’re doing OK I suggest you follow my achilles rehab program at physiorehab.com

I have a slight indentation/gap in my left leg/calf insertion point but still can point my toes and stand on my tippy toes? I rolled my ankle laterally and it hurt but I felt no calf pain and still no heel pain to this day and it’s been about a month. I heard a snap but it was more like an ankle roll. Had some swelling for about a week but I was back to playing basketball after that week with no problem. I pass the Thompson test, and have still been able to run and jump fine with minimal soreness after. Why is there a small gap when I point my toes? what’s going on?

Could still be a muscle tear, but if you’ve strengthened it up and it’s functional, then just be guided by your symptoms if in doubt, get it, assessed by a physiotherapist

Just joined the club 🥲 Some kind of rupture while playing my second game of Pickleball (2nd day). The day before, I had played for the first time, and we were there for a few hours. I was sore in some little areas the next day but I didn’t think it was much of anything knowing we would play again. I’m 30 years old, strong and healthy. For example, I’ve gotten my deadlifts up to 455, I can hit 50 pushups in one go, close to 20 pull-ups in on go, throw heavy kettlebells around, I’ve been running 5-6 miles once or twice weekly, I’ve even worked up to run 12 miles straight at a low running pace, several times. I’ve competed in two 10ks this year a few months apart. The most recent was Memorial Day weekend. Since then I had barely worked out much or did any running. Just a variety of upperbody exercises and movements. So I can’t say it was strained because of all that prior training. But the inactivity and then partaking in a new type of lowerbody exertion, then once more without properly allowing my body (tendon) to fully recover, then allowed that one foot placement to rupture my Achilles. I can’t say I really warmed up specifically, we kinda just eased into playing a lightly competitive game. Then the second game right after, like 5 minutes in…. It felt like what I imagined to be a high speed baseball hitting me between my heel and calve. This is wild! For my overall strength and ability to move the way I can, I’m rather flexible, all around good movement. It’s funny and so annoying to accept that this happened but it can make sense too. I think this was one of those “freak accidents” mostly. I’m not necessarily use to the type of movement that I exerted yesterday for those few hours or the first two games played today before the injury. So it’s possible the new activity recently could have caused unknown strain that was then completely exposed by an improper movement, weight distribution, and flexion of my right Achilles on that 2nd day of Pickleball. I’ve had a knee and ankle injury before but this seems much more debilitating, it’s almost unbelievable to think that it actually happened during frickin Pickleball of all things 😂 LONG STORY SHORTER… I’ll be back! And I’m going to use this as a learning experience in so many ways. Things happen as they do. We cannot control everything in our life, but we can live in its presence. Life isn’t controlled it lives. Get knocked on your ass 5 times… GET UP 6!

I had surgery on my Achilles tendon 6 weeks ago after a complete tear. My wife did the Thomspon test on me today. My foot did not move. Is it possible that the suture did not hold and the tendon ruptured again or is it due to the shrunken calf muscle?

Hi Dan, did mine 2 days ago here and also based in the UK. I'm waiting for diagnosis. From what I have read (and as Tim has said) if the gap is more than 2cm surgery can often make sense. I also read that for young active people, surgery can offer better outcomes (re-rupture rate is marginally lower after surgery vs. non surgery). I would speak to the doc to understand why, but also trust them as they wouldn't be putting you in surgery now for no good reason (big backlog due to covid).

I blacked out for a moment when I did mine playing basketball. Felt like someone knocked me with a sledgehammer in the back of the leg, woke up, looked around, tried to step forward and couldn't. Wouldn't wish that pain on anyone.

When i squeeze my calf, my foot still moves, But the area where the Achilles tendon is located has become thicker than the other leg. And when I do any calf workout or leg workout then I feel more in the injury leg calf, so do I need to have surgery? But if not, then how much time it's gonna take to recover me with a non-surgical process, I'm a sports person so I want to recover as soon as possible

This is really intreasting I ruptured mine 8 weeks ago but I'm in the UK and my treatment was very different , I was placed in a cast for a week with my toes pointing down then fitted with a vacoped boot which was gradually adjusted and I wasn't allowed to weight bear for 3-4 weeks after the injury. I'm still not allowed to drive either even though I don't need the leg for driving.

This approach is also common in Sweden. If you are not an elite athlete, it seems like golden standard now is conservative, cast-treatment. I had that myself in 2021. Fully restored now. 40+ male that Will not do anymore comback in Highjump 😂

HELP ME 😢😢 FOR 1 Yr iv not been able to go on tiptoe at all on my right foot. Includes swollen achilles tendon 😢. A Yr ago while dancing at a gig I felt like a massive kick from behind?? Does this mean it's totally snapped or partially torn please? When I walk its painful.

Did mine playing tennis a few hours ago. Thought someone had shot me. I Howard the pop. Very painful. It’s 3 hours later still hurting but less. Using ice. See orthopedic tomorrow. , so if it was very painful that is better than if it hurts less?” Wishful thinking?

Hello, i have a few questions. I tripped and i heard a pop and then felt the worst pain of my life in the arch of my foot and heel. It was the most intense burning and pain ive ever felt. Now my ankle heel and all around it is bruising and swelling pretty bad. Its turning purple on my arch near my heel towards the top side of my foot and i can kinda feel something poking out kinda. Its definately not a bone though. Its hard to walk, im trying to limp, but OMG. Can you please shed a lil light on my situation for me please please.

@@Tipofthecaptomelady hello, my prognosis is good. I was in a hard wrapped splint for 3 weeks until Sept 3. I’ve been in the boot since. My latest PT visit has me now with one heel lift only in the boot and walking with increasing weight on the leg in the boot with crunches minimally. PT said I can put weight on it as comfortable but must use the boot to prevent any sudden movement which would stretch it.

I ruptured my right Achilles and when I tried to stand my leg collapsed and could take no weight on it at all , I was put in a thigh high half cast with my toes pointing down as far as possible due to some other issues the surgeon told me surgery was not worthwhile as the risk of infection was higher and instead I was put in a moon boot with heal risers and over a period of 6 months slowly lowered the risers this was over a decade ago and I still have issues and can no longer run to add to that I have a soft spot in my left Achilles that may let go if put under to much stress .... Ankylosing Spondylitis is what is causing many issues with both my Achilles still .... it really was an unpleasant experience

That sounds like you may have an Achilles tendinitis and the thickening is probably inflammation I suggest you either get that assist treated or at least have a look at my Achillesrehab program physiorehab.com

​@@elijahhelton3403 I was given a cloth brace that fits over my entire foot and goes up the leg about 10 inches. It's like putting on a enforced sock with laces, and velcro to tighten it up so that it is snug to the foot. There is also a hole for the toes so that they are exposed. Basically it's for stabilizing the ankle. I used this for about 7-10 days, but I was recovering from a rupture, not a tear. My tendon separated completely. Wearing this allowed me to have confidence when I started to gear weight on my foot, I advise that you ask about it if you don't have it already. I also used 1 crutch for the 1st 5 days, not sure if I really needed it, but for me I just wanted to give this repair a bit more time to recover before I put the full weight on it. It was also at that point that I started in on therapy. If you want to get back the usage of that foot, you'll have to do those exercises like they tell you at home. I am fortunate to have a treadmill at home, and that helped me immensely. 3 months now from surgery and I am able to walk fairly well. My issue is that my heal is still sore from the tendon, and my toes hurt when I use the full extension when walking. However, I think that I injured my toes when I was getting up one day from a knee down position. I pushed off my toes forgetting that they have not been used in 2 months and I think I sprained them, doctor agrees. Feel free to ask any other pointed question, best of luck.
