Sans lost his bad time eye when undyne tried to save alphys when she failed to commit suicide after rebuilding mettaton and undyne used his eye to help alphys and he killed a gaurd and took its eye and the blood coming from his skull after the incident turned the eye blood red idk what happened to pap though mabye somehow he survived an attack from frisk but this is long after the neutral run occurred. If you don’t know why horrortale even exist It was basically a natural route of the normal undertale where frisk kills asgore and fights omega flowey. After that toriel came back to take queen and promised to keep every human safe and treated like other monsters. But the monsters didn’t like that and overpowered her and elected Undyne to be queen. Undyne most of the time didn’t make any mistakes so it was smooth sailing the underground. Until disaster strikes, the waterfall water had been poisoned, the core ran out of power, and multiple monsters either lost their lives or went insane. Undyne also wanted every human to fall down here to be killed on sight and monsters did that with no hesitation. Now sans tried to stop undyne to fix the situation but undyne denied and stabbed a spear in his skull. And there is more but I have already “yapped” enough. Thank you all for reading!

Actually the hole in his skull isn't from Undyne, the bear is just crazy. He got it when Alphys and Undyne were extracting his eye and it ended up blowing a hole in his head.
