The second incident seems to have been cut off before a proper explanation was given. The right engine failed, but the co-pilot misidentified it and told the captain the left engine was the one that failed. So they kept trying to use the damaged right engine to keep the plane in the air, while running the perfectly fine left engine at idle. The captain was preoccupied with the ATC communications and didn't put aside time to assess the situation (the ATC comms were pretty full-on), and didn't even get to complete that failure checklist they mentioned. Hence the crew management failure black mark. Also the co-pilot only received minor injuries.

This sounds like TransAsia Airways 235 where the crew misidentified the problem and shut down their only running engine.

No mention of engine failures in report ? Seems the ATC kept asking the same questions, even as she confirmed she had a good understanding. How many times did she ask how many on board. ?

@@donfisher8035 "The teardown of the right engine showed that two high-pressure turbine stage 1 blades were missing their outer spans... The blade failures caused downstream (secondary) damage to the low-pressure turbine, resulting in a loss of thrust..." All there in the report.

ATC in Honolulu was number one. So clear and emotional, the dialogue with the pilot seems taken from a movie. I'm glad the two guys survived. The two guys that flown a 45yo 737 as daily routine, just to remember.

​@ko7577The engine failure was attributed to oxidation and corossion within two engine high-pressure turbine blades causing stress fractures and subsequent breakage. The breakage caused secondary damage to the low-pressure turbine resulting in loss of thrust. The pilots correctly identified a loss of thrust in the #2 (right) engine at the time of takeoff.

they did said it could be volcanic ash but still have a ball to go through rather than change their direction, i dont know how airliner thinks can go full daredavil rather then use some fuel for direction change

Volcanic ash apparently doesn't show on radar because there is no moisture in it to reflect the radar signal back to the receiver.

Sadly, the quality of the videos has been on a decline over the past few months. They feel rushed, endings are too abrupt, the explanations of accident causes are way too brief, editing became sloppy, subtitles are riddled with typos or distractingly illegible. I hope this channel recovers.

I agree, I've recently been rewatching older videos and the quality certainly used to be higher. The editing and overall production used to be much better that it is now. I still watch almost every upload, but there has been a noticable change in quality. I've no idea what the cause is and it's not for me to speculate on that, it could be personal issues that are obviously none of my business.

It's folks like you that'd just let it die. Many comments here today speaking of the same defects that normally do not plague this otherwise great channel.

I agree! Allec Ibay's channel explains every detail, almost to a fault! AND he outlines his white lettering in black so you can see the letters no matter the background. This channel's quality has been lackluster lately and this video proves that perfectly.

Finally, a story with a much happier ending. I know that planes crash with all souls lost; but, there should be MORE stories where the people survived. After a while, it's so depressing to read story after story where there were no survivors. I can't tell you how happy this story made me feel.

Is there something personal going on with the flight channel guy? If thats the case then just let people know im sure they would understand. The recent videos just seem to be older episodes combined and put into a "new" video. Also the text needs a shadow or making black if its going to be on a white cloudy background. I feel like in the past this would never have been looked over. Also the video cuts off before the full explanation is given.

If I had a channel and people believed the quality had gone down, I'd want to hear those views. Your subscribers and audience are in essence your customers. The thumbnail is an attention-grabbing one that appears to show the plane splashing into the ocean, building on the title of "Falling Fast into the Ocean", when in fact neither is true. The plane was never "falling". It was a huge glider once the engines failed, and the pilots' ability to land the plane-on an island hundreds of miles from any mainland *and* effectively level, not pointed down at a massive angle as shown in the thumbnail-is one of the greatest heroic efforts in modern aviation history.

Green Dot Aviation is an up and coming flight channel which I have gravitated to. Hard working owner with a great narration voice and good detail. I suppose owners of established channels either sell them to people trying to cash in and not uphold the quality, or the owners just lose their interest and passion a bit. It's kind of understandable, people's lives change.

yep, Green Dot Aviation is a good one. I also watch Mauricio PC, it's in spanish but somehow I still can follow. And Mauricio's english channel, MPC flight recreation, not updated as often but still great. He covers accidents that are not always so well known.. And of course I follow Chloe on Disaster Breakdown :)

Your videos are usually well produced...this one has not lived up to your usual quality. Abrupt end to the video/not your usual outro. Also, you need to do what other videos of the same genre do and outline your white letters with a black outline, that way no matter the background the words can be seen. Can't see white letters against clouds in the sky! You guys are better than this 😮

I remember there being a lot more to the accident in the waters just outside of Honolulu. Wasn't there an actual problem with the plane due to its age? Something to that effect? The pilots then mismanaged the handling of the incident. Poor CRM. (And there was A LOT more regarding what caused a KLM plane to fly directly into a plume of volcanic ash! The story of that plane's survival is truly remarkable!)

The controller did not understand that there was an emergency because the pilot did not use the codeword for emergency, which in the US is "We do not wish to declare at this time."

The KLM flight disregarded controller warnings and a NOTAM specifically warning them not to fly thru the ash plume. You forgot to cover that part.

For the second accident: What do you mean by poor CRM? The pilots did well under the circumstances. They didn't have the altitude to work with.

part of the story is missing - according to the thread started by @shaydeNZ - "The right engine failed, but the co-pilot misidentified it and told the captain the left engine was the one that failed. So they kept trying to use the damaged right engine to keep the plane in the air, while running the perfectly fine left engine at idle."

I was a passenger on a Flying Tigers cargo jet that landed in Anchorage shortly after this flight. We all knew what happened. I’ve got more lives than a cat. That could have been us.

May I suggest you change the colour of the text in your videos because quite a few times it's impossible to read when its against the sky background because both the clouds and the text is white.

I guess the channel creator(s) are going through some difficulties. My suggestion would be to reduce the frequency of uploads if it helps maintain the quality. Make a declaration video on that so the viewers know. People watch you for the quality and effort you put into the videos, they would not mind fewer videos I think.

Suggestion: Next video, Maybe you should try xplane instead. Because xplane has better crash effects, Like engine fires and etc, MSFS doesn't have those. Maybe you can try xplane for a bit and if you like it you can move there
