Thank you! The side effects are often either unexplained or as a result of calcium levels. The latter need to be evaluated so make sure your doctor knows that you are having them. Otherwise, there isn’t much to say about them.

How do you feel? I will start Tymlos soon and in afraid, because after taking Boniva for my Osteoporosis, both of my femurs broke just by walking so now my osteoporosis is worst

This was a helpful episode for me, and reaffirmed that I made the correct decision in my treatment. I had terrible scores per bone density scan in my early 60s. I was on Tymlos for 20 months. Dr. wanted me to start Reclast when I refused, he placed me on Fosamax. I took it for several months. I did not tolerate well, urinary issues . I find very little research about this side effect. My symptoms resolved after I discontinued the drug. I now take Calcium/Vitamin D/ Vitamin K. I am more aware of my dietary intake of Calcium and I exercise. I wonder if I should even get another bone density if I don't intend to take any prescription medication. My mom is 90, and she took Fosamax for a short time 20 + years ago. Now, she takes supplements.

Thank you. I have just been prescribed Tymlos. I’m a 58 year old woman who went through menopause at age 42, had a combo of osteopenia and osteoporosis since then. I have been trying to get a Dr. To help me with this and all I got was exercise, take vitamin D & calcium. I was prescribed fosomax for a few years when I turned 48. In 2022 I got 2 compression fractures in my thoracic spine and have has severe pain since then. I started to see a rheumatologist and he has been so helpful. He told me he has not seen a case of osteoporosis so severe in someone my age. Had I known it was severe I probably would have been more careful, I had no idea I was at such a risk even though I had tried to get a Dr. to address this. It has taken my insurance company more than a month to approve of the medication.

THANK YOU VERY much for this post...it doesn't definitively determine my path, but it DOES SUBSTANTIALLY decrease my fear of negatively effecting my bone metabolism in the future IF I end up choosing the Forteo Rx recommended by my bone health physican to get closer to normal: I am 45, been surgically menopausal since 37, have a history of prednisone for RRMS & sarcoidosis, & a current spine DEXA of -2.7 L4 at -3.2 😞, while hips & femoral neck are still osteopenic. I have the potential opportunity to REVERSE menopause through preserved ovarian tissue transplant, so there's A LOT of options on my plate to choose from... very blessed, but stressed to make the best decision. EXTREMEY thankful for your ACCESSIBLE expertise!

I have cerebral palsy (59F, ambulatory with DME). Diagnosed with osteopenia in my 40's. Oral meds and Reclast no help once I hit menopause. R hip AVN. Have been on Forteo for 13 months so far. Recent DXA showed big BMD gains in L femoral neck, overall hip and lumbar spine. Side effects limited to mild leg cramps on occasion. Supplement with Calcium and D3, also take Prempro. Periodic blood draws include checks of my calcium and D3 levels. In PT regularly.

I was on Tymlos for two years and it raised my numbers from -3.75 on the DEXA scan to -3.50. It doesn't seem like that was very much.

No. That is certainly not the outcome that I would be looking for! The question I would want to answer is why are you continuing to lose bone or is that even the problem. The balance of bone metabolism is complex and numerous variables need to be considered.

@@susanne4168 Calcium levels are the only common side effect. The osteosarcoma scare is covered in this video and I don't think it is a real concern.

My Dr is considering giving me this drug. This content is very useful to me as I don't like to take drugs that I don't know what they do. Not happy to take Alendronic Acid. Need meaningful bone density. Thank you so much

Hi Dr. Lucas - so grateful to have found you. Great niche - and much needed. I will be joining your master class at a minimum. I'm 58 yo male, have a dexa of around -3.9 in the spine, I sustained 3-4 fractures, the latest one without trauma, diet and exercise is good. Extensive bloodwork is normal, so no root cause was found - a diagnosis of no diagnosis - idiopathic. I'm working with my naturapth on gut and metals analysis. My doctor wanted to prescribe forteo, but when she found out I had a high-grade papillary urothelial carcinoma lesion (localized and hadn't spread) she recommended a bisphosphonate ( IV zoledronic acid) with the theoretical risk that increasing bone turnover could increase the risk for cancerous mutations to occur in bone cells. Not looking for medical advice here, but curious if your research on cancer and forteo found evidence to support that risk?

I had taken bisphosmatmates for years (Fosamax then Boniva) the switched to Prolia and had been on that for8 years. I suffered and Atypical Femur Fracture which was surgically repaired in November 2023. I am 74 years old and my endocrinologist started me on Tymlos this week but wants to switch me to Forteo in a month. Right now I am concerned about the side effects of each (which one is tolerated better) and what happens at the ed of 2 years.

Great information. I was just given a list with these two drugs (and Evenity) on the list and thankfully you have videos on all. Don't know that's the path I want so time for more research! Good evening. I tried to subscribe to master class. The notice says under construction and the site is not safe. My doctor put me on to forteo Which I was so afraid to take but after listening to this video I am a bit more settled . I have lupus. For plus 20 years I was on prednisone. I have multiple spine fracture and oesteoporis on my spine and both hips. I struggle to get out of bed as my tummy hurts after the fracture and I can only walk short distances. After listening to you for a week now. I am trying weights. I struggle because of exaustion and the Dr took me of prednisone. I am on plasmaquine and mycocept 500. I upped your recommended nurients. My gut is always an issue.. Eating is a struggle for me. And it isyour videos that gives me encouragement. My iron are always low. And I am 59 years old from South Africa. Thank you for your so positive advises

Thanks for your video! I’m 57, and my Dexa shows a -4 in my L3 and L4. My mother and her mother had osteoporosis. I work out, eat well and take quality supplements. All of my blood tests are normal. My CTX is 396 and my P1NP is 48. At what point do I say to myself that my genetics is playing the biggest part in my osteoporosis and take Tymlos (which is what my doctor is recommending).

Thank you so much! I just had a DXA yesterday, and was so disappointed that my numbers went down in the last 2 years (2022: -3.3 spine; -4.5 hip to yesterday, 2024: -3.4 spine; -5 hip)!! And this is despite EVERYTHING I've been doing for a year (a lot of resistance training, SUPER clean, high-quality animal protein, fairly low carb diet, no sugar or refined crap,11K steps a day, vibration).. plus I've had great numbers for D for many years (supplementing with D3, mag, MK7, Omega 3 for over 15 years), etc. After seeing my numbers yesterday, I feel like I had the wind knocked out of my sails, as I thought my numbers might not go up, but at least not go down that much (well, the spine didn't go down by much, but my hips sure did!!)... Now considering a drug, I guess. I'm 65 and have never fractured a bone in my body. I'm tiny though (4'10") and under 80 lbs - so also hope the DXA shows it's worse than it actually is.

Doctor, I am on Foteo for 7 months and stopped. I got really sick in the hospital for 8 days with pancreatitis because of Foteo side effects. I am 63 years old.

I will be prescribed Tymlos starting this next month. My rheumatologist recommending one year Tymlos, then one year Prolea, then one year Reclast. I’m terrified. Thank you for doing the videos. I wish I had more confidence in my treatment plan since Reclast falls into one of the worst drugs for osteoporosis.

YIKES. I'd be scared too. Why not two years of Tymlos while doing HRT (see Dr. Doug's videos) plus other supplements and weight bearing exercise (not walking), impact if you can, and skip the Prolia and Reclast. Have you downloaded his workbook? It's free, and outlines many of the things he uses.

@@IndydiMy rheumatologist claims that I have to take a bone stabilizer like Prolia or Reclast after finishing Tymlos or my bones will decline even faster. I have a follow up with him in December so will pose this question at that time. Thanks for the suggestion!

@@misiuhagen4926 Dr. Doug has fantastic success with his patients, and he doesn't use Prolia. He's gotten me off Prolia, which involves an intensive regimen. It's not a good drug. You can do an anabolic and then all the other supportive things Doug does without using any drugs that suppress bone turnover. Don't let your doctor do something you're not comfortable with! Your gut instinct is right! My rheumatologist was handling my osteoporosis, and I finally decided that even tho he's a good doctor, he didn't know enough about what he was doing to manage my coming off Prolia. That's why I got involved with Doug's team. But he has a ton of free resources. You don't have to be an actual patient to benefit from his wealth of knowledge.

Dr. Lucas, wonder if you can do a review of Aclasta. As I turned down Prolia injection, the endocrinologist (my doctor referred me to) suggested Aclasta infusion for me. I read negative side effects of this drug. Thank you for making videos on osteoporosis.

Hi. Newly diagnoses 50 y.o. surgical menopause years ago. No hormones. T score - 4 in spine. Suggested not to do natural and jump straight to Forteo? Not sure about this. Also, told I would need to take calcium with forteo since either is a builder even though my calcium is normal.. what do you think?

Now that the FDA has removed the 2 year limit for the use of Forteo, is it theoretically possible that indefinite use of Forteo can full restore bones? It seems to follow that if it stimulates bone regrowth that regrowth will continue as long as it is used - no?

Hello, This was changed in 2020 but I'm not aware of any changes in prescribing patterns. There are still no intervention studies longer than 2 years and insurance may also play an issue. It's an interesting idea but I'm not aware of any planned studies to evaluate prolonged use.

I'm a 54 year-old male who has been dealing with osteopenia for 20 years. Started with lifestyle modifications and then took a 5-year bout of Fosomax.. My numbers are holding steady, but now we are considering Forteo of Prolia. Most studies are based on women with osteoporsis, and I am not in this category. Would a consultation be reasonable for me? I have faith in my endocrinologist, but I must admit I am still nervous about more drugs.

I’ve been diagnosed with osteoporosis and have a small wedge deformity in my spine. My endocrinologist wants to start a bone builder med but says it needs to be followed by something like reclast or the bone builder won’t really work. First we are doing a work up to see if there is a cause for my osteoporosis. I’m 58

thank you for this very imformative video. I was recently diagnosed with osteoporosis with a Tscore of -3.2 Im a post menopausal 5'3", 120 pd 54 years old, fitness instructor, eat healthy, active individual. My Dr. wants to start me on Tymlos, but Iam scared to death. I hardly even take something like tylenol,and prefer a natural approach to healthcare. Hearing that you sometimes use this drugs does make me feel better about taking it, although I have not decided if this is the route I will take. Maybe I should reach out to you and your team for other alternatives??

I have had parathyroid tumors (2) removed in 2018. Prior to that I was diagnosed with osteoporosis by dexa scan in my late 40s. Long family history of osteoporosis and I’m a small, low weight lady. I’ve read your book. Sounds like anabolic drugs are your preferred pharmaceutical when drugs are required. I don’t want to take drugs. Seen the results from my mother. Really no help to her and still broken bones. With lots of stomach complaints. I have changed my diet to almost total carnivore/keto. Taking all the right supplements per your videos. Stopped calcium after all the negatives. Would you consider former parathyroid tumor patients ok to take Forteo or Tymlos? I have my PTH checked annually by endocrinologist to make sure I don’t get more parathyroid tumors. So I’m quite worried to not go there again. Endocrinologist pushes me all the time to do drugs. Thoughts on my situation?.

Hello, I can’t give specific medical advice but there is no contraindication that I’m aware of for prior PTH tumors and synthetic PTH analogues. You’d have to check with your team for your unique case.

Hi Kim, I also had two PTH tumors removed in 2015 and was diagnosed then with OP, I was only 47 at the time and have been doing hormone therapy to halt the bone loss. Same happens with me, my OP clinic constantly pushing drugs and I give them the same answer each year.... NO!

Thank you for reaching out. So currently I have decided to get off the synthetics and now on the estradiol patch and BIO-ID progesterone. I even ordered a Menopause panel on my own via LabCorp to monitor estradiol levels to make sure I'm getting enough to cover bone loss. The Bioidentical hormones were going to cost $3000/year. I decided to do patch/pill treatment temporarily until my appointment with a Functional Medicine Doctor in January. It will and was too difficult to stop my current hormone therapy cold turkey to pursue Bioidentical at this time and the effect it had on my body was rough. I will revisit in a few months. Every time I exercise I feel much worse. Dr said don't do anything that makes me feel worse. Due to needing and SI operation the doctors wont do with out 9 months of Forteo. I hope the drugs work without exercise.

Exercise is the key to longevity. I have plenty of patients that have limitations. We help them to figure out what they can do rather than focusing on what they can’t do.

I have 5 compression fractures, T10, 11, 12, L1,2(L4 was had bone cement). I was taking Letrozole, after breast cancer. After fracture I was taken off Letrozole. I am waiting a couple months to see if any of the fractures heal by itself. What kind of medication would you recommend?

I started on Tymlos due to having severe osteoporosis and multiple fractures. However, I quickly developed hives and had to stop. Do you know if there is an increased risk of being allergic to Forteo as well?

I have stage 3/4(I go between the two all the time)…I have hyperparathyroidism. I am currently taking 20 mcg of calcitriol. It has brought it mostly under control, but my numbers have gone from 1000 to 70-90. Kidney disease was diagnosed as late stage/end stage 10 years ago. I have improved on a pretty much carnivore diet, and recently added MK-7 at 200 mcg per day. My kidney function continues to improve, but my PTH numbers remain high. I don’t know what to do. I also have MS. And really bad arthritis in my lumbar spine.

If our patients have subtle elevated PTH we first look at Vit D. If still elevated after correction refer for eval for PTH secreting tumor.

I have taken Movymia( I am told its the European alternative to Forteo) for 18 months, I do suspect a continued bone loss. Also I am hesitating to continue taking Movymia through the winter, as last winter was very difficult with stiff legs, joint pain etc. The doctor suggests a yearly Reclast injection, which I told him I dont want. I would think an overall assessment of the bones would be in place before making any further decisions. Whats your take Dr Lucas?

Newly diagnosed osteoporosis less than a month ago I do not have a thyroid or parathyroids had them removed because of cancer 30 years ago so if I do not have parathyroids, I’m not producing any of the hormone, correct and if so, then what is the cause of my osteoporosis? By the way, I am a 66 year old, white female, overweight, but not obese and I am prescribed Fosamax 70 mg once a week

I will be staring Tymlos soon, but I’m afraid, because I have taken Boniva for 4 years and stared with lots of pain on my bones and all of the sudden I was walking and my left femur broke in half and the put a rod and 2 days they did my other femur that it was going to brake soon, and I have a fracture on my back too, and other problems and they can’t do surgery because my bones are very bad, my back surgen say that is the worst bones he has seen, is there a place where I can see other people talking about their experience taking Tymlos?

Hi, last month I fail 8 steps stair that cause bone broken at left leg and right shouler. Doctor said even I am 43 but the bone is like 70 of age!!! It is terrible if I have to spend many months till my bone fully recover, so I asked doctor is there any drug or remedy to help shorten the time of bone healing, then doctor recommend using Forteo. He said there some research that not only for Osteoporosis, it can also help to heal for bone fracture. Have you ever heard about this?

Yes, we used forteo for difficult to heal fractures in my orthopedic practice. It was off label then but may have received an indication from the FDA in the interim.

Hey doctor, An endocrinologist in diabetes specialist gave my mother Forteo(Eli lilly) or Rockbon(abbott brand). We are thinking should she take this medication or wait for to see another bone specialist to be appointmented after 1 month. She has -4.7 t score and L1 fracture. She is also taking calcium and vitamin D medications. What can you recommend about staring these injection? Thank you

the heart palpitations can go on for 3 hours a day- and cause stress on the heart- my cardiologist thinks it's not a good drug for anyone with heart issues, but it does seem better than bisphosphonates that gave my mother A-fib and also fractures in her feet! p.s. the background music is distracting. Also, I've heard (and my endocrinologist said) you lose what you gained if you don't follow the Tymlos with a bisphosphonate- but you don't seem to agree with that?

What are your thoughts on taking Evista? Have you discussed this drug? I have osteoporosis, 52, and family history of breast cancer. I’m not considered candidate for HRT.

Hi there, I haven’t done a video on it but maybe I should. Short answer is it does improve fracture risk but is not designed for long term use. Still need an answers there. Also increases blood clot risk. HRT and family history is a tough topic. I have a video coming out tomorrow on estrogen. I think you’ll find it compelling. Most notably treating the issue you know you have rather than treating the one you are afraid of. A long conversation for sure but make sure that decision is made between you and someone that knows the literature really well.

What about stopping Tymlos after 2 years? I am told that I will have to go to Prolia and/or Fosomax type for the rest of my life. I have osteoporosis with severe risk of fracture I am told, 68yo, began supplements in Jan, no fractures yet, I move alot but no regular exercise program. Have tried PT, etc over the years to help with periodic back pain. I would like to try to build bone without drugs but everyone has told me...Don't bend over...freaks me out. Also am in the need of a tooth extraction due to resorption of the tooth (could have been caused by too quickly moving my teeth with invisilign braces). Oral surgeons and dentists are scared of me being on these drugs and doing treatments now and in the future. So I am stuck not doing anything. And not sure if I can really improve my bones without these scary drugs. Any thoughts and help are welcome.

It depends on the unique case. I’m not a fan of Prolia in general but if bone loss needs to be slowed there may be a place for that. Forteo on the other hand is better for bone building.

So, if I'm understanding you correctly, though I'm sure you can't say specifically until you know why someone is losing bone, it sounds like you're saying anyone's bones can be improved without pharmaceuticals if they have the time (and comply with prescribed protocols, of course?) That's the thing...we're not particularly compliant, or we want to be but life gets in the way, or we don't see progress so we wonder why we're bothering. Etc. But, that's true of drug regimens too. There's a reason reclast is just once a year, right? I'm trying to envision giving myself a shot every day. I could do it but as the number of things I have to remember to do at what time goes up, I can see where something else could fall off, next thing you know, all you're doing for your bones is that one shot per day. Which is the other problem, at least, for mw, with pharmaceuticals? I'm just old enough to never have not had antibiotics and vaccines and you go to the doctor to GET a pill, you were lucky to have easy access to medical care/pills...so even though I also was taught to try to stay healthy, it was always like, "But there's a pill/shot for that, so don't sweat it " My mom took fosamax and now Prolia without a second thought (which is kind of weird because she's kind of anti-vax, even anti-supplement ) My dad is personally helping keep the pharmaceutical companies (and doctors) afloat. All to say, it almost feels crazy to not just go with the flow, but...recent times HAVE made me think about things I've just accepted as true or inevitable. My insurance company denied my doctor's request for Evenity, they suggest they might approve it if Prolia, Forteo or Tymlos prove to be contraindicated (how, by trying them?) They don't consider Evenity "medically necessary." Hence thus video. I feel better about forteo or tymlos now, (doctor didn't suggest them, not sure why,) but my husband, surprisingly, suggested I keep trying the natural route--at least until I get my next DEXA, due in January. (Heck, I'll be there before a drug is in hand, right?) So that's what I'll do. And for those wondering why I share so much info online, it's because maybe someone else is asking the same questions, having the same thoughts, and just needs to know they're not alone. If that makes me crazy or stupid, meh...been living a long time, probably I'm not going to change now.

Hi Cindy, Thanks for sharing. Short answer is yes, bone can be improved naturally but that has to come with some kind of action. lifestyle, nutrition, sleep, supplement and more. We design most programs without injections so that's not an issue.

Hi Cindy, thank you for sharing your thoughts on this. I share your feelings on these drugs. I 74 with osteoporosis dexa is - 4 was on prolia for 2 years after my diagnosis on 2019 after a fall with a T12 compression fracture. Now my endocrinologist wanted me on Forteo which I believe was not approved by my insurance. Maybe it's providential. Anyway I have following Dr. LUCAS, bought his book, teaked my diet, add more strength training on my exercise routine, change my calcium supplement to Bone Up, stopped my omneprazole..now I am shopping for Liv plate.. All these I am in gratitude to Dr. Lucas. I haven't had the opportunity to be in his master class. Maybe I will soon.

Im using forteo for a bit over 1 year now. Dexa at 1 year L- spine improved 4.9% (3.4 to 3.1), but both hips got worse -7.6% ( from1.7/1.8 to 2.1/2.2). Other test results were insignificant. What could cause this disparity b/t spine and hip? I was focusing on weight/resistence training but not so much cardio. Lately ive added walking and ecliptical 30-45 min daily. Love to hear ur comments! TY!

@belly4608 we see this discrepancy sometimes. We see it in both directions too. It sounds like the metabolic benefits of Forteo are providing the right impulse but the questions I’d want to answer are around other causes of bone loss, hormone, gut health, nutrition, nutrient depletion, inflammation etc. When we see a discrepancy often it could be to the stimulus for bone growth too. Is the training side loading the hips and spine adequately?

Read the prescribing information before you accept a prescription. Femoral neck increase at the end of the two year maximum duration is 1.5% and then goes away. Will this reduce your lifetime risk of breaking a hip? Not for me.

Correct! However, we have to recognize that the studies ended with no good next step. This is the issue with short term drug therapy with no comprehensive plan in place. When we use these drugs, which is rare, we are making sure to create a comprehensive plan, that will support bone improvement overtime. I look at these drugs as an opportunity to push the lever on bone metabolism for a short period of time to get somebody out of a dangerous situation. If that is not paired with, a comprehensive plan to continue to improve on health then we are likely to see the loss of those benefits and potentially an increase in fracture risk.
