What does 4 by 4 5 min mean?

It means that he is doing a total of 16 five minute VO2 efforts, in four blocks of four. Without knowing his exact timings, it would be something like a block of four 5 minute efforts, probably at 120-130% of FTP, with 2 or 3 minute rest between each. Then a longer rest of maybe 10 minutes before the next block of 4 efforts. Then the same again for the 3rd and 4th blocks of 4 efforts.
