Dr James Gill joins the team at Dr Finlay's Private Practice
Whilst I'm still a "True Blue" NHS GP, there are some things which the NHS doesn't cater easily towards - for example helping someone to look at all aspects of their life in order to be able to improve their 10km run times, or perhaps get prepared for climbing Kilimanjaro and sorting out high altitude medications like Diamox. Being able to take the time to view the whole patient and assist them in achieving their goals is one of the reasons I have joined the team at Dr Finlay's Private Practice
There are a surprisingly large number of overlaps between helping people perform in the exercise arena and helping people to recover from the fatigue and general damage that occurs from Covid, which is one of the reasons I'm also passionate about helping people with long Covid maximise their fitness
Video consultations are national, and face-to-face consultations are from Birmingham.

It may not be as exciting to hear from others as it is for me, but I remember when I first started practicing law that I was satisfied with providing a good service despite not being able to do 100% of what I envisioned myself doing or being blocked for "reasons". This satisfaction fades quickly and it eats at you. Hearing that Dr. Gill is able to keep his livelihood while also helping to soothe the "moral injury" is a key step in reaching the maximum capability and fulfillment of any profession. I know intimately how positive of a change this is/will be for Dr. Gill and I look forward to seeing the effects of it! I know for me I don't mind the long hours or increased responsibility when I can feel my soul becoming enriched from fulfillment. Best wishes, Dr. Gill!

I appreciate that it covers long Covid. I’m not in the UK but I’m lucky enough to have a long Covid clinic doctor locally and helps with a lot of symptoms. Even though my physical activity is nowhere near my pre-Covid capacity, I have been able to live my life mostly again and handle traveling.

Thanks. The service let’s me give patients the TIME to really dig into problems for things people wouldn’t “trouble the doctor about” but actually can still be improved

Would you advise people to still apply to work in the NHS Dr Gill that's still my goal only in the Medical Administration side of NHS

Good luck with your new job. Are you involved still with the NHS? My mum who worked in a hospital in New Zealand for a while went private full time because similar to the UK, our Healthcare system isn't doing the best with nurses going private or going to Australia.

As someone with an active lifestyle I absolutely love this and have actually been wanting to find a GP (PCP here in the states) who subscribes to that model to help me as I am nearing my 40’s so it’s nice to see practitioners pursuing it! I’m lucky in that my job actually offers me services to help with that/help find what I need which is crucial to working in the retail industry and helping prevent/reduce/delay injuries/pain/joint problems which are unfortunately relatively common with the type of activities my body is put through on a daily basis. Good for you and enjoy!

So only people who are well of enough financially can get the kind of healthcare you're talking about at the private practices. The NHS needs to be invested in so that it can provide as much preventative healthcare as possible too.

The NHS certainly does need investment But unfortunately the government refuses to on a way that helps patients and the service, rather than making their friends money whilst damaging the service I only have one “me” and the NHS is very much a service. I love it, but it’s really taxed beyond it’s limit, so the focus is on “pulling people out of the river” There isn’t FUNDING to set up the service I want in the NHS… because “improving good health” isn’t viewed as important as “treating poor health” I’m doing this in my own time. Not changing my NHS job, and in a selfish way - it’s what I WANT to do. I want to help someone succeed in a 10km race, or get into running, or sleep better. The things that I can’t do on the NHS, because from a funding perspective “it’s not a priority” I have to play by the rules. So I’ve decided to ALSO set up my own game. I think that’s fair 😊

While I empathise with the sentiment of wanting to work in areas you’re passionate about, there is only a very small percentage of people who can afford that. Those who can’t, do not have the luxury of choosing an exceptionally well qualified and experienced doctor. Makes me a little sad, and seems to follow the pattern of the NHS destroying itself.

Unfortunately it’s no no win situation. A lot of medics are leaving healthcare generally due to moral injury, not being able to treat patients adequately. It is entirely a government issue. They want to break the NHS. I’m not leaving medicine, nor the NHS, and I have not reduced my NHS hours. I’ve merely tried to minimise my own moral injury

I never consider it good news when someone exits the public health service for the private sector. In Canada it's a real struggle to find a GP, and I haven't had one for years. Walk-in clinics are okay, but you never receive the same level of care as that from someone who you've developed a relationship with. Nonetheless, I hope you do well in your new position.

He says in the video that he still works for NHS. He's not exited public health, just put a foot in private as well to expand the work he does.

@@abbybailey7402 Every hour he spends in the private sector is an hour he doesn't spend in the NHS. That provides less service to those who can only access a doctor through the NHS.

Unless I’m doing private work on what was previously a day I wasn’t employed in the NHS… Not that I have to justify this, but I feel I want to respond to the idea somehow I BELONG to the public service. My Thursdays have always been my “sport day” and I don’t work then. I can do a long cycle 90 mins to the private clinic, see the patients, improve my own mental health helping people without the NHS time constrain stress, then cycle home. So I’m still getting my training in, and actually try to reduce the mental harm I’m feeling by being under the pressure of 10min consultations.

It’s motivated by my own health. But even if not, there isn’t anything wrong with earning money to pay bills 🤷

@@DrJamesGill agreed. You always ask how how is the patient feeling but nobody asks how you are felling. Stress is a massive issue that can prevent people from working

If I was trying to make money, I can assure you, this wouldn’t be it I genuinely love my job, HOWEVER there is something called a “moral injury” which occurs when we know that we could be doing more, but can’t I’m trying to help myself by making a job change that means I can do everything I can for the patients I’m currently not able to help

God forbid doctors wanting to make money just like every single other person in society. Do you think that we are somehow immune to expenditures? As if the mortgage lenders will say ah well you can have this months mortgage on us mate since you’re a doctor
