Hi Nick, I'm my mom's caregiver and it's our first time with CPAP therapy. She's got the resvent iBreeze. Would you mind please telling me how to access the clinical menu to adjust her pressure? Hers starts at 3 and goes to 20, and she feels like she's asphyxiating when it's both below 4.5 and when it's above 9.5. Her tummy keeps filling with air, her ears feel like they're about to pop, and the high pressure is just terribly uncomfortable. Her AHI is currently 21.8. I really would love to help her. Can you please help me help her? Btw, I absolutely love your videos!

62 years old, had untreated sleep apnea for 15+ years and in complete denial. My advice to anyone, if you are told you have it, do something about it, or like me you may end up with Diabetes, a stiffening of the right side of the heart and moisture build up in the left lung due to the heart working harder to force oxygen in and CO2 out...Only been on CPAP 6 weeks and truthfully I've been struggling with it... Masks, pressure, leakage and dry mouth everything you've mentioned.. found all your videos very informative and very helpful. Being in the 🇬🇧 and under the NHS we don't have the access to the data and settings that you have.. It takes sooooooo long to speak to a clinician to ask about fine tuning and often you've got what you've got and get on with it..

4 days in to Cpap therapy. My doc had me at 16 to start. It’s been really hard to adjust. My mask is good, and I’m wearing a brace to stop my chin tucking in. I’m so tired, and I can’t face another night being woken up by the difficulty exhaling. My stomach is bloated when I wake up and it’s so uncomfortable. I just changed the settings to 10. Thanks for this video. I’m taking this into my own hands and just hearing you say to experiment is all I needed. Thanks.

I had an almost identical experience! My max pressure was at 14 from the doctor. As a result I barely used it for years. I didnt realize that I could change it and greatly improve the effectiveness. My AHI averaged at 9.5. I set my max pressure to 10 last week and now wear it most of the evening. Finally feeling rested in Nebraska!

All of you need to stop your bullshit. Stop being babies and actually try to acclimate to your situation. I have been a dme tech for over 11 years and can tell you with the utmost certainty that you aren't doing anything good for yourself. If you understand the word therapy you should also be able to understand that your titration was set at that level because that is what was ACTUALLY WORKING OVER A CERTAIN PERIOD OF TIME. You can of course go ahead and ignore my warning and do what you want but you'll suffer the inevitable consequences, not me. Switch your settings back to what was prescribed and stop thinking that therapy is something that provides effective effective results after only a few days, or be a wannabe doctor who is also a nuclear physicist and whatever you think you are. But seriously ffs stop thinking you know better than a doctor.

Okay I know it’s only been one night but what a difference!!!! I’m so happy that I came across this video. I reset my numbers last night and got 0 leaks. I slept great and my nose is not dry. I couldn’t figure out what was causing the excessive dryness but after watching this I believe it was the high pressure and the leakage it caused. Thank you so much this video has been life changing. 9.5 holy ####. That is wild. The problem first time when you get the machine " set up " by a person. First you are not used to pressure like that. I remember when me and a person set it up. Felt like i could not catch my breath. I am very trained, powerlifter. But even for me it was very strange feeling. On my 4th year now and now i begin to mix and trix with the machine. Wish and new stuff earlier.

Ok, I found your tutorial on the AirSense 10 part 3 of 3 which showed how to get access to the clinical menu. Now I'm good to go. Appreciate you man! You are the best!!! I haven't used my machine for many months because I couldn't get through to the doc that the pressure he had it set for made it blow out my mouth. He even wanted it set higher so I put it in airplane mode so they couldn't turn it up more. Of course that didn't help because it was already to high. I finally just gave up because I wasn't aware I could set it myself. I found that I was getting better sleep without using it. My wife decided today to do a search and found one of your videos. Now I can start using it again. I'd like to find the video on how to remove the modem. I don't want to risk airplane mode accidently getting turned off. I subscribed and now am going to join. I don't know how to put into words how much I appreciate what you have done for those of us who desperately need this info. Thanks again from the bottom of my heart! YOU ARE A REAL HERO!!!

I was started at 8cm, but despite a 0.1AHI, I was having poor sleep quality as well as a lot of abdominal bloating in the morning. After a month of not being able to reach my sleep specialist, I took matters into my own hands and lowered it by 1cm after reading up on my side effects. Best decision I ever made.

@@thankyou62 hope you figured this out if not you need to look up the provider menu. Getting to that menu is different for each machine. This channel usually shows for the most common machines how to do it and what each setting does.

@@CPAPReviews I tried ResMed auto with humidifier a couple years ago and felt like I was on a ventilator. UN-natural breathing even getting to sleep was almost impossible and if I did go to sleep I never made it over 2-3 hours. I think mine was set 5-15? If that was a 5 going to sleep it was a lot of pressure that was far from natural. Caused a lot of anxiety. Three different mask. In a month I returned it and tried to get some sleep. Any suggestions if I try again? This was at the beginning of Covid and had very little close contact help.

Excellent video!!!! I have been waking up with dry eyes and feeling like I had been in a fist fight! My neck and chest muscles were so sore!! I have lowered my pressure and kept my AHI under 5. My doctor said I shouldn't adjust my own pressure since here in the U.S it is a prescription. I told him too bad. He can kiss 💋 my a**. Feeling better already. 😁😁😁😉

I’m so glad to have found this video! This is my machine and I feel my pressure was too high. I had dry eyes, mouth, throat… I woke up feeling like my torso had been beaten and had excess air/gas in my stomach. I have begun the process of adjusting down and will try it tonight. Fingers crossed!

I’m watching this video again, taking notes this time! I know they say your body needs to ‘adjust’ to it but why do I feel way way waaaaay worse. Instead of feeling fresh as daisy like I expected, instead I get headaches that lasts all day long, light headedness, nausea, painful boating and super dry coughing fits. I DRED going to sleep. I actually woke up during one of of those super high pressure times and it felt like my head was being squeezed like it was going to pop off. This is the most informative video I’ve run across. Nick, thanks for the information you present in this channel. I ran across your channel when researching the loss of connectivity of my many years old AirSense 10. Turns out it was a 3G modem and they recently just turned off 3G in my portion of the US. However, the info that you provide is second to none, I've been on CPAP/APAP since Feb of 96 (yep that's 26 years). I've learned more from your video's in an hour than all that I've been taught over the years from the medical and or DME suppliers. Keep up the good work. Once again Thank you for the explanations.

Amazing! My machine was set on min=4, max=20 from the beginning. My supplier insisted that it had been adjusted specially for me. I struggled with mask leaks and noise. When I found your channel I opened the therapy menu and discovered that ALL of the adjustments were set on the default values. The supplier had never set anything. Reduced the Max from 20 to 13 and the problem is solved!!!! AHI is now .7. Yes, seven tenths! Thank you so much.

@@Michael-vh1wc I experimented with changing the minimum, but it doesn't seem to make much difference. It's on 8 or 9 now. I like to start with a pressure that I can actually feel..... On my Resmed 11 I can barely tell if the machine is running at a min of 4. When I started with CPAP 10 years ago, I used a gradual ramp but I don't find I need ramp at all now.

I have just concluded my installation on a 5 ton Trane. After looking at several videos I focused on yours Jim as your recap of wiring layout was very clear. Also, your control box layout looked just like mine. I have done 2 starts so far and went from 75 or so amps without MicroAir down to 44 on the first start then to 33 on the second start. So far so good. I will update after 5 starts.

Am systematically adjusting the max now and using the data on the resmed app that keeps track of everything the machine is doing to monitor the AHIs. I have used a CPAP machine for 15 years. Have become very dependent on it. Since 2021 I have used a ResMed 10 and have loved it. However 2 weeks ago I contracted COVID 19. It was terrible. Felt like I was going to die. Had to go to the emergency room. Could not get sustained oxygen levels lying down or catch my breath. Using my CPAP machine I felt like I was trying to breath through a straw. Found and used several of your videos to learn how to enter the setup menu and was able to experiment with pressure settings. Saved my life! I don’t know if you can do this but can you do a video on how to change pressure settings to comfortable levels for people dealing with COVID or severe upper respiratory infections, extreme congestion and or nasal congestion. Love your videos.

I'm new to the CPAP world and I have to say it is not as bad as I had been led to believe. One simple trick which has worked for me is to ensure my top lip is clean shaven before I go to bed and to put a small amount of moisturiser on, as I use a nose pillow. The result is a relatively leak free nights sleep. Simple but effective.

I switched everything to manual, turned the heat to 86, humidity to 6, lowered pressure from 10 to 7, disabled ramp, and disabled EPR. Now I have no bloating, no dry mouth, and my AHI is 0.5

Helpful. One of the difficulties of CPAP is the exhale. Once i discovered EPR, set it to 3, I could exhale properly, adjusting the positive inhale pressure becomes easier. I created my own hypoxia by having pressure too low, which created 'events', which drove the AHI number up. Once I bumped the number up to closer to 10, things started to improve. Ill just say this-no doubt you can pay $1000+ to have a titration study to determine the correct pressure, humidity, temperature, etc. Do it if you can afford it. Probably the best solution, so im told. If not, this is a helpful guide to get setup. My opinion is getting control of the exhale is vital because without that, the mask will come off quick.

Resmed 11 here. I've turned just about everything "auto" off and turned it to cpap after trial and error. I custom adjust as needed. I have the machine for neurological hypoventilation so I can go low but if too low I feel like I'm suffocating. Found 8 is a sweet spot. Sometimes I go a little higher. I have nasal n30i and find if I wait to place the nose piece in my nose until the machine is ramping up and suck in as I place it, it automatically goes to the correct spot to seal so it doesn't leak. Sometimes a little tweaking is necessary. Also, tightening the headstraps does not help leaks. Actually, having the headstraps a little bit loose is better and seems to allow the actual seal to do the work of stopping leaks. I'm a side/stomach sleeper and am still searching for the "perfect" pillow. But I've found a flatter pillow where I can keep my nose at the edge working best, especially wanting to keep the exhalation vent uncovered. Also, the back of my head is a bit flat...yep...so I put a hairclip on the lower back of my head and that keeps that back mask strap from moving. No more sexy for me...oh and I tape my mouth. But I feel better. Hope this helps someone.

Thank you very much for your videos. You have helped me immensely. I have an airsense 11. I just started doing cpap sleep therapy a week ago. It's been a very strange start for me because my sleep therapist tested me for sleep apnea, then after I tested positive he gave me the machine and configured it. The first night I was doing great. After we spoke on the phone he told me that based on my results he would adjust the settings to try and optimize them for me. The next night I woke up in the middle of the night with really bad aerophagia. I called my provider but my sleep technologist had promptly left the company. I decided to do some research and found your videos and many others. I went for it and started making changes myself and things improved immensely. Meanwhile my provider passed me on to another technician but by the time she called me I had already started changing all the settings myself as per your videos and things have gotten a lot better since that crazy second night. I believe he lowered my pressures a little too much so for myself when the maximum pressure is too low I get bad aerophagia.

Thank you this helped a lot. One problem I have is when the cpap starts every night the pressure is strong despite being at the lowest level (4). Also I find myself waking up at night having to turn the machine off and on again because the pressures too strong. I’ve followed your tips for the working out the maximum pressure.

Thanks Nick! Just bumped up my power level. What scares me is I started on power 7 and still had a bunch of events all the way to power 13. I know 20 is the end. Even when I have .4 events per hour I can feel it in my jaw and wrecked. Sleep Study is huge in America, it's big business here. Lifetime patients. My ahi is 1.2. Just checked it. My sleep doctor is impossible to contact. Thanks!

@@philiphungate3121 Hey brother Phillip. My dad is a vet and gets his cpap stuff from them. He will have up to 38 events per hour while using cpap and on auto setting. I think cpap just measures your events and doesn't prevent them. I'll be under .5 events per hour for a week then BAM at 17 events an hour. It's the same deal with my doc. It's become big business. I've lost about 60 pounds and I'm hoping to get off of cpap. I learned most things from online because doctors are no help.

@@philiphungate3121 One of my doctors at a state hospital told me they cant help me and to talk to Dr. Google. I've had 5 surgeries on my head, have two holes drilled in my skull and a new ear drum. I went to visit someone and her husband was a surgeon. Stayed two weeks. Greg and I got along great. He would go slice up people then he would get home and we would have a cigar, whiskey and sit in back by the pool and listen to classical music. Old Navy man. We had many laughs. He said "Jeremiah, I have a hard time making friends". I told him "I have a hard time trusting doctors but I trust you". Greg's wife says he is one of the last honest surgeons. Still out there but rare.

Thanks for what you do and very helpful information. Been struggling with Resmed 10 CPAP for a year and half with these various symptoms. No doctor has provided such thorough instructions such as you. I dialed down my max pressure last night to 6. Prefer using nasal pillows because I can't tolerate anything on my face. Even so, they're still irritating despite my current one month usage AHI being 1.4, total AI 1.0. I have leak 29L/min without chin strap since removing chinstrap and trying to train myself to keep mouth closed when sleeping because I notice hair loss which I read is another effect of these straps. You mentioned lowering air pressure which can minimize leaks so will try dial it down to 5 next. Tried using a CPAP cap with chin strap which thins hair just as bad. Going to try the Resmed Bella nasal pillows which can be used either as a ear loops or head strap. Hopefully it would solve the hair loss issue. Any suggestions?

Hey, have you ever tried mouth strips instead of the chin straps? - kzbin.info/www/bejne/fYK0pHmrhb6ebLs. They're getting pretty popular now and a lot of people prefer them over the chin straps. Also regarding the hair loss, you might look at getting some thick mask headgear covers. You can find them on amazon etc. Or you could also perhaps look at wearing some sort of hair net / cover before putting your mask over the top. I'll have to do a bit more investigating. What model mask do you currently use? IE ResMed P10? I know some people use this mask that has no headgear called the Bleep DreamPort Sleep Solution. Maybe check that out. Cheers

I lost 150lbs. I lowered it down from 12 to 10, but last night I noticed that it still felt too high, and I’ve experienced a lot of gas lately. I put it down to 9.5 last night, and Boom 💥 no more gas. No more aerophagia.

Hey Nick! Great video! I have finally bought my Resmed Airsense 10 Autoset. It's a beautiful machine. Very clever and it's silent. I was on fixed pressure of 9 with a Ramp set to 5. My apneas went down from around 11.7/15 per hour to between 2/5.5. When the sleep therapist set the machine to AUTO and has it set to Auto Ramp (it shows up 4) and the auto settings are at between 4 -12 I got the following readings: (Events per hour) 27/1 - 5.1 Ev, 28/1 -7.0 Ev, 29/1 - 8.2 Ev, 30/1 - 3.4 Ev. I woke up during the night/morning of the 29th and saw the machine was at 12 psi. This was after around 4/5 hours of sleep (I sleep late...) I feel it's good that the Auto setting can come on and take over when I need it. The sleep therapist told me to wait 2 weeks. What's the best way to get the Apnea events as low as possible? To increase the lower level from 4 to around 8.5? Keep the ramp at 4? or 5? I know how to get into the clinical menu but I feel the therapist will have a fit if I go there! I am still feeling moody and lethargic and sleepy with no get-up and go in the mornings.... like I am coming out of anesthesia! I appreciate that you are a busy guy, however, any hints and tips that you may share with me would be great! Thank you, Nick.

Hey Michael, thanks for your comment. Normally you do get better AHI results with a fixed pressure machine however your AHI levels seem pretty good on Auto also but if you wanted to improve them even more then yes i would try experimenting with your lower level and bring it up a bit from 4. Probably try 7 to start and then you can increase a bit more if required but just see what feels comfortable. Regarding how you feel when you first wake up in the morning. This actually has nothing to do with how good a sleep you had but more so what STAGE OF SLEEP you were in when you first wake up in the morning. It's like the lottery haha. So if you wake up just after slow wave sleep or RemSleep then you feel like your hungover and it takes like a good 30 min to feel alert but if you wake up just after stage 1 or 2 sleep then you will feel like you can just jump out of bed. I recommend you check out this cool app apps.apple.com/au/app/sleep-cycle-sleep-tracker/id320606217 which helps to solve this issue and get's pretty good reviews. I love it. It can also take a few months of therapy before people really start to notice improvements in how they feel during the day but judge it in the afternoon not when you first wake. That being said, even if you don't feel a lot different, the proof is in the numbers which are showing a big improvement in the quality of your sleep breathing. Have a good one mate. Nick

The ONLY way my pressure can be adjusted on my AirSense 10 Auto is by my pulmonologist contacting the DME provider and THEY adjust the pressure. I CANNOT adjust the pressure myself. BUT, I've been doing CPAP FAITHFULLY for just over two years, and I'm STILL tired and sleepy during the day! I CANNOT WAKE UP!!

Your friend that does a 3 part series recommended you. Great information. You guys are very good at give us this information. Any suggestions on a flex vs c flex? My therapist way back in 2005 recommended c flex but this was back when I had a CPAP instead of APAP.

Awesome! Thank you. A-Flex and C-Flex are Philips pressure relief settings. They are very similar but A-Flex reduces the pressure at the end of inhalation and C-Flex reduces the pressure at the start of exhalation. Just try them both out to see which one you like the most but they're very similar. Cheers

@@CPAPReviews ended up at 8.6 cm and slept great, think I am going to try 7.6 cm tonight, I never questioned the place back in Ontario where I purchased it in 2019, and I stopped using it early last year as I had lost weight and thought I was sleeping better. Started using the Airsence 10 again early this year (2021) and I had to have the mask headgear really tight to stop the mask from leaking and here it was at 20 cm, duh, but thanks to you I have it on track now. One more thing Nick, how do I get the report that you got on the machine you have there? I can get to something called my report I think it is, but none of the stuff you show is there. Hi Nick, I was tested by a sleep clinic and my AHI was measured at 67 /h AHI. I am using the ResMed AirSense 10 AutoSet 5 - 15 The first couple of nights the AHI was down to 3.8 / 2.6 but started creeping upwards. Lately, I wake up tired as can be. I am tempted to set the max a bit lower OR start with a fixed pressure test to find the optimal setting. Could you please send me the instructions to get into clinical settings as well as your suggestions

Thank you for this video. One problem I have been having, for about the last 6 months, on occasion, is a "totally" dry mouth. It's awful! I'm not sure, but I think it may be my sleep position changing a lot at night. I am a very restless sleeper until I get into a deep sleep, which I think is not too often. I'm a big guy, at 6" tall, and 300 pounds (which I'm working on getting down to a proper weight). I have an AirSense 10 CPAP and a full face mask. Been on CPAP since November 2010.

Nick, Thanks for doing these fantastic videos. I haven't found anyone near as f=good as you. I was just switched from CPCP to BIPAB, which eliminated my need for supplemental oxygen. I am struggling with mask leakage and your videos have been a great help. The video on the Quattro was particularly helpful and I switched from my F20 back to an older Quattro with great improvements in leakage. Congratulations on your latest family addition. I have four children and they are all wonderful

The last seven days, my average is 8:47 sleep with average AHI &Ai of 2.8& 1.5. My medical supplier called to see how I was doing and I told her, thanks to you, I had everything figured. Great channel! Great content! I just got my machine 2 days ago. The first night I slept fairly well and woke up with more than average energy. But after the second night, I have a morning headache and feel groggy. My nose and eyes are dry. The tech said the settings are automatic and do not need to be touched. Thank you for this. I need to analyze my data and adjust accordingly.

I have AHI that varies from as 0.3 - 0.8 with my pressure settings to 4.0 - 6.2 do you think I should reduce the max pressure more or is this perfect ?

He does mention in the video that as long as you are under 5 AHI, you should be good. Your average AHI is very low, so in the video he also says you can keep lowering your pressure as long as you keep under 5 AHI. Are you sleeping well or do the pressure changes wake you up?

Some people will find it difficult to get under this number so when it comes to leak rates, if they're not bothering you too much and waking you up and your AHI number is still low then you don't really need to worry about them too much. Hi Pete, great question mate. So it can vary for a number of reasons. 1. The position you sleep in and how much time you spend in supine (on your back) sleep. Most people have a lot more sleep apnea when they're on their back compared to on their side etc so the more time you spend on your back the more likely your AHI will increase. 2. Your diet (especially alcohol consumption). 3. Allergies / congestion / colds etc will have an impact. 4. Weather, how hot or cold it is, how much ambient humidity is in the air. And there's also many more factors that can influence the AHI. My advice to everyone is not really pay to much attention on a night to night basis. You really just want to be looking at your weekly or monthly AHI average. This will give you a good ideas as to the overall control of your sleep apnea. An AHI of 11 is still very low. It's important to be your own advocate too. I had doctors tell me I couldn't have apnea, despite over twenty years of symptoms that ALL could be explained by apnea starting when I was around 12. My job changed and the new insurance allows going to a specialist without a referral so I got an appointment on my own. Lo and behold apnea was confirmed! I'm still a bit tired during the day (there's medication I'm on that can cause that), but the splitting headaches and desperate need to sleep during the day are almost entirely gone, I don't wake up in cold sweats desperately needing to pee, and sensitivity loss in my fingers and toes is much improved only a month in. Get checked!
