You could say he’s “actively” hitting the high vocal in that opening. The rest of the lower, gravelly distortion is happening at the same time as an overtone because of how he has his tongue and jaw positioned, creating a semi-occluded vocal tract, causing his air to kinda separate and bounce around in two separate “mouths” inside his own mouth before leaving it as a vocalized combination of two tones.
