As a DA. I'd prefer and recommend to take FMX in a sitting position. Also I'm confident to say my xray taking skills never disappointed any Dr I've worked with! With or without the rings. I am the final boss of radiographs!! Come at me Pam!!

That makes it more difficult. Somehow you must just be very gentle and place the holder on the lingual side of the tori. Or just take a panograph if you can't take pas and bitewings.

I have a question I can’t put things in my mouth without gagging terribly… I have a hard time flossing and brushing sometimes and tongue scraping is very hard to do. All because of my gag reflex… I dread going to get a crown replaced that I need .. is there anything I can do for an extremely sensitive gag reflex? I never used to be like this !

Once had a patient come to the clinic with severe gag reflex like you, she said that she uses ginger root before the appointment, forgot to ask how.... About brushing : don't wet your brush before using, just put a little amount of paste and dry brush, use extra small baby size brushes since the gag is mainly caused by the brush touching the tongue so a smaller one is better , and you can even brush without a toothpaste sometimes

Sedation is marvelous for gaggers. Often gagging is caused primarily from anxiety.

great videos as usual .. what are the dangers of x rays on doctors ? I usually hold the sensor in my clinic while taking the x ray and get exposed , is that dangerous on the long term ? would love to know your opinion . thanks

Minimal because the radiation is so low. Assistants are not even required to wear radiation monitoring badges anymore. No, the current studies state the radiation from modern dental radiographs is so minimal that lead aprons for staff are not needed. Also, if you read the studies, they state "given the low doses received by staff involved in dental radiography, routine personal monitoring is generally considered to be unnecessary." Agreed. Sensor should be facing toward the tooth being radiographed AND the wire should be coming out of the PT's mouth, not going in... it could distort the radiograph AND damage the sensor wire. We were taught that by sensor companies at lunch and learns!

Yes, very safe, according to the data I have. The assistants wear radiation monitoring badges and they never register unsafe radiation levels. Yes, you're correct. Even with Nomads, DA's should be wearing Dosimeters. Unfortunately, not may dentists provide these any more. She is not wearing one, or it would be on the outside of her lab coat.

Digital imaging in the Dental setting is about 1/3 the exposure of film based traditional exposure. Some states do not allow handheld X-ray devices like this. Yes, they are safe if used properly. Several shots taken in this video were definitively not done properly as far as Clinician position as it pertains to how the device was being held and aimed. If the State Radiation Agency happened to be on site observing there would be either a warning or a violation issued. Just because the images are coming out good doesn’t mean the decision was proper.

Thanks for your comment. We will look into it. We felt like we were following the guidelines. Most dental assistants coming out of dental assisting school that have come into my office do not know how to take radiographs. We are just trying to teach them methods of taking radiographs that work well.

Pam would be very valuable asset to most dental offices. Unfortunately most dental assistants are incapable of making quality radiographic images. My observation is that very few know the difference between a quality image and one that is not useful for the doctor to help make a diagnosis. That being said I often choose to make single periapical images myself. When a new patient requests to have their most recent radiographs sent to my office I am 90 plus % astounded how useless the images are..

I just hired a new dental assistant to replace Pam. After 40+ years in the dental field, Pam has gone to 1/2 days. Last spring I asked Pam to apprentice the new dental assistant full time for 6 months. After 1 year of employment, the new assistant can now take excellent, diagnostic radiographs, pour models accurately and do many other things very well. There is no substitute for experience, but my experience has been that dental assisting schools are doing the best they can, but assistants fresh out of school are not able to move into a mature dental practice and really contribute until they have been trained in our practice and been on the job for at least 1 year. We have implemented the dental assisting section of DentistryMasterClasses.com to help with the training of dental assistants. I can only imagine how difficult it must be to set up a dental assisting school and deliver a useful assistant upon graduation. If you think about dentists fresh out of dental school. They know how to do a few basic things, but real training in more complex things comes after dental school and are not mastered until the dentist has been practicing and continuing their training after dental school. Again, I feel dental schools are doing the best they can, but in all professions experience and top tier post graduation training are necessary, in my opinion, to become really proficient at nearly anything.

They should be using a thyroid collar. Always! Even with NOMAD's, it still emits radiation. Also, she has the wire going IN the patients mouth, it should be coming out of the mouth, the opposite way, other wise it can distort the image and the PT is biting on the wire, which is NO-NO to most dentists who've purchased the sensor, as they are a pretty penny. Also, the RIN's should not be placed on the counter directly NOR on the lead apron like that, that's unnecessary cross contamination. There should always be a barrier between a surface and or saliva. And that lead apron looks like one that you can't wipe....She should be putting it on a tray or paper towel. These are things I learned after dental assisting for over 10+ years and now teaching dental assisting at a college. That's per OSHA AND RHS.
