It’s so easy to forget that things like Botox and fillers are actually medical procedures with real life complications. Nowadays, they are so common that they’re almost viewed in the same light as going to get your nails or hair done. I had a complication with filler, which was luckily able to be resolved by dissolving. Luckily, it shook some sense into me to realise that these things are to be taken seriously and why it’s so so important to go to a qualified medical professional with lots of experience.

I'm glad you mentioned guiding patients to not overdo procedure. I get the impressison that lip fillers can be easily overdone. I'm often disappointed by the lip fillers I see on influencers.

I tried those procedures and they were okay but didn’t last very long and they were very painful. I would not recommend them.

How do we know what provider to look for if we don't know what kind of treatments might possibly work? Can you please do a series on what kind of non- surgical procedures are available? What you do and dont expect from them, if you do a few id them when first being treated, how often you do them initially, and in the future, who is a good candidate, what to expect? Or can you do a few videos on specific categories of concern? If its difficult to answer these questions without seeing the person and knowing their concerns, could we submit photos and answer a questionnaire, not for medical advice- but as examples you can use as educational tools for you to review via video? So we know what is out there for what kind of concerns & in what kind of state. Would be extremely helpful. As an example, I have a sunken under eye, and am *very* mildly jowely on the left side of my face only, and want to promote collagen production while I still can. I know very little about this option, but I'm thinking about microcoring and wondering if its a good choice, and is there another option, or a group of options, that can *effectively* keep up collagen production? Is it good timing, is it expensive, is it overkill and if so why, what other options do I have at my age, and at what price range, what kind of treatment and prevention should I get? As far as my eyes, I don't know if i would consider filler, but would something like PRP or Sculptra be a good option?

Hi! I think your videos are great! I’ve been going to the same dermatologist for about 4 years now and I have super sensitive skin that I’ve been trying to address for a long time and my appointments with my Derm tend to be a half hour long but I see my actual doctor for 2 minutes and she doesn’t really recommend a whole lot. I’m happy to get a laser treatment or micro needling to even out my skin tone but it’s never like yeah that would benefit you. It’s always just short and sweet. Can you give me any suggestions on certain peels or non-invasive procedures that would help make my skin look better. I’ll happily send you a no makeup face on IG if you’d like. Thank you!

I’ve had four ablative laser treatments, but had no visible improvement. My derm was so concerned that he performed the third procedure for half price, and the fourth one for free. Also, I’ve had four rounds of fillers for my lips and nasal-labial folds. Each time, i simply absorbed them and no improvement could be seen once the initial swelling disappeared. The derm who performed these procedures was a highly respected, well-established doctor. Could these failures be attributed to something wrong with me (as the derm asserted after giving up on me)? Or something the derm did wrong?

It’s incredibly hard to say without seeing and evaluating someone in person. It’s certainly never a patient’s fault for their response (or lack thereof) to a treatment. Sounds like it may be a good time for a second opinion!

I just returned from getting Botox around my eyes. I have the accordion lines on the bottoms of my cheeks when I smile. I was told that micro needling will help with those lines. Is this true or will only plastic surgery help? Thank you!

The first time I ever got botox I got masseter (for TMJ) and crows feet, and my smile was so wonky unfortunately. I couldn't fully bring up the corners of my mouth. Do you think this is likely from the masseter or crows feet botox, as I'd love to at least get masseter again to help with TMJ relief! Thank you!!

Impossible to say without seeing the muscles/movement in action. Both can be culprits but usually if someone has trouble pulling the corners of their mouth back into a full smile it’s usually a masseter botox complication in which the botox has drifted into another muscle called the risorius (the muscle responsible for pulling the corners of the mouth out to the sides).

I have a bad reaction to Botox which caused my eyes to droop severely, what can I do to reverse it? It has been over a month since I had the procedure. Thank you

I’m so sorry that happened to you. Unfortunately, very little can be done to make Botox wear off faster, but hopefully it gives you some peace of mind that no one has permanent drooping after Botox, and that the lids will return to their normal state once the Botox wears off.

I do not agree with the state licensing issue, becasue as you mentioned a RN can do that after a weekend course but so can a MD. I don't think it matters the license type it think the amount of training spend counts more than what license someone has. The only filler that completely ruined my lips for over a month, before having it dissolved, was in fact performed by a cardiovascular surgeon, someone that literally can change someone's life by operating on their heartm- he probab;y had over a decade of training on human body, but maybe a basic esthetician that did months of training on injectables is bette than someone with a medical degree.

Totally! It’s not just about the degree and fundamental knowledge, but also about the skill. I think the key is having both for optimal aesthetic and safety outcomes.

I will take an experienced injector any day. As someone who sat on medical review boards I can tell you there have been many problems with board certified doctors who just plain and simple didn’t do a good job. Thanks to your channel for helping to educate us all on what to look for. Often times I look at the person with whom I am doing a consult and if they look “over filled or over Botoxed”. I know the are not the person for me

Hi Dr. Sam, I have just turned 60 and having always looked after my skin with decades of dedication I am now seeing signs of aging that “annoy” me. A line between my eyebrows and either side of my mouth. These are my only concerns. Would you consider me too old to have any procedures? Ie. procedures that would have enough of an effect. Trish x

Say more about a long term plan. I am reluctant to go in if I don't have a particular procedure in mind and I don't think my dermatologist has never said come in to discuss a plan, though we have talked a little about what would be most effective for my goals. The economics of going in for only a chat put me off.

I know a lot of people are hesitant to pay for “just a consultation,” but in reality, you are asking your dermatologist to give you an evaluation and to recommend things based on their expertise. That has value in and of itself regardless of if you choose to invest in a procedure thereafter.
