Cost cutting. Pressure on all for on time performance. They just want to get the airplane in air. Sometimes windshield are also not cleaned properly because no ladder is present. I have myself cleaned the windshield because to get the tressle will make the flight delayed. Landing lights not working. Pathetic state of working and finally everything will be blamed on pilots.

Many navigation systems will control the autopilot and fly the plane down to the inner marker. You can also blame the investigators and or Smithsonian youtube channel for not knowing what they are putting out. But it protects any navigation equipment manufacturer.

The maps may need to be reviewed to be more user friendly but at the end of the day he is a pilot and he should take precautions and use more critical thinking when flying over known-to-be-dangerous mountainous terrain

I always love seeing this series recycling the actors. The blue-shirt investigator apparently had survived a lost 737 (DHI574), a 737 overshooting a runway (GIA200), a 737 landed in a river (GIA421) and after all of those circumstances, now a seasoned air crash investigator

People should remember this. PIA were once the shining example of how to operate a national carrier. They gave a lot of help to Emirates when the latter was gaining their feet in commerical aviation. Emirates today learned a lot of their operating style from PIA. Its shocking to see how much PIA have declined.

@@battlec43 No, the people who designed the chart assumed it was fail-proof, which it wasn't. The pilots were experienced. If the investigators found that the chart and cockpit was designed in such a way that it could lead to missing a detail easily, then you can't simply blame "pilot incompetence". Furthermore, it's easy for you to sit at home all relaxed and criticize others; I'd love to see you be in the hot seat. I have; during an IFR approach you're satires with workload (flying the plane, briefing the approach, performing pre-landing checks, communicating with ATC, etc). Therefore, if pilots are already saturated in that phase of flight, then the cockpit ergonomics and the IFR approach charts should be made as to minimize the possibility of misunderstanding (like what happened in this case). Aviation occurences don't happen merely because of one thing; it is a string of events that enable such an outcome to occur. Look up the Swiss Cheese model. If ONE factor happened differently or did not occur, it is very likely this crash would not have happened. So don't disrespect a deceased crew for an honest mistake when they did their duty to the best of their abilities, because NO ONE is infallible to mistakes, no matter how experienced they are.

Yeah, it heavily points at pilot error in the official findings, but it's more of an inadequate cockpit layout and confusing charts. The pilots probably did the best they could with the severe limitations.

2:33 has anyone paused this and looked at the chart? If you look at the line, 10,500 ft is at 13.0 miles, not 16.0. At 16.0 miles, it’s above 10,500 ft (his thumb is blocking the altitude).

I think this is a summary of episode Kathmandu Descent that is similar to but not the same as The Lost Plane (Thai Airways Flight 311). They both crash head-on into a mountain in Kathmandu killing all on board. I can't find the former, Kathmandu Decent, full episode that is a Pakistan Airline.

a system is strong as its weakest link..a thumb caused a plane crash..nearly as good as air france with tape over the speed sensor causing a wrong input in flight computer confusing the silly pilot, stalling the craft..tape is used when washing the plane...gaffer tape 4 in square ...total loss....
