Today’s guest is one of the world’s leading experts on the neuroscience of addiction and recovery. Anna Lembke MD is Chief of the Stanford Medicine Dual Diagnosis clinic and Professor and Medical Director of Addiction Medicine at Stanford University School of Medicine. Dr. Lembke has testified before Congress and consulted with governors and senators from Kentucky to Missouri to Nevada. You may also recognize her from her appearance on the hit Netflix movie “The Social Dilemma.” Aside from this, Anna has also written two books: “Drug Dealer MD” and her latest book, The New York Times bestseller “Dopamine Nation.”
Our conversation covers an array of important topics to help highlight the REAL connection between science, addiction, and recovery. Anna explains the neuroscience of addiction and how it affects the brain and decision-making. We get into how to prevent addiction, what to do if you are addicted and the different recovery options. Our convo gets into the subject of relapse including why so many fall back into their addictive behaviors even after sustained abstinence. She's also spot on about obsessing over your trauma, your brain is constantly in addiction mode and will latch on to anything and everything that stimulates it including negativity. When you obsess over trauma, it becomes a mood , and over time that mood becomes a temperament.

WoW. I couldn’t agree more, my brother. The Universe delivered this video ‘and’ your comment to me at the Perfect time and Moment. I was just Contemplating your thoughts and message, not too long ago, as I’m trying to make some really needed changes in my life. Opioid use got hold of me, unfortunately, and became a terrible situation in my life (Guilt/My Mind was the Biggest Component, as I simply ‘Knew’ what I was doing, creating, and that it couldn’t ‘work’ in my life- no matter how much control you think you have…). The Depression is so awful, as my Brain is repairing itself from years of taking them. It’s Incredibly difficult and i sometimes fall off for a day or three. Thankfully jumping right back on the Healing Boat, as My Desire to Feel Good is DOMINATE. At any rate… w/respect to your thoughts and comments, You Inspired Me, to Share My thoughts and feelings as well. The following Applies to Both, Negative OR Positive (Chronic) Thinking… 1) Thoughts, which if continued in the ‘same’ ways, becomes 2) Feelings, which if continued in the ‘same’ ways, becomes 3) Beliefs, which if continued in the ‘same ways’ AND for a period of time, becomes 4) Reality (Manifested), which if continued in the ‘same ways’ (same thoughts, feelings, beliefs, patterns of thinking, same routines…), becomes our: 5) Personal~Reality. Then, Ultimately and Inevitably, this Recipe (Intentional or ‘Not’…) has Shaped and Created, what is NOW (Known As, and) Become- 6) Our ‘Personality’. I am struggling, every 5-6 days I relapse. I know I’m strong enough to over come cocaine. It’s such a horrible experience. The idea of a clean future excites me. I will try hard as I can. Time will heal .. I’m going to focus on, excersise, sauna use, reading, and spending more time with family and friends..

Take Away...man I don't know where to start. The whole video was incredible. Great questions and thank you for your transparency. I'm about to start my journey to recovery from what I would say is stimulation addiction. In my addiction I just pick a flavor for the day (alcohol, weed, sex, shopping, working) really anything that will give me that high or that sweet spot. Of course alcohol does it the best and sometimes all of these in a day. I'm high functioning and on the outside I manage my life, family and finances well. But inside I'm about to jump off a cliff. If something doesn't change soon there might be a point of no return. I know I would be happier without this struggle I've just got to start my journey to get there. What I learned from your video is life is gonna suck for a few weeks or a month or so but that's okay. As we said in the military "Embrace the suck" lol. I can do hard things! Its okay to be bored. We weren't created to be stimulated all the time. There's peace in that quiet place. In short I guess that understanding that pain is part of life. Its gonna be ok! Good things come to those who wait!!

I was a professional high functioning addict with a high lifestyle. I moved to the outback before pandemic. Simplicity and nature, desert and sea saved my life. I live a healthy simple lifestyle like our ancestors. I am fulfilled, happy and grateful.

I come and go with alcohol ,and solphadine an opiate over the counter med here in Ireland , I stayed off the booze for a week and was fine but let myself get annoyed with some negative comments from a family member and reached for the bottle of wine out of anger and BOREDOM (my big return )to substance ...that said I surfed for the first time in years (splashed) more like it ..but now from tomorrow I start cold water swimming again im not looking forward to it but realise the pain pleasure reward Ann talks about , I did it for a year and it was great.. im also increasing my juice and veg intake as I know the nutrients intake helps..... its not going to be easy but I feel optimistic it will help me along the way, thanks again this is a BRILLIANT interview.

I am in recovery from alcohol, narcotics, benzos, alcohol, cigarettes, and entering my 39th year of total abstinence. I find Dr. Lembke's interview right on the money. She covered all the basis and left no stone unturned. She is definitely a healer. My personality has also changed from a convicted drug dealer to an enlightened " drug healer". I would love to meet her someday.

Gratitude attitude brings far better long lasting happiness naturally … just start with the smallest things like gratitude for a sunny day or the presence of trees and flowers, the kind action of a stranger or genuine greeting smile. The glass is half full attitude defeats all the negative emotions. Service of kindness to someone else with no need for a reward is what brings long lasting happiness…

Thanks for the informative information. I am just coming off my methadone tapering and have really struggled with joy and motivation to do daily tasks. I honestly didn't even think about my dopamine levels. Learning how to live again. This podcast helps because understanding how I feel will help me stay sober and not fall into old patterns.

Much as I love to hear you speak, I can't help but question your views of reality. Boredom is possibly a lack of appreciation and contentment, life is boring when you don't have a deep connection to self and or what you bring. Life can be deeply happy, and this is spiritual and results from healing trauma held within the body and allowing the spirit to shine from within. When trauma and addiction are present the spirit cannot reside within the body, which is incredibly uncomfortable. When we change and release the trauma the need for addiction abates, until it can no longer co exist. There are many alternative techniques which allow for deep processing of old emotions and rewire the neurobiology of the brain. The spirit is first and foremost who we are, not the other way around. Addiction is a lack of connection to self. You cannot connect to trauma you need a person to connect to their heart and once connected to themself, they connect to others and community is built, woth no place for addiction.

Not everyone has major trauma that led them to addiction. My life was good but drugs came into my circle of friends and I tried and got addicted after a few times. For me and many others the addiction itself was the only trauma. There’s no oh if I just remember that time I got spanked as a child and let it go I’ll be all good and no longer addicted. Can’t speak on behavioral addictions but Drug addiction at least doesn’t work that way.

I’m sure it can help for those addicts who had a major trauma but the drug addiction itself it’s own physical chemical imbalance that needs to be addressed first and foremost and than later in recovery what you mention can help for sustaining their recovery in the long term. Prob why that step that could address that comes in later in 12 step programs.

@@dougbopst1 That pain & pleasure are located in the same part of the brain. There has to be homeostasis between the two like a see-saw & that life is not meant to be euphoric on a daily basis. Hope that makes sense 😊

Great Video I have 8 years and 3 months sober and clean and it's because of My daily Connection to my higher power meditation journaling and having a morning and daily routine that includes spirituality and AA meetings. Also going to the gym and fitness is key for me😊

You are spot on! That "deep down emptiness" that you speak of is separation from our creator God. We are seeking to fill a void that only he can fill. The pain and shame of our sin coupled with the disconnect from God is a burden that we are not able to bear. Here is some wonderful news WE DONT HAVE TO BEAR THE BURDEN! :) The word of God says come unto me all ye that labour and are heavy laden and I will give you rest Jesus died in our place to save our souls from hell he paid the debt you owe for your sins on the cross! and he rose from the grave to give you eternal life which begins the moment that you believe this truth. The word of God says that if thou shalt confess with thy mouth the Lord Jesus and shalt believe in thine heart that God hath raised him from the dead thou shalt be saved. Once you are born again by grace through faith in Jesus you can have a relationship with him and that relationship as long as it is lined up with the word of God will bring you peace that passes understanding And he will fill that void that nothing else could fill

9:00 he says Happiness and contrasts it with pain I think he is equating pleasure with happiness Happiness is not pleasure Pleasure is not happiness. you can be happy and in pain at the same time

The best cure is not letting your mind love fears by stopping by thoughts that empowers bad drinking and eating, what ever you do or don't if its bad and defeats what spirit created, science can't master spirit or death that happens by fear of death that's dusk to spirit.

I have a lot of respect for Dr. Lembke and her work, this is probably the third interview that I've watched of hers and I will definitely be ordering a copy of her book. I do, however, believe that dealing w the underlying trauma is actually paramount to staying off of an addiction after recovery and that not doing so is at the core of why at least some people relapse. This is, of course, just a hypothesis on my end but I strongly suspect that it's true.

The sooner the disease of addiction is actually acknowledged by everyone as a "thing" the sooner steps can be made to address it , the world today is creating alot more addicts because there are so many more things to get addicted to , the fundamentals of physical addiction is now used in everything, take pac man ridiculously addictive music and that has led all the way to the current climate .

Really good questions. Interesting how it leads to traditional values that we were taught as kids (my generation, anyways): Do the hard things and go the distance. But society evolved to give us everything, with less effort. Social media, movies, advertising- all show us how being wealthy promises that we can have every exciting and sumptuous thing and experience, without effort; and isn’t that what addiction is? The fun without the effort.

This is great guys/gals :) Thankyou from a mum who needs to be informed.. P.S. I listen to Eckhardt Tolle .. being more present is important.. switching off the ‘stories/narrative’ we all (not just those with addiction), suffer from. Stopping the narratives, especially moving away from negative worrying thoughts, by observing our thoughts & not following them and obsessing with them and believing them.. try to not buy into them .. let them go & observe our breathing, observe our surroundings etc etc Thankyou so much for this wonderful talk 🥰👍👍

I think it's so funny how people seem to think that. It's sometimes a moral dilemma that people are addict and all this stuff because people like to feel good. And drugs make you feel good doesn't seem very complicated to me .... who likes to feel bad. I don't know anybody everyone I know likes to feel good so if you find something that makes you feel good obviously you're going to continue to use it. The reason that you're using, however, every attic design ever question has always been the same. They are so sad. They have lost something they love or someone they love, they are grieving. It is pain. It is grief. It is trauma that they are dealing with it, it is the root of their addiction. While it sounds great to sit around and deal with your trauma and all that it is not reality. This is where plant medicine and the science behind maps comes in. If you can take a plant from the Earth that God has provided though, and it can make you have a little visual hallucination that deal with your emotional trauma, which is absolutely the most important. And you are able to release that pain. You have let go of the root cause of your addiction. And Walla, things begin to happen.

Smoking my last two cigarettes trying for second time I am preparing myself like why I have decided to quit and quit smoking weed or hashish it is just time I take care of Natalie at 57 because I want to age well .I have to embrace stopping it is very challenging iam Alone my adult child gave me an ultimatum to change and I love her yes I had a rough life anxiety depression I also turned to sel mutilation body focus illnesses such as tricotillomania i wasn’t like this before I guess it is years of not letting go of toxic people and toxicity so good luck to us all we make the first step by deciding it

It's tough. It's really tough, bit the more you stay away from negative people , the better you'll feel around your peers. I wasn't ready for the÷ jealousy. People who used to use you, will be disappointed ÷.

I’m sorry but it’s almost always trauma. I tried the rooms for 12 years and for me it almost killed me. The shame of relapse etc. When I went to therapy and did the work the using, smoking phone etc fell away on its own

This is a good analogy, but it doesn't fully explain addiction at a physical neurological level, which has more to it. Eg. It's all about dopamine neural synapse receptors. Eg. Addictive substances that produce more dopamine, create more and more dopamine receptors, on the other side of the neural pathways between neurons. Eventually, over long-term addiction, there becomes many more dopamine receptors, in order to receive the abundance of dopamine. Once this happens, the brain craves that same amount of dopamine production, in order to accommodate the new neural receptors. And if the brain doesn't get that fix of dopamine that it has adjusted to receive, then that's where you get side effects such as depression and psychosis etc. For quite a long time, until the brain gets it's next fix. Otherwise, going cold turkey, will cause all these negative side effects, such as depression and cold sweats etc. The brain has to readjust to reducing stimulation over time. Eg. The dopamine receptors are like hungry little mouths, there to sponge up the excess dopamine neurotransmitter endorphin. Just like fat cells multiply to compensate for extra fat consumption and digestion. Eg. The body is trying to adapt to the new lifestyle diet.

Of course, it is. It's a better thing to use, then say morphine or fent because it doesn't give you a high per se. But it is an opiate period and using that. For years To get clean is a joke. Because how are you gonna get off that now? I spent 7 years on methadone and I spent 25 years in addiction. I got off of a 140 mg under 3 weeks using psilocybin And because of that I did not endure one physical withdrawal detox symptom at all. No runny knows no chills, no sweats. Nobody aches nothing and I did that. The beginning when I decided I was going to quit the next day after. I did my first dose and take the shrooms. I went down 20 mg held that for a few days. Then went down 20 more until I was at 0. I am now successfully off of 7 years of methadone a 140 mg, and I did it in 3 weeks time with the science of psychedelics in addiction.

Most people do not understand that methadone is an opiate agonist. Buprenephine is an opiate agonist, and suboxin is an opiate agonist, with an opiate antagonist, naloxene in in it.

Ironic Paradox; to live a good life; stop looking for happiness 1.2 AL: I tell people to do a counter-intuitive activity: instead of finding a way to comfort yourself, do something hard [ she sounds like Goggins!] Do something mentally and physically hard. Referring back to the pleasure-pain balance, this is why we do this. When we press on the pain balance, gremlins hop on pleasure side. We get an opponent-process reaction that will move our needle/baseline toward a pleasure homeostasis. By hormesis, introducing tiny toxicity, we activate body’s regulating mechanism [ endocannabinoid endo dopamine endo serotonergic systems] to upregulate. Hence, we are making our own dopamine. Do a physically hard activity; exercise is key. Hence do tasks where dopamine is not primary, but secondary, due to effortful endurance. AL: A fascinating thing: we all use rewards to frame time We use rewards to give our days shape. But if u take away rewards, instead of having an activity to look forward to, all you are left with is this moment. This could be all you have. This moment. that's exactly wrong what you said :"in life we're not always happy and in life there's pain..." that is exactly the mistake and exactly what people my except to take with them but when pain appears in there life maybe not on the first wave of pain they will use but a few waves after they will use. and that is because life shouldn't have pain in them at all, and the fact that it's so common is an epidemic of all wrong, of all being wrong, full on the wrong direction and wrong doing because of the wrong starting point

The reason people use is to numb their emotional trauma psychedelics, open the brain and the neural pathways to rewire. Not only your addiction receptors, but your emotional trauma is dealt with and lifted your brain is reconfigured.... Most people have no idea all the generational trauma that they're carrying around with them until they do something like ibogaine or peyote or something, and it opens them to all of that trauma. Let's them revisit that deal with it and let it go. And that's how they're able to release that and once that's released, which is the main reason their using anyway they are able to tackle that and move on.
