Chronic Fatigue Syndrome (CFS) and Myalgic Encephalomyelitis (ME) affect millions of people worldwide. Many lives and many families have been devastated by these syndromes, and the cost to both communities and society as a whole in terms of lost productivity is enormous.
Yet they remain Cinderella conditions, poorly-researched and under-treated, orphans in terms of medical effort. Worse still, their annexation by the psychiatric establishment in countries such as the United Kingdom as psychological in nature has lead to untold misery. Patients are desperate to recover, but are told their illness are the results of ‘faulty belief systems’.
Dr Sarah Myhill is one of the world’s leading physicians in the fight against both Chronic Fatigue Syndrome and Myalgic Encephalomyelitis. In this fascinating short film, she describes her journey from general practitioner working in the National Health Service (NHS) in the UK to a doctor specialising in the patients other doctors considered hopeless cases.
Initially specialising in allergy, Dr Myhill found that dietary approaches such as elimination diets produced good results, and often also helped her patients with CFS/ME. But there was a group among them who did not improve, and she began a forensic level of research to find out why.
Thinking about energy production and the human metabolism, she looked at how we create the energy we need to live, and how we spend it. An imbalance on either side of that equation can lead to fatigue, exhaustion and disease. Working with the brilliant Dr John McLaren Howard, she was the inspiration behind the creation of his ATP profile test. The most basic unit of energy production in our bodies are our mitochondria, the tiny organelles that live in each of our cells. It is our mitochondria that are the tiny power stations that create our energy. By creating this test, and carrying it out on a cohort of patients and matching their clinical profiles with their results, Dr Myhill, Dr McLaren Howard and Dr Norman Booth, a physicist from Oxford University, developed the world’s first objective measure of mitochondrial function, which is an essential part of human energy production.
In describing her model for recovery to her patients, Dr Myhill uses the analogy of a car. You need the right fuel for the vehicle to run - no good putting diesel in a petrol engine. You need a functioning accelerator to move forward - the work of the thyroid gland (which is discussed in more detail later in this playlist). You need a working gearbox - the adrenal gland, which is such a critical player in the production of many hormones required for life and for energy. And because life is always a struggle between getting the nutrients you need to live, and processing both natural and man-made toxins, you need a good catalytic converter - the role played by the liver and the kidneys.
Empowering her patients to take responsibility for what they eat, how they live, and how they engage with the process of regaining their health, has always been a guiding light for Dr Myhill. Much of my life is either spent in bed or on the couch. It's depressing as hell. I think many people believe I'm just "lazy" which makes CFS even more depressing. I'm looking forward to reading this Dr's book. I hope I can get some relief .

I can relate. People are quick to judge but don't have a clue. I lacked energy for 6-7 years. And I was gradually losing my hair and I felt really tired and bad. I rarely left the house. But I realized that I was not eating the right food. I decreased my caffeine intake. I consume a spoon of coconut oil in my morning coffee daily. I added a green superfood powder to my apple juice. And I recently started using adaptogen herbs supplements. Now I am trying to incorporate more fruits and vegetables to my diet. Chia seeds are good too I've heard. My energy is definitely increasing. I just wanted to share.

She is selling books, others sell medications others therapies. Unfortunately it is left us to decide who do we believe in. We went from patients to consumers. yes, I do understand she has to make a living and this has nothing to do with the work she put in or the information she is providing.

I am from Ecuador, it is a small County in southamerica. Sadly, nobody here believes that fatigue existes, and many people think I'm just lazy. I have tears in my eyes right now because you are telling me that I am not crazy. I miss my former life. After I got my thyroid removed, my life changed. It is a great one of a kind product for getting rid of chronic fatigue syndrome without the headache. Ive heard some amazing things about it and my old buddy Taylor after many years got amazing results with it.

So clear and precise. Straight to the point. Just researching my child's possible condition age 12. A dancer, Scout, athletic. Now nothing! Drained. Not diagnosed yet, but I know they have it. Excellent video, very concise overview. For five weeks recently I was on a gluten free, low oxalate whole foods diet (no sugars, refined / processed foods) whilst supplementing with high doses of quality marine phytoplankton (DHA), chlorella (detoxification), allimax / allicin (antimicrobial), digestive enzymes, garlic seed extract (anti-oxidant), glutamine (soothes gut). BUT TO NO AVAIL. I don’t think I was consuming too many carbs. What did I do wrong? Too little or too poor quality sleep (6hrs somewhat broken)? Too much brown rice (250g pd 80kg male)? Blood tests have shown no issue with thyroid. I need to get hooked up with dr Myhill.

I think your work is great. The only thing I can't wrap my head around is that I had stopped carbohydrates well fast carbohydrates nearly completely I lost a lot of weight. But I got even more fatigued this was before I had chronic fatigue syndrome. Before that I had postural orthostatic tachycardia syndrome and that is why I went on the low-carb diet. Once I put carbohydrates back into my system my energy levels got higher. I would notice though after eating them I would feel tired but I did notice during the long run my energy would be higher. I know that carbohydrates are considered one of the main sources of energy used in the body. I had read that if you cut carbohydrates to a low carbohydrate diet this can cause more stress on your body. You start to go into ketosis mode which your body is burning fat as fuel which is why are you lose so much weight. But what I have read is that it's more stressful and harder on the body to burn fat because it's much easier for it to burn carbohydrates for fuel instead so your body is working overtime. Also carbohydrates I had red were important for many different functions such as the brain, the adrenal glands and many other areas of any energy production and so on in the body.

MsAlexandraraven I experience something similar, and found is that I did really well on the zone diet which is moderate and not low-carb and was very easy for me to maintain for five years. While there are aspects of it slightly more complicated, I just found that if I stuck to the diastases of getting around 30% of my calories from protein at least 30% from fats and no more than 40% from carbohydrates whenever possible, I never got tired after eating like I had experience my entire life. I’ve been doing a ketogenic diet now for a year, and don’t notice much extra energy, in fact I actually get tired after a meal which never happened on the zone diet. I’ve stuck with it because it helps so much with inflammation has drastically less and migraines and arthritis issues, but hope to be able to go back on the zone diet. It’s so simple and all about balancing macro nutrients and blood sugar in a way that decrease inflammation and increase overall health energy and performance. Ironically my strength went up in the gym, and I was so surprised because I had to cut my carbs down by 2/3 and b4 that I had always noticed carbs made me stronger and feel better working out. Hope this helps.

interesting she referred as us as a car... my Endocrinologist made the same reference to me when she dx me... your like a car going 120 miles per hour on fumes and you have burned up all the fumes so its burnt out...and it dies....it cannot run anymore as there is nothing inside of it to consume- that is you on deaths door... I certainly was in a very bad state by the time I found this endo,, I had gone to allot of doc for years who could not be bothered with finding the cause of my condition.... (I am in the USA)...

Adderall works great for me! Discomfort after exercise? Goes away after I got my muscles back in shape! Sleep great now too! Micro-doses like 5 mg/a day. No more brain fog! It works for me! More organized, things running smoothly! I eat less, am far less hungry so its easier to control the diabetes! I know why docs are worried about prescribing it but I believe the AMA & Company are still in 1965! It's cheap, quite easy to stay in compliance with supervision. "Speed" or Adderall, or whatever you want to call it - it works!

Where can this mitochondrial function test be done? How can it be requested from a physician and how is it done? How can they be convinced that it's necessary if they aren't familiar with it, without resulting to sounding like "I googled my own symptoms so I know better than a doctor"?

I really feel for you Michael, I was once in that position myself. I guess I got through that bit by focussing on the little bits of my life I did have some control over, and cutting what I knew was unhelpful - in my case, sugar. Good luck, and I hope things ease for you.

I was told that it was all in my head and that the inflammation was in my head and we don't treat that here. No reverse T3 or Hash imotos test allowed either. I can't do thyroid Px or diat.earth they tear my stomach up. Do cfs people have a hard time processing. Amino acids ? I wonder if beans are bad to eat there so easy to make when your sick I will be sure to buy her book.

It makes me furious every time I hear someone who is suffering is not allowed access to the relevant tests. Dr. Myhill does talk more later in the playlist about thyroid issues. As for individual foods, for myself, the only way I know for sure is to take them out of my diet for a while, then put them back and note how my body responds. I also have to keep my expectations of myself and what I can do realistic. There's no perfect way to do this stuff! Hope today is a better day for you. Sian, LTBM team member

How can we get any of the recommended tests performed? GPs don't want to know. And then there's the issue of could a layperson interpret the results.

I think it probably depends on the level of your fatigue Mariska. One of the things I do is to try and really listen to what my body is telling me. I've learnt to live differently as a result. There's also a good source of information to be found at Dr Myhill's website - drmyhill.co.uk

I'm pretty sure that my physical breakdown into CFS at the age of 42 was due to the low fat diet advice current, over training on my part and the stress of being a single mum with a full time teaching job. I've never regained the energy that I had before that. At 64, I am eating a very low carb high fat gluten free diet and feeling better. Your supplements sound interesting.

I eat well, don’t overdue it, do stretches and some yoga every morning, walk in my neighborhood two to three times a day, am in good physical shape (blood pressure, not overweight etc.) and am dog tired, dizzy, incoherent nearly all the time and it’s getting worse and worse. What medication can help me? My sleep isn’t the best and I take a medication for that it’s the fatigue that’s the most debilitating for me. There needs to be a medication FDA approved for ME/CFS if there is one I’m all ears. Seems in the past all my doctors wanted to give me was an SSRI and I hate those. I invite all those that are taking medications that help them. No disrespect but I’m not looking for a diet type plan, just medication. Thank you,

hey doctor, so im trying to look all over the internet to see if i can find some sort of cure for a very dear person to me, so his syndom is muscle catastrohpe, basiclly what it means is that he cant recover as fast as we normal ones, lets say if you workout and over the night you recover, but for him it takes x 5 times longer to recover, which it leads to him not being able to do much now days, he cant gain weight, he have very slow movments, being sick from work and all thing for over 10 years, just spending time home, and the doctors in sweden are saying there is no cure for him out there yet, so im just trying to find out more about it. would love for a feedback to where exactly i can search for more information about it and see how far the doctors around the world have come about that syndome. thank you for your time.

How come ur a doctor who gets it but I have been to a buttload of doctors who don't no compassion and r stupid. I hate to say it seems like ur one of the few smart doctors out there..

I have severe CFS, but I baffled by the completely conflicting information on diets that nutritional doctors recommend. More and more doctors seem to be advising plant based diets now, with an array of evidence supporting this. I don't see how high carb vegan diets and low carb keto diets can both be beneficial to health. Surely someone is wrong here. I trust pretty much everything Dr Myhill says, but I am unsure whether the keto diet is sustainably healthy in reality. For example, the eskimos who eat primarily animal fat/protein actually have a genetic resistance to going into ketosis, which suggests that actually ketosis is not a preferred metabolic state of the human body. Any ideas?

I don't know Conor - but I completely identify with how hard it is to make your way through the maze of conflicting information. Dr. Myhill's position is pretty clear, and her website drmyhill.co.uk and new book, the PK Cookbook, lay out her thinking. I feel better on the PK diet than I did off it, but I can't answer your question on sustainability, I guess time will tell. There must also be individual variation in what suits each person. If the opportunity presents itself, I will run your question re Eskimos and genetics past her. Do you have the reference? Hope today is a not too bad day. I have Lyme disease myself, which was initially diagnosed as CFS and then as ME. It's a bugger... Sian, LTBM team member

I am very sorry, I forgot about this comment, but I have included the articles below regarding the Inuit diet and ketosis. If you or Dr Myhill (or anyone else) could explain these and why they don’t suggest against a keto diet then an explanation would be appreciated 😊. Heinbecker (1928) www.jbc.org/content/80/2/461.full.pdf+html , (1931) www.jbc.org/content/93/2/327.full.pdf+html and (1932) www.jbc.org/content/99/1/279.full.pdf+html suggests that Eskimos are not in a state of ketosis while fed their traditional diet. Even if they were in ketosis, the health of the Inuit isn’t something I would personally strive for, and they are shown to have similar levels of cardiovascular disease to non-Inuit populations (Fodor 2014): www.onlinecjc.ca/article/S0828-282X(14)00237-2/fulltext Furthermore, from what I can understand, recent genetic studies seem to suggest that the Inuit had a selective sweep of a mutation which actually reduces the prevalence of ketosis in these populations: www.cell.com/ajhg/pdf/S0002-9297(14)00422-4.pdf I think by reducing fat movement into the mitochondria. This would seem to me to suggest that they have adapted away from a long-term ketogenic state, and one could assume potentially because it isn’t a healthy state to be in constantly for the human species. I believe that this is because the human species never adapted to eating such high quantities of animal products. There are several articles that suggest that we evolved (even in the paleolithic) eating largely plant matter. I won’t link them here because I want to keep this focussed on ketosis, but I would be keen to debate all of this more, if anyone wishes. I am just stating what I have read, and don’t claim to know everything on the topic, but this counter-argument does seem fairly convincing in my eyes. If someone can refute it then I would appreciate any advice or evidence.

Fatigue is only one symptom amongst a myriad of symptoms for M.E. . I'm not a car and it doesn't matter what 'fuel' I put in my body I will still be sick. It is simplistic and silly to make these kind of generalisations. A person with M.E. would never say these things only someone who will profit by selling the hope of recovery where there isn't any.

When our mitochondria are damaged, the Energy furnace component of every cell it converts glucose in an oxygen deficient environment to Lactic acid. It's called anaerobic glycolosis. That's why Minimizing carb intake, or eating smaller portions more frequently is beneficial. An experiment is to Cut out ALL cow dairy products for a month or so and see if your pain levels decrease. My theory is that our cellularly Lactic acid is first cousins with the Lactic acid in cow dairy.

Sound like a lot of good info, but continuing with the detective work did she have any impressions of the causes? All the causes may create the same problem with the mitochondria , but when I read about people having Lyme disease, I think those people have nothing to do with me. I lost my energy after an accident. 3 month later it was just gone. that was 10y ago. I would like to know why, not to try everything like a post below listed everything but toad legs. Thats what doctors do with medication too.

I can't say for sure but from what I'm learning it's because ANY major stress to the body (added to the environmental toxins in our lives/food) can cause our energy production bottom out. The body is attacked (by a virus, lyme, an accident, etc)and may be staying in attack mode or just has no reserves left--never gets enough back at a sustained level to feel well again (life is never stress free and our diet is sadly lacking, etc). My CFS started after childbirth 15 years ago (was already showing warning signs that I ignored, I was 33 when I had him). I'm some better now but definitely not "well." Best wishes to you and your search, I'm going to read her book next!

So true every time I mention my shoulder , knee's or finger's "arthritis " pain either suggest a steroid shots , operation to hack me up , or ibuprofen which if anything make matters worse .. Money grabbing ,pill pushers ! Since I've been on Keto /LCHF my health has improved more in these 4 years - than all the years I've seen doctors .

Chronic fatigue : difficult to find something. The radiation of microwaves (magnetron)oven creates tension in the muscles.That causes pain and also bad working of the guts: food is absorbed badly,so you have to little vitamins in the body. Also bad feeling in throat and tension in the rest of the body. Tension in the muscles is also bad sleeping.

Please have a look at the Gupta program/Amygdala retraining. I went well with it and I was absolutely disabled. The cause for the condition is an abnormal function of a brain structure called the Amygdala and the Insula. You can retrain the brain with very special techniques. It's neurological and has nothing to do with psychological issues or depression.

I did wonder about this Doctor as I have CFS, it worries me a lot she she is expanding her skills into other fields such as Diabetes. As someone that has actually extracted Mitochondria from plants and animals and worked on them I am struggling to see how you can actually test Human Mitochondria without having to donate rather a lot of body tissue for the extraction, I like my liver and don’t much fancy donating a chunk for an ATP/ADP assay. After seeing this video I’ll add her to the list of nearly but not quite, maybe one day someone will find a cure a be altruistic enough not to write a book giving you the answers, as long as you buy it of course
