Peripheral neuropathy is a common condition that I see every day in my office. It's also one of the most frustrating conditions to have and to treat. My name is Dr. Andrew Schneider, and I'm a podiatrist in Houston, TX.
Peripheral neuropathy is nerve damage that affects the extremities, both the hands and the feet.
There are many causes of peripheral neuropathy. Too many to count. That's part of the problem with treating it. The most common causes include diabetes, aging, vitamin deficiencies, chemotherapy, alcoholism,
smoking, and some medications.
Let me also be clear, that in some cases, peripheral neuropathy is caused by the aging process. While we can help calm the symptoms, we don't have the fountain of youth. In fact, if a nerve is completely damaged, there's no way for it to regenerate.
The reason that peripheral neuropathy usually starts in the toes is due to the simple fact that your toes are the farthest point from your brain. Your brain receives impulses from the nerves and translates that into sensations. From your feet, all the way to your brain...that's a long way to go and plenty of opportunities for things to go awry.
Nerve pain comes in different forms. Some people feel a numb sensation. Others feel a burning and tingling sensation to varying degrees. Some feel pain so intense that it feels like their feet are on fire.
It's important to differentiate nerve pain from other causes of pain. If your feet are hurting, when you're standing and walking, that's usually a musculoskeletal pain such as tendinitis. If the pain primarily occurs when you're off your feet, such as when you're laying in bed at night, it's more likely to be nerve pain. To further muddy the waters. If you are pain-free when you go to bed, but you're awakened by your feet being in intense pain, this can be due to a circulation issue.
Some topical treatments do help nerve pain using a topical analgesic that contains menthol or capsaicin. There are many such products available over the counter. These analgesics work to provide another stimulus and essentially confuses the nerves as they send impulses to your brain. This can help reduce the pain you are experiencing from peripheral neuropathy. Many people have found relief using topical CBD preparations.
As I said, the pain you're feeling is often due to a combination of factors. Nerves travel along with other structures, such as tendons. If the tendons swell, it puts pressure on the nerves and can increase the nerve pain. That's why stretching, which is helpful to mobilize the muscles in tendons, helps to reduce the pain from the nerves.
If your foot flattens when you stand, that causes the nerves to stretch and become painful. Wearing insoles or custom orthotics to support the foot can also take pressure off the nerves and reduce the associated pain. Don't minimize how something so simple and approachable can make a difference in the pain you are feeling.
Nerve pain is also linked to circulation problems. When there's not enough blood flow to the nerves, they're deprived of oxygen and increase in pain. Massaging your calf and thigh helps increase the blood flow. Compression hosiery also provides support to the circulation. If you can increase your circulation, you'll often find some relief from nerve pain. Also, it is essential that you don't smoke. Smoking is one of the most preventable risk factors and can lead to poor circulation.
If you're in a situation where you feel like your feet are on fire, we can deaden the nerves with a topical lidocaine cream or patch. Some medications treat nerve pains such as Lyrica and Gabapentin. The problem is that these medications have significant side effects.
If you're diabetic, the best way to lessen the nerve pain is by getting your blood sugar in good control. It's not an easy thing to do, but it is worthwhile to minimize the side effects of diabetes, including peripheral neuropathy. Taking medication or insulin is not enough. You need to control your diet to lower your blood sugar.
Finally, sometimes peripheral neuropathy is caused by a physical impingement of the nerve. If I suspect that that's the cause I'll refer you to a neurologist who can measure the nerve impulses as they travel to your feet. Sometimes physical therapy can help a nerve impingement. Other times you may need surgery to free up the nerves.

I have neuropathy in both of my feet, seems like the right foot is worse at times. I have been to foot and ankle doctors and was told to just wear compression socks, although they do help some they do not help with the stabbing pain on the bottom of my feet. I started to do my own research and found that Alpha Lipoic Acid would relieve most of the pain as well as vitamin B12. I figured that I had to start some where so I started to take alpha lipoic acid first (it does take up to 3 weeks to notice the results) let me just say that it's helped me so much!! I also soak my feet at night with 1 cup of white vinegar and 1 cup of baking soda. I have little to no pain. After taking the Alpha lipoic acid for 3 months I wanted to see about taking vitamin B12 it works as well but in my opinion the sundown 600mg alpha lipoic acid works for me. I just wanted to share this with others. I realize that everyone is different and just am hoping that this helps someone else. God Bless, stay safe and take care!!

Funny I just had a conversation today about neuropathy with a lady who turned 103. She’s amazing, she leads the exercise class in the personal care home, plays the piano for their resident’s Bible study. I grew up in the same rural community and hadn’t seen her in 50 years. She remembered my name and all of my brothers names. I left home after high school (now 70] so she caught me up on the community gossip. Just an aside…. Good info.

I was diagnosed with Diabetes April 11th with an A1C of 12.6. The doctor said i must have had diabetes for "a long time". I was in the hospital for 3 days from a kidney stone and thats when they diagnosed me. When I got home and for about 5 months peripheral neuropathy hit both my feet: burning, uncomfortable stinging, tingling, especially at night. I found an online supplement that has the following: Vit B12, B6, D3, Benfotiamine, Alpha-Lipoic acid, and Acetyl L-Carnitine...I am just over 4 months into taking the supplement and my feet are about 95% back to normal. I significantly changed my diet, and my next A1C test on August 1st was down to 5.6. Most nights I check my blood 2 hours after I eat and I am regularly getting readings of about 97-115. Don't give up, if you were like me and neuropathy is in beginning stages, those supplements will 100% heal you.

If you are in a Cannabis (Marijuana) Approved area, I have found exceptional success with SMALL amounts of hybrid cannabis to relieve 90% of any symptoms. It takes about 20 minutes if I use drops, or with vape (which is very mild) it is quicker. Here, in Canada, my doctor suggested I try it. Years later, it still works on the neuropathy, but it hasn't given me any buzz after using it for the first 3 months. But that's fine by me! But just in case, DO NOT DRIVE WHEN CONSUMING CANNABIS, even if you don't feel buzzed.

I'm so much pain I just want to die!!!! My Doctor first stated me on 300 milligrams of Gabapentin 3 times a day and 5 milligram Percocet 2 times a day. Truthfully I was taking the Percocet 3 times a day and it helped me with the pain that the Gabapentin didn't work on. And I was truthful with my Doctor about it. He then reduced my Percocet to once a day at bed time. And upped my Gabapentin to 600 milligrams 3 times a day. The he took away the Percocet because he was worried about addiction. But of course he's not worried about me being addicted to Gabapentin, Now I take 1200 milligrams of Gabapentin 3 times a day. And all I do is lay in my bed in pain. I'm sick of it and I'm thinking about just ending it all. I know it's my fault for not getting my Diabetes under control but I'm sick of the bullcrap about the "Opioid Crisis". My Grandfather took Opioid pain meds for 25 yrs after braking his hip. Without them he would have died way sooner than he did. It's okay for us to be addicted to other drugs that don't help us but not the ones that do. The only "Opioid Crisis" in America is the people who want to get high. I don't want to get high I just want to be able to walk again and get out of my F-n bed. But hey! At least Doctors are getting rich with kickbacks from Big Pharma and seeing one Patient every 15 min.

I’ll agree that the opioid crisis has led to collateral damage for those who truly need these medications. I’m sorry you’re in such miserable pain. Look into a treatment called Qutenza. It’s reasonably new but is helpful to treat neuropathic pain.

I was on crutches, never to walk again,I then had three table spoons of powdered tumeric in about three litres of water, I then drank this over a hour period. Went to bed, woke up , I am walking again. Still take smaller amounts of tumeric now. But I firmly believe in litres of pure water and tumeric powder. If you still can't walk you are not taking enough tumeric powder and water. Added pepper may help. I've been walking again for nearly three years. Try it ,your worth it.

I have mild peripheral neuropathy. I don't usually have any sharp pain. And most of the time my feet do not get "burning hot." They just get hot and they feel jumpy and I get this urge to rub my feet into each other. But I did find something that gives great relief to me almost every time. I give my feet a foot massage with coconut oil. You Tube has some good videos on the practice. I spend 3-5 minutes on each foot and lastly I hold my toes firmly in my hands for about a minute. Then I wipe them dry with a paper towel and return to bed. I am continually amazed how it cools and calms my feet so I can sleep. Of course, this may not help everyone. But it is free and is very unlikely to have any negative side effects. I hope it helps others like it has helped me.

Thank you for an excellent, informative and very helpful video. I am 65 + male, and some years ago (pre COVID) I became aware of some numbness in my feet (both) + periodic tingling in my fingers. Diabetes is in the family and I presented my symptoms to my GP who arranged an HbA1c blood sugar test - it came back negative, so the GP referred me to a neurologist who performed the tests you mentioned: testing nerve signals. That also was negative. At this point the GP seemed to lose interest. A couple of years later my symptoms were persisting and, I would say, getting worse and I went through the same process again with the same outcome. I then kinda gave up, thinking “maybe it’s just me getting old”. Then COVID happened and accessing medical services became v difficult. The symptoms continued getting worse, especially after eating sugary food (cake + chocolate which I love!) or a lot of carbs (a whole pizza or big portions of pasta etc) and I became suspicious that despite the HbA1c tests always showing negative (they “look back” over the previous 3 months or so) I *must* be diabetic! Recently I have done some research on the internet which flagged: 1. HbA1c is unreliable. For a significant number of people (20%?) high blood sugar does *not* show up 2. Vitamin B12 deficiency can cause neuropathy as it is essential for healthy nerve function. Action - I have taken the following action - assuming I have diabetes and being more careful with my diet (modest carbs, no cake, and just 2 squares of 90% cocoa chocolate a day) - taking vitamin B12 10ug/day (+vit C+D) supplement - trying to do more exercise. The symptoms have not vanished but I no longer feel nearly so much pain which had become much worse in my feet and also my fingertips (to the point I had to use my little finger on the iPad). I intend to present this “empirical” data to my GP and hope, this time, he will give me helpful advice that is tailored to my experiences! Hope this account is helpful.

If you think you can simply cut down on your daily carbohydrate intake to assist curing your diabetes. You need to eliminate ALL carbohydrates from you daily food and drink consumption. Being selective about what carbohydrates to drop from you daily meals Or snacking intake is absurd. Drop ALL carbs. You will never achieve the weight reduction success your body deserves. I was a 200lb mobile fat berg with rapidly failing memory. Now I am a slim 12st and can walk further than I could as a far carb-junkie.A life with zero carbs is vital if you really want to cure your diabetes or simply to live to see your 70th birthday like I do next year. Your body will thank you by adapting to it’s new zero carbs life very quickly. You will notice that your brain becomes more alert and your sleep will be so much more refreshing too. I was really shocked at how easy the initial change to nil carbs was and how now a few years on, I am more mentally alert, can walk further that I used to, am more mentally alert and enjoy life more than I did when I was a carb addict. Please decide to drop carbs from your life. You will be very pleased at the results. Search the web for carb-free menus. You need to decide right now if you can adapt to a carb-free diet. You must if you want your life back. If you do want to live to a good age, then get serious about eliminating carbs from your life.

When taking ALA, be sure it is the R -type not the S- type. R-type is more expensive to harvest but is far superior to the S-type. I have found that suppliers of ALA can be reticent about the type of ALA they offer, some don't even know about the types. Also, take Methylcobalamin not Cyanocobalamin B12. The cyano is derived from cyanide and is cheaper and not as effective as the methyl type, Hence, it is used in most supplements. Down below James West says the same with additional supplements which I too endorse.

I've started having the PT use the dry needle therapy on my hands and feet. It has immediately helped the arthritis but the neuropathy will take a bit longer, it helps if the condition is a circulation issue. I did notice immediately the color of my toes returned to normal. Medicare pays for the therapy if your doctor prescribes it. Neuropathy may or may not be completely healed but it's worth a try, no pain involved.

I've tried several things to treat the neuropathy burning sensation in my little toe while sleeping. The only treatment that works for me is a cool (ie. air conditioned to 60 degrees or less) bedroom with my foot exposed to the cool air. Socks with a hole cut to expose the toe prevents the rest of the foot from getting too cold. Strange and looks dumb, but it works for me. I'm 80 years old and otherwise in good health.

Just wanted to say thank you for this video. I am 68 years old and I deal with a numbness in my feet, sometimes it is painful but mostly just numbness. I was going crazy trying to figure out what was wrong. I have no illness to blame it on so it was troubling. After really thinking about it ,l believe that during the pandemic l was not walking as much and not even moving around the house as much as before. Sitting watching Netflix a lot . I found great relief in using a foot massager every morning, in fact my feet were much improved in two weeks. Just plug it in and 10 mins later ( l have a coffee) I am done .I still am not out walking everyday ,l don’t know as l will take that up again as l do feel like l am not up to it anymore ( l think l grew old or lazy in the pandemic) but if l can control the nerves in my feet with a massager l am very happy. Along with what the dr had to say give a foot massager a go.

He was wrong right off the bat. Nerves do regrow when they have the chance. Remove the factor damaging the nerve, and it will slowly heal. It does take a long time, which is why it is assumed the nerve doesn't heal or regrow. It takes a long time. I have healed some of my nerve damage using Yoga and nutrition. It might take 20 years, but the nerve will try to heal.

I've had idiopathic periprheral neuropathy since 1999. It appeared during a hospital stay due to leg thrombosis and which turned into lung embolisms. I was first told "no treatment, no cure, so suck it up". 12 years later I went to another neuroplogist who first did 3 days of testing, first to determine what limbs were involved (both feet with symptoms, both hands without symptoms) as well as a beginning of exclusive diagnostics to rule out possible causes. She then referred me to the head of neurology at the local hospital who repeated her tests and added some more, including a lumbar puncture, which ruled out HIV and MS. Since no one could find any causality, it earned the title "idiopathic". The neurologist put me on Gabapentine and Cymbalta. Minimal pain reduction but pretty horrid side effects, both physically as well as cognitive of nature. She switched from Cymbalta to Effexor. No reduction in the negative side effects. I've been on a regime (what my neurologist calls my "cocktail" of substances, all with solid empirical studies behind them, some alone, some in conjunction with certain other substances. I've tried also some substances which were empirically just as promising, but which didn't help, either not at all or in the long run - for example CBD oil. I would add substances throough a trial and exclusion process. Three weeks on and documenting my subjective pain levels. Then three weeks off, doing the same documentation. Again ALL these substances are scientificically tested, with several double blind studies, not just one or two. What I now take on a daily basis which keeps the pain in check, but does not totally elleviate it: R-Lipoic Acid 300 (240) mg - 1-1-1-1 Acetyl-L-Carnitine 500 mg - 1-1-1-1 N-Acetyl Cysteine (NAC) 1000 mg - 1-1-1-1 Omega 3 (fish oil) 1000 mg - 1-1-1-1 Borage Oil 300 mg (GLA = Gamma Linolenic Acid) - 1-1-1-0 Magnesium 300 mg - 2-0-0-0 Both my neurologist and my GP have my list (which I also keep in my wallet - it also contains a brief summary of my other medical conditions and prescription medications I take) and have patients who similarly don't do well with Gabapentine/SNRI drugs and try my "cocktail".

Non Diabetic Peripheral Neuropathy runs in my family. My Father has it, my brother had it, and now I have it. My brothers was so debilitating he died of a morphine sulfate overdose. My father takes a opiates to deal with the pain, has for years. When it hit me in my late 40s I was referred to several doctors who gave me many drugs like Lyrica and others that did little to help. After all kinds of tests, worthless injections and referrals I was eventually labeled a drug seeker and left to deal with this pain on my own. One doctor told me because I don’t have diabetes I shouldn’t have Neuropathy pain. I was in so much pain I became depressed and lost all hope. I finally did some research and found some people said Suboxone was great for nerve pain. So I faked having an opiate problem, got on Suboxone and finally found relief.

I sympathize with you. I'm not diabetic & have peripheral neuropathy. In beginning was told my two numb tingling toes was arthritis. 7 yrs later, I can't walk without debilitating pain & loose my balance as well. I now use a cane & looking into a small portable scooter to atleast be able to get out side for a roll about. Can't get a doctor to take me serious regarding the increasing issue. No doubt, in near future walking will be extremely minimal & only in my home. It's not what I had planned at this age. 😒

I have been dealing with non-diabetic nerve pain for about 3 years ,ever since I was diagnosed with L-4 L-5 problems . I have tried everything shots, diet, exercise, foot soaking ,acupunture ,massage , chiropractor, vitiamins.Until recently , I have found something that works for me,and I would like to share..First off shoes are important , I see a foot Dr every 9 weeksfor maintence. .we walk a half and hour everyday. For some reason, certain fruits have a negative effect on me as soon as they touch my mouth,so i disregard eating them .I found this by accident and continue to do this every morning. I still need my caffinenated coffee when I wake up, thats not going! I first , chop up and boil a redskin potato .I then beat and egg with a little milk , then I take some of the potato and mix it with the egg and milk. I get the frying pan ready with virgin olive oil and very little butter. I pour the egg milk potato mixture into the pan and after a minute or two I flip it over. until done . I found that the hard seeded rye has also some effect on my neuropathy. I toast it and here again very little butter or olivio spread. I do use a unsweetened apple sauce insted of sugarfree jam ,because there is no negative effect. I will tell you this , when I am eating this first in the morning most important meal ,I feel a slight warmness down my legs to my feet , calmness in general .I have been tested for everything under the sun and used creams , b-12 b-6 etc etc .This and watching salt intake has worked for me. I have been cooking only fresh vegatables and no processed foods for the past 2years and only till now has the egg potato worked for me..... all them pills ,turmmeric , ginsing , that everyone claims, has done nothing, just sayin. i i

I underwent 15 sessions of chemo over 6 months after cancer surgery. Neuropathy during the chemo took the form of intolerance to cold, to the point where I had to use a glove to take products from the fridge. Right after the end of chemo, I started getting numbness in the toes when I lie down. No pain, just slightly unpleasant. The right one is more seriously touched, but there is no I pauvrement when walking or running. It’s slowly getting better, but 6 years after the end of chemo it’s still here. And I think it will remain as the Folfox products do destroy nerve endings…

I've been dealing with extremely uncomfortable feelings in both my feet that do not ever go away and they started a day after a long walk at an airport. it’s worse and more noticeable at night and it makes sleeping a daily struggle. Sometimes it feels like my feet are on fire with nerves firing, like I just ran a marathon and it always feels sore along with a tingling sensation that kind of resembles pins and needles. The biggest worry for me was diabetes but I taken blood tests and doctor confirmed I am not even borderline diabetic. These symptoms started happening to me a day after I took my dad to the airport. It was a huge airport and I walked around a whole lot and the next day I got this agonizing pain on my left foot that had me screaming in agony, then hours later the same pain on the right foot. It was extreme pain I couldn’t walk for a day. The extreme pain eventually started to fade but unfortunately it left this other pain and tingling sensation in both my feet that I did not have before that day. Could a long walk at the airport really give me neuropathy? Is that even possible? I have not been responding to neuropathy meds and so I am conflicted and part of me thinks maybe it’s not neuropathy that’s causing my foot pain and the tingling. I’ve been seeing a podiatrist for over a year and they still haven’t been able to give me some relief so they kind of ran out of options and sent me over to a neurologist as they suspect this could be caused by peripheral neuropathy. I told them I suffered from really bad plantar fasciitis and so they thought maybe that’s what it is but after injecting both my feet I felt no relief of the pain at all and that’s when they figured out it’s more than just the plantar fasciitis. The neurologist did two nerve tests and found out some abnormalities with the sensory nerves in my feet and so prescribed me medication for Neuropathy I was finally hopeful for once that I would maybe get some relief. Unfortunately by now I already tried three different drugs used to treat neuropathy and none of them worked for me or relieved my pain. Gabapentin, Pregabalin, then finally Duloxetine. Sometimes the neurologist would say take an extra dose but it did not make a difference. Please help any advice is appreciated I am tired of feeling so much pain in my feet and doctors failing me.

I got it from platinum-based chemo. It faded somewhat over a period of a year after the end of chemo but some of it is still there. There was some research on a drug to get rid of it but I haven't kept up with the literature. The main issue is when my feet are on cold surfaces. Platinum-based chemo can cause extreme cold-sensitivity. So a good solution is to wear socks most of the time or make sure you don't walk on cold surfaces. Not always easy to do as I'm in a very cold climate. I'm also a runner and I think that helps with blood circulation - I suppose that might help too.

Thank you for explaining what is happening. You’ve given me more info than my local medical care team has bothered to do. I’m so frustrated that my neurologist said there is no evidence of nerve damage in the lower extremities. But I have feet on fire at night and so SOMETHING IS WRONG! a a partial result with little explanation doesn’t help me understand what is happening. And when my doc says my blood sugars are good, what she means is that an a1c of 5.9-6.5 is something she won’t argue about even though it’s abnormal. Such abysmal care! in these past three years with Covid and all my health issues I’m learning that there is no medical person who I can trust with the complete story of diabetes. I am lucky if I come across a video here and there like this one to help me understand a little more about my body and what is happening.
