This is very interesting to me because I was recently diagnosed with osteonecrosis of the medial femoral condyle. My doctor is recommending a partial replacement. I am 68 years old, and a very active marathon runner.

I have recently diagnosed as SONK in MRI , wondering how can I tell if it is fracture? The MRI 8 months ago in the same area shows perfectly normal.

Is there a way to differentiate subchondral fracture from osteochondral lesion of the femoral condyle? I had a case of a 30 y/o male with extensive full thickness cartilage loss, slight depression of cortex but a curvilinear half moon shape hypointense line touching the subchondral bone plate, no bone marrow edema, with meniscal radial tear.

with subchondral fractures, typically cartilage damage is not a major finding, if it is halfmoon shaped it sounds more like osteochondral lesion, if without edeme etc, potentially old OCD
