I actually appreciate these videos as a patient, even though I watch mainly for the ASMR component. Knowing why a doctor asks certain questions and what to expect from certain procedures is very useful. Also Dr. Gill is just a very knowledgeable and thoughtful doctor -- if only they all had the same rapport.

Yes, we provide a minor ailment service at our pharmacy but it has become more limited recently. I think we still offer olive oil drops for ear wax though. I'll have a look later.

I’m liking this blended approach, but I also really like the deep dives and the examination videos. I imagine different topics would lend themselves to different formats. I’m not a med student at all, but I’m the engineer type and love learning from an expert whose able to explain the subject matter very well such as yourself.

I’ve been looking forward to this! 🙌🏻 Had glue ear 30 years ago when I was 4 and had to have grommets put in and went to the specialist quite often, so this is great to have some inside knowledge so to speak! May I also recommend Audiology Associates on here, they do ear wax and skin removals from the ear but he also gives some really useful information about the inner ear etc and shorts as well 😊

Love this. So i work as a floornurse medicalward on a middel size hopsital. And on of my standard questions when i get report on a patient that they say have bad hearing is ”have you looked in the ear?” And manny times the answer is ”no” its just a accepted fact that old ppl have bad hearing. Just a week ago i got a report from the admiting docktor. -The patient have some troubel Taking direktions due to bad hearing. -Have you looked in the ears? -No the patient have bilateral hearingaid. -.... its common whit wax plugs due to hearing aid.. When the patient arrived to the floor and i checked the earcanals they Wher bouth filled whit Wax. Also if a patient have hearing aid and still hear bad. Dont forget to check the batteries.

Great video. I was following your instructions as I performed an assessment on my wife, and @4:22 when I removed her tympanic membrane to observe it, it caused her great discomfort. I also had difficulty putting the tympanic membrane back into her ear.

Had a hearing test a while ago struggling so much with hearing people. Passed with flying colours, nothing visible, beep test not a problem. Not even excess wax. Ten years later, it's finally diagnosed as auditory processing issues! Now I'm medically clear to say "Speak up sonny!"

I am not a medical student or expert in any way, just enjoy learning about stuff. I do like the regular exam videos as they show the whole process including the interaction with the patient, building and keeping rapport while performing the exam. On the other hand videos like this one are also great because I actually learn about the details on how things work on the inside. This one hit even closer to home because I've suffered through 2 months of recurring ear infection last year before christmas, took 6 weeks of increasingly strong and sophisticated antibiotics before it finally stopped, and yet i still occasionally feel some pressure inside the ear, even though a full ENT examination can no longer find any issues in there. So to get all this information presented in a very simple way that even a non-medical person can understand how it all works together is really great. Long story short, to answer your question in a lame way - please keep doing both of the video types, they both have their value :)

so i know exactly what it feels like to have your inner ear blocked and unable to equalize. roomate got a pair of ferrets without warning me at all, and didnt care for them properly. they created so much ammonia it blocked up my left canal and left me with a splitting headache for a solid month before a nurse at a local clinic figured it out. gods, the relief was amazing.

So doc, what happens if a patient comes to you with ear aches, and say they may have gotten it from headphone/air-pod usage. What changes would you observe in the ear? Would it be affected mainly in the canal, or would it affect the inner, more delicate parts of the ear?

Dr. Gill, I have had a persistent problem with my ears for at least 16 years and still do not know exactly what it is or how to fix it. Every time I mention it to a doctor they kind of just brush it off. I always feel like my ears are clogged/muffled and I have to do that thing where I move my jaw or blow air out of my nose to relieve the pressure. It may be worth mentioning that I frequently got ear infections as a child and had to get ear tubes (they were eventually removed). Also, in my teenage years, I got a sinus infection literally every 4-6 seeks. And I had a few very bad spells of vertigo in my late teens. Any ideas and/or advice? I would very much appreciate it. Love your videos!!

I have hyperacousis (environmental stress, not physical pain variety) and I developed the habit of not cleaning my ears because the wax buildup blocking the canal was the only relief I could get. I don't do it anymore now that I've moved out of the city and there isn't as much noise.

Hi Dr. Gill! I had tubes in my ears as a child. My allergist was very surprised to see scarring on my eardrums from these tubes. She said the scarring was white! How often do you see scarring on eardrums?

Quite often. Scaring isn’t really a problem, just indicated that the ear drum has had a hole and healed

At the start of the video it is mentioned that ear problems are a large peoportion of a GP's work load. Is there any explanation behind that? Why are ear problems such a large part of the problems that you deal with as a GP?

Honestly not sure. Perhaps as a classic ear infection comes on quite quickly and is rather painful. Meaning people seek care sooner

In my country yóu can get a physical once a year from your doctor. They usually check blood, urine, blood pressure, heart rate, and listen to your heart and lungs. When you get older they also do things like prostate examination and a colonoscopy. For anything additional you either have to ask or have a specific issue with. Do you have any suggestions for which other things I should ask or do the things I have listed already cover the most important things? Thanks.

The safest way to help clear the eustachian tubes, saline nasal spray. I used this to help clear mine after cortisone shots to the inner ear to treat sshl and it still works for allergies. 😎👍

Amazing timing for you to post this! For only the second time in my life (first being when I was 8) I had a bad ear problem (something in my ear sitting right on my ear drum causing a lot of pain) and I had to go to the dr and An ent to get help for it! It’s nice to see the exam procedure explained here :)

@@DrJamesGill they originally thought it was a bug in my ear, to which I subsequently freaked out and told them to just take off the whole ear if that was the case. It wasn't though, it was actually a piece of debris the blew into my ear from a fan that I had on next to me. Who knew that could even happen, but here we are!

With Otoscopy, I think my biggest fear is of damaging the patient's eardrum by mistake so I never really do it properly out of fear... Is there any chance we can perforate the patient's eardrum if we go too deep with the Otoscope? (In a normal ear exam as opposed to say otitis media when the eardrum may be bulging)

RN here: as an oncology nurse I did not do this examination. But I've needed to do be able to do this in my current practice (not oncology 🫤 which is my love due to patients). This video was so helpful in showing me the anatomy I need to study up on, the physiology I need to review, and the observations I need to report up to the physician. Just wondering, how much time would this normally take? Patients should not be expected to undergo this examination for long as in many cases they won't tolerate it beyond a certain time limit. Also, if the patient is lying down, is that a good position for the exam or do they need to sit up?

I have the totally useless ability to contract(?) my middle ear at will. Hear, that weird lousy "sound" you can perceive when yawning ? Well I can produce it at will. The only useful side it has is that I don't need to yawn or pinch my nose and blow to equilibrate my ear pressure. I just do that.

I have dizziness, tinnitus and hearing loss... the doctor said it's bppd... I tried many treatments with different specialists in the Netherlands for 14 years... none of them worked for me...So I gave up on doctors and decided to live...

My gf gets frequent middle ear infections... so much so she has a standing prescription for antibacterial drops. I'm going yo show her this so we could understand more about the issues
