Many have asked about the benefits of Vortioxetine for generalized anxiety disorder. A recently meta analysis concluded that it had the lowest effect size 0.12 (measure of efficacy) of all the medications reviewed. This further supports what I already said in the video that it does not work to treat anxiety disorders. You would be far better off using duloxetine as the SNRIs had an effect size of 0.36 if there is depression with combed anxiety. For those interested in natural products Silexan a lavender extract had a large effect size of 0.86. Hope this helps

My psychiatrist is prescribing me this for PTSD with depression due to sexual side effects (genital numbing) I had with citalopram 20 years ago that never fully went away. I am still nervous because of my already limited sexual function and not sure this is worth the risk. I asked about Buspar instead but was told no. Wellbutrin ramped up anxiety and was just not effective. Is there any other drug I could ask about? Without a doubt I need to do something since I've almost become a shut in.

@@ShrinksInSneakers sadly 3 doses of trintellix 5mg and the whole region is numb. So numb I am leaking urine because I can't feel when I need to go. As a nurse, I am curious if there is something genetic that makes certain people ultra sensitive to this. She said every other option will likely have the same effects. She would like me to continue to see if the effects subside in time or if the benefits to my mental health are worth sacrificing sex. Hard choice considering my spouse also has a stake in this. Just wanted to let you know, I remain the outlier.

I have TRD, severe enough to become delusional, no executive functioning, cognitive issues, catatonia, and other stuff. Been on 10mg of this drug for 2 days, bumped up from 5mg for 2 weeks. The nausea and dizziness were hard to tolerate the first week, along with some headaches. Stomach pains were common, and I forgot to eat with my dose once, and it was unpleasant. My energy and mood had some improvements. The nausea is mostly gone now, I haven't needed a nap in 2 days, I'm cooking healthy meals (and doing the dishes!), showered at least every other day, and felt well enough to go out with friends - and enjoyed myself! This has been a miracle drug for me so far. I was looking into deep brain stimulation, TCMS, and ECT after 26 years severely depressed.

Triltellix works so well for me. I take 20 mgs daily for the last 7 months and my MDD is so much better. I swear by it for my treatment. In addition, there was zero weight gain.

This medication has really helped me a lot. I have adhd and take medication for it, but life circumstances caused some depression that really showed up as terrible cognitive disfunction. Figured out it was depression because my adhd meds were not working like they always have… I had no motivation to do anything at all and the brain fog was so heavy. Started taking this and I’ve been able to get things done and started doing things I enjoy again. The price is terrible though and my insurance won’t cover it, so I might have to stop taking it soon. $500 for a 1month supply is just not doable for me and my family.

I went through the same the thing exactly. adhd and meds for it but then after while lost motivation brain fog. Doctor suggested Trintellix but wasn't sure about it. but from your comment it seems working for you. can i ask if its stimulating?

@@MoBadawyy I would not consider this medication stimulating. It's important to remember the cognitive enhancing effects are in the context of depression not ADHD or other disorders. Hope this helps

Oh my gosh see this was why I tried this medication to begin with. My psychiatrist and I thought it was SUPPOSED to be weight neutral. 🤨

I want to say that this is the only drug that did not cause me negative effects on sexual activity. The only negative effect on me is a slight lightheadedness, as if I drank a jug of beer. But it doesn't seem to affect my mental capacity and memory.

I tried vortioxetine during a few months. It slightly improved my mood, did nothing for my anxiety. Almost no sexual dysfunction. I had to stop because it made me restless and I felt exhausted after a few weeks of bad sleep. My problem is more anxiety than full-blown depression, so now I started sertraline (Zoloft) and I already know it's a better choice for me despite some side effects.

i took it for three months and it was killing me. intense vivid dreams! i couldnt sleep for more than a few hours, and would wake up with a rapid heart beat. also vomiting every morning. had to stop.

Trintellix is the only med I have ever found to help with my depression. Multiple SSRIs and SNRIs did absolutely nothing for me, Abilify made me want to kill myself, and landed me in the hospital. TMS was being suggested, but it wasn’t covered by my insurance. I finally saw a new psychiatrist who put me on Trintellix, and I had that experience I heard from so many people who tried to convince me to take antidepressants for years. My depression lifted and I didn’t feel like I was swimming uphill all the time. I had quite a few years of remission, but have been cycling through mood episodes again for almost 3 years now and the doctors I have seen won’t change my meds because they don’t know what to try. It’s so frustrating.

I appreciate that you mentioned that people can be statistical outliers insofar as to the efficacy of medication. I've heard many a psychiatrist say quite bluntly for example that antidepressants even with mood stabilizers of various kinds are ineffective in bipolar disorder without exemption. Not to get much further into it if you have a remission of your depressive episode it's because of a placebo effect or the natural course of the disorder. I understand statistical significance but at least you bring in your clinical experience to give a more complete phenomenology of the matter. Very refreshing. 👍

I’ve been on multiple antidepressants over the past 30 years. All of these medications have a black box warning that they can possibly cause suicidal thoughts. I’ve never experienced that side effect until I tried Trintellix. After about 2 weeks on the medication all I could think about was the desire to die, how I might do so, and I felt absolutely miserable. I stopped the medication and resumed my prior medication. It took about a week for the Trintellix to leave my system. I have no doubt that had I continued the medication my thoughts of suicide would have consumed me. Glad it works for some but this particular medication would have killed me.

Could I hit you up with a video request? Like apparently not a small number of patients, I currently take both an SSRI (for anxiety / depression) and a tricyclic (functional dyspepsia). I'm aware that mixing seretogenic meds can be risky in theory but.. I know that it can be done. I've never really understood how you guys navigate this gray area of avoiding overly risky combos that could lead to seretonin toxicity. Would be a very interesting topic to cover, IMO!

I covered this recently in a video talking about serotonin syndrome. There are safe combinations and unsafe combinations. Before I make a choice to combine medications I'm asking a lot of questions one being do I need to combine these meds and does the research tell me it will be more effective, sometimes less is more. Hi I'm not sure if your still commenting but does this work well with anti psychotics like clozipine, I've tried many anti depressants for my severe anxiety and depression but none seem to work . I'm paranoid schizophrenic with autism and ADHD I already take pregablin I've tried propranolol recently it didn't work. I was also wondering when vortioxetine is combined with pregablin should I eventually get relief from anxiety and depression?

My father was prescribed this about 2 months back for his depression and GAD . After a month of usage I started to see behavioural changes gradually leading upto hypo mania. Seeking help from whoever has had a similar experience and how they resolved it. I’m hoping the creators of this video will respond as it’s been a very distressing time for us as a family.

It's probably because the diagnosis is incorrect. When someone develops mania on an antidepressant it's usually because they are in the bipolar spectrum of illness and would be better off using a dopamine blocking medication or second messenger medication. @@mangkokhayon5039 I think manic symptoms always matter and in cases where overt manic symptoms have been identified its best to avoid antidepressant medications. This is based on my research and does not mean I would never recommend antidepressant but since we are speaking in general for most patients they are best avoided. Hope this helps.

I have been on vortioxetine for 4 years after being on one SSRI or another back to back for 18 years previously (mostly sertraline and fluoxetine). I also had a nast 6 months or so on venlafaxine. In my experience, vortioxetine differs to the SSRIs in that it feels "clean". I don't know how exactly to describe it- I have functioned very well on both sertraline and fluoxetine but there was always a small sense of being drugged. Sertraline particularly made me dull but cushioned. Vortioxetine does not seem to make you a bit dead inside like SSRIs. However, this may actually be a problem for some people. Since being on vortioxetine I have experienced overwhelming rage and other emotions too have been enhanced. I hadn't experienced anger, more than annoyance, since I was a teenager, which coincided with the time I had been on SSRIs. It is possible that vortioxetine, for some unknown reason, actually causes anger but it seems quite likely that it, instead, doesn't blunt emotions like SSRIs do so anger that was supressed by SSRIs for so long is now coming out. I'd be interested to know if anyone else has had a similar experience or what it is about the pharmacology that might cause anger or not shut it down in the way SSRIs do. Edited to add: the only other side effects I have noticed were intense itching in the first few weeks and an inability to tolerate alcohol. I could drink plenty on SSRIs but 1 glass will give me severe anxiety the next day I had only previously experienced after drinking really excessive amounts. Again, it would be interesting to know why it has this effect when SSRIs do not.

irritability can be a sign of serotonin syndrome. If you're taking 20mg maybe you can talk to your provider about backing down to 10mg to see if the medication is still effective without the severity of side effects.

@@adamhumphrey4084 I have been on 10mg. I spoke to psychiatrist about this a few weeks ago and he says anger is not a side effect and he believes I am just feeling emotions that I am not used to due to them being dampened by SSRIs. He has recommended increasing to 20mg to see if this lessens the anger and improves more positive emotions. I am titrating slowly as medication changes tend to bring on panic attacks. I'll update in a few months whether it works.

This med does inhibit GABA. What does it mean in practise? I thought everyone wants their GABA stuff to work well for its calming, relaxational effects. Is there something Im missing here?

Hey. I agre with what you have said regarding anti anxiety. I have tried so many ssri's in the past but just can't find one that calms my anxiety. duloxetine just knocked me out. I told my doctor, it's the terrible anxiety feeling that's cousing my depression. Iff not for the anxiety I wouldn't be so depressed! So he puts me on a medication that is not designed for anxiety. can someone please offer me some advice. I don't have the space to explain just how bad my anxiety is ruining my life.

Interestingly, SSRIs like escitalopram still relieve depression (and are no less effective) in rats completely lacking serotonin transporters. SERT is actually lower in depression and tends to rise when depression is relieved. Research seems to point to the modulation of the trk-b receptor as being much more relevant to antidepressant action. I think we're going to find in a few years that the serotonin re uptake inhibition is actually responsible for side effects, not the therapeutic effect.

It's a brand name and not offered as a generic medication which drives up the cost. When they formulate a new medication it starts out with a patent that lasts for a number of years once that has expired it will be offered in generic versions. I do not like the cost of these medications either and believe they should be more affordable and accessible to people who need them. Hope this helps

This is a good one, my short answer is I don’t think there is a big difference between these serotonin reuptake partial agonists medications and SSRIs or SNRIs but I will cover the medication in a full video. There is a slightly better side effect profile included reduced risk of sexual side effects

I am Taiwanese. I once complained to my doctor about my poor cognitive function. I have poor memory. I have poor concentration. My doctor put me on Vortioxetine. But after I took it, I felt it was ineffective. I think I have a long-term mental disorder causing cognitive decline. For example, I can't go to the bank or the post office or other places where I have to do complicated things. Because I can't handle the complicated stuff of communicating with people on my own. So my situation is very serious. But strangely my IQ score was 104. It was a test in a hospital. My psychiatrist seems to think my cognitive decline is due to depression, but I don't think so. I was suspected of schizophrenia early on. But many years have passed without a diagnosis. Doctors now think I'm not schizophrenic. So there is no answer to my situation in psychiatry. I'm confused. Don't know what to do. Taking antidepressants is not taking antipsychotics nor is it.

Unfortunately you have fell trap to the system we’re all they wanna do is push drugs my advice to you is yoga meditation get your mind right and start again you are what seems to be high functioning otherwise take the time and meditate you can do this, you need to do what you need to do to allow your body to release serotonin the feel good do something that makes you happy or that used to make you happy get an adrenaline rush you have to alter your body chemically yourself medication will never fix it it will just compound your problem and keep you hooked to the system forever you got this you can do it

What do you think about taking vortioxetine and pregabalin simultaneously? I got it prescribed, but since reading about both I am not sure if it is a good idea and whether I should go see another specialist. I also read that pregabaline is highly addictive. I have been treating depression for the past 5 years (23 y.o.), have a little bit of anxiety and maybe some borderline/avoidant behaviours. I have been on venlafaxine recently.

The answer will depend on a number of factors and I cannot provide any medical advice as I'm not your doctor and these comments are intended for educational purposes. I would always say diet, exercise, and psychotherapy are good starting points for any depression treatment. I would not say pregablin is highly addictive but there can be some risk for abuse and if its being used to treat anxiety it's off label and has limited data although it has better data than gabapentin which is often prescribed for anxiety (see my video on gabapentin it will help clear things up). Vortixetine is a fine medication for the treatment of depression is it better than other antidepressant medications it does not seem that way from the research. It's also clear that it's not effective for the treatment of anxiety disorders. The combination may be helpful but I usually start with a single medication and assess for improvements prior to giving multiple medications in my practice. Interesting about GAD. Anyone have anecdotal experience of using it for that? Sounds like such a promising drug but so many depressives have comorbid anxiety so... that seems like quite a drawback.

I would say some people will tell you it works for anxiety, this is where many people do not understand who the research is interpreted. The research studies will tell me about generalities, so in general this medication was not helpful in large scale randomized controlled trials. On a case by case basis in individual patients it may be a great medication thats what makes this job complex we have to apply generalities to individual patients

I have taken it for 4 years and my diagnosis is recurrent depressive disorder and GAD with panic attacks. I'd definitely say anxiety and panic attacka have always been more of a problem than depression. I had been on SSRIs for 18 years previously and I'd say it works just as well/ no better or worse than them but overall has less side effects.

Does vortioxetine increase glutamate like Esketamine? Esketamine is what worked for me, so I'm hoping vortioxetine does the same so I can switch for instead of Escitalopram and Bupropion

I take nac ( n-acetyl cyseteine) with my brintellix. Read about it. 😊 i have had no issues with my medication, no nausea, nothing .

Currently on this for MDD since I’ve tried so many, this medicine is so expensive compared to the other anti depressants I’ve taken. 😔

No thiVortioxetine is the only antidepressant that improves cognitive functions, by acting like some older "dirty drugs" on many signaling substances and their receptors. The reason that patients feel clearer in their thinking is because vortioxetine increases the level of histamine, among other things. The impact of its positive effect on cognitive functions should certainly not be underestimated. Vortioxetine does not have a better antidepressant effect than regular SSRIs, but with the ability to make patients think more clearly, get better memory and counteract apathy and anhedonia, means that it is generally better in the end than usual SSRIs/SNRIs/TCAs. In addition, a big plus for the lack of annoying side effects. Of course, it will never help ALL patients with depression and/or anxiety, but many may benefit from it if they have already tried other antidepressants without good effect. The biggest disadvantage is that it is still very expensive.s video don't tell the truth. It is

My psychiatrist prescribed this to me for anxiety today, but I think he’s just run out of ideas to be honest. Its my situation and wiring that has put me in a state of feeling shit all the time, and judging by what others said, seems like an ineffective drug. Im also really sensitive to drugs in general (vomit from most of them) and have sleep disorders so this drug just sounds horrible.

Started 1 week ago. Was on Effexor for about 2 years. Anxiety is through the roof since beginning. No focus on daily life as I’m focused on the intense anxiety.

It’s not great for anxiety although some have supported this idea. Anxiety is one of the more difficult things to treat in my opinion and for some reason it’s viewed as less severe than depression

@@ShrinksInSneakers I am also undergoing TMS therapy with it. I had just finished ketamine sessions as well. I think maybe it was too much at once. I felt like my chest was going to explode. It’s slowly calmed down. I’ve dealt with major depression since childhood. Ready for some relief.

The sexual side effects are lower with vilazodone and I would say they are about the same in terms of risk for both medications (lower but no completely out of the question). They are highly effective. I good general rule of thumb is older medications are more effective but newer medications have less side effects. I’ve had some mentors in my career who believe MAOIs are the best overall medication but I’ve not seen any head to head trials to prove it.

I have been taking vortioxetine 1 month now. I have reached the dose of 20 mg starting with 5 mg. Although i have less anxiety i have noticed less connection to my emotions and all senses overall, is this a side effect? It feels like emptiness and numbness....does it get better as the time goes by?

I am only taking 10mg, and have done for 4 years, but I have the opposite problem. After being numbed by sertraline and fluoxetine for the previous 18 years, vortioxetine has brought my emotions back, particularly anger, unfortunately. Perhaps you could try going back down to 10mg and see how you get on?

@@calvin3446 it has helped a bit yeah, only just a month in but it is easier to get up in the mornings and haven't been as down. Feels like there is something just underneath the surface though. Better but not 100%, more like 80%

I take mine first thing in the morning with an anti nausea tablet as well and have been fine. They say the nausea wears off *fingers crossed*. Really hope this works for us. Have tried so many different other options, just really need this to work :( have so much to do

I'm not a fan of antidepressant medications in people diagnosed with bipolar disorder, they are often destabalizing, so I usually reserve antidepressant for treatment resistant cases where other options have ben tried. There is no evidence that this medication works to treat OCD or bipolar II. Hope this helps

It's just another SSRI drug that will cost 10 times as much as a generic off-patent drug. There's been no real progress in antidepressants in over 30 years and a 2018 study showed Amitriptyline, one of the very old antidepressants and also very cheap is the most effective.

Dosen't work...less sides effect? yes..but if you dont have energy or positive mood is a waste of time. No energy, cognitive is not what they sell...and the rest frustration that get everything worse cos time pass and nothing good happens...20 mg per 6 months......

It was my first SSRI i got prescribed I felt it for my Depression butt didnt help for anxiety.Escitalopram worked better for anxiety.Didnt feel any cognitive changes on brintelix and i had zero side effects on this drug,not like Lexapro sexual,weight.

See my response to your other question, I put the link to the study. I would agree my clinical experience indicates lexapro is a better option if anxiety is the primary target, and the research backs up the lack of efficacy for Vortioxetine in GAD.
