There are some excluded hospitals which are not full filling the criteria of the insurer that’s why cashless and reimbursement is not possible in these hospitals and also I want to inform you these are as per IRDAI guidelines.

It's not about claim against smoking, drinking...if one will not mention the same then insurance company can reject claim for other diseases citing hiding the truth. Hence, it is recommended to disclose the same .
