constipation definitely can make pain worse and depression worse. Absolutely sure because an enema = boost in mood, lessened pain not just water intake, but poor eating.

I'm curious how her vids had helped you so far? What technique against depression have you learn from her? All I see is book knowledge that she makes it available to anyone on youtube, but nobody that comes here is interested in becoming a doctor.

Have you tried gentle self-massage on your stomach area and gentle massage on your back muscles? Might help with your nausea and muscular pain.

Dr. Marks, your video on the bidirectional link between depression and chronic pain (I suffer from both) helped me to understand more of what I experience daily. As a veteran and corporate IT professional, I ignored earlier signs because making money and providing for my family was paramount. Today, I have more time and reminders that chronic pain is real and isn't easy to deal with. However, thank you and I appreciate your clear and coherent explanations of these debilitating issues.

This makes so much sense. I've always known that when I get a depressive episode I suffer more from back pain. Luckily with DBT skills and mindfulness, and some of the medications you mentioned I'm doing so much better! Thanks so much for this video, Dr Tracey !! 💖

So you rather have your brain depending on drugs to defeat depression than learning how to defeat it without drugs? And then what 4 or 5 months later increase the dosage because your tolerance increased. Why would you be satisfied depending on drugs rather than motivating yourself to find someone who can help you defeat the issue without it? That's like" I need to stop smoking because it makes my clothes smell so I'll just buy a nicotine patch....no more smell." That won't solve nothing but put more money in therapist pockets. While still depended on drugs that money goes to them.

Oh yeah for sure, when I'm depressed, everything hurts way more. Especially if it's a humid day. Humidity kills my joints. My muscles also feel so heavy when I try to move, it hurts. Oh and the headaches, the headaaaaaaches.

@@JadeDRail i can call a taxi but i will be sedated for the procedure. Plus it's out of town so instead of paying 40 dollars to go and return, it would be over 80 dollars an i need an adult to accompany me.

I've had chronic pain from severe ankylosing spondylitis for 28 years and learned that I own my pain. I'm not sure why but that really helped me and I'm naturally a happy person with a high tolerance for pain . A paper cut can be worse than my pain for a person who has a low tolerance for pain . We all own our pain and cannot really compared each others pain. I feel very lucky to be happy ...

I've had chronic eye pains for 18 years and always wondered why there are days when it feels extremely exhausting, and as if I can't do anything right or think straight due to pain, and yet other days where it almost feels as if the physical pain is at the same level but the pain acts almost... motivational and I get tons of things done (though sometimes with small errors so the cognitive impairment at the time of pain spikes might be the same whether or not I feel motivated or not). I've had the brain stuff explained to me once by my doctor, but don't think I got it back then. Your video explained it in a way I could understand, so thank you for that.

I've entered a serotonin syndrom from trying duloxetin while already being on SSRI medication.I have chronic migraines that have only gotten worse with years. I've had GAD for a long time, but when pain became worse than ever, I suddenly developed very serious depression. I am medicated and it saved my life, but pain was definitely what caused depression, not depression and anxiety causing pain, as much as doctors tried to convince me for years now

Thank you. I am living with chronic pain and decreases mobility in one of my hips. At the beginning there have been a muscle tension that never loosened up. No Therapie i tryed helped. Now the joint is in an arthrosis state. The muscles and tendons pressed it together for over a decade. I feel like handed down from one doctor to another. Now i get meds that prevent the reuse of serotonin. I hope they will help. I'm age 37. I love nature and walks and i hope one day i can hike again. Or at least do longer walks. Your videos help to understand my way while i go.

Bless you, Dr. Marks!! Due to Failed Back Syndrome and 9 years of unrelenting pain, I know I am in a pain/depression loop. My surgeon is suggesting a pain pump implantation at this point. I am digging deep to understand how my brain works and if I can unravel the years of the same reaction to my pain in order to help myself before I make such a drastic decision. You are such a large part of the learning curve for me...warmest regards

Is it really FBSS or arachnoiditis? You may want to look into arachnoiditis as a lot of doctors don’t want to diagnose it or don’t know what to look for

Remember that soldiers injured in war can have significantly diminished pain when they realize that this injury will send them off the battlefield and they will live to go home to their loved ones. Seen during all wars in which there was a good chance you would live to be shipped home. They weren't nuts; their pain was reduced by the meaning pain had for them.

I suffer from chronic pain have for years I’m always down. I have trapped nerves and multiple slipped discs and it’s been getting worse. But because no one sees the pain no one understands everyday is hell only reason I’ve not ended it is because of my daughter. She’s the only thing that keeps me going she makes me push through the pain and push through the tears.

What depression does to your brain? An enlarged and hyperactive amygdala, along with abnormal activity in other parts of the brain, can result in disturbances in sleep and activity patterns. It can also cause the body to release irregular amounts of hormones and other chemicals in the body, leading to further complications.

I already take 3 meds for epilepsy that are typically used for depression, bipolar disorder and anxiety: Keppra, Lamotrigine, and Trazodone. I think they negatively affect the part of my brain that feels depressed, while positively affecting the epilepsy part. It’s weird and not fun.

I have fibromyalgia/chronic fatigue syndrome (contracted chickenpox when I was 20, during Nursing finals); I developed depression when I was 12. winter is a season of hell for me and some days I am incapacitated, those worst of which is cognitive decline. NSADS are my go to for managing inflammation, along with things like ginger to further reduce inflammation.

I also suffer of Fibromyalgia, chronic pain, chronic fatigue, depression, insomnia and gastric troubles are all part of its symptomatic frame...Hang one, don't dispare. fortunately since diagnosed in 2008, Fibromyalgia only manifest by periods of crisis, with good treatment and follow up, I always manage to overcome the crisis and carry a reasonable normal life.

@@sabina_coeth9662 I'm so sorry you understand. : ( I was prescribed Mobic, modified diet more (no eggs, dairy, nightshades, nuts; already didn't do grains) both of which has decreased the inflammation. I'm not always 100% with the nutritional changes, but I don't hurt the way I used to. but it's still summer. tonic water with quinine is also a lifesaver for me. I had severe stress for 10 yrs and even after a year, I'm still wiped out. but not as bad as I was. thank you for your response. many blessings to you; may you enjoy and happiness and health!

Thanks for sharing this video. I have been diagnosed with chronic pain, stemming from fibromyalgia (but I just think they gave it that label because it was handy. I think it could be a made-up illness to describe or capture the pain we have that they can't attribute to anything else.) I also have been diagnosed with TRD and MDD. And nothing has helped. I'm pretty sure the pain came first, and the severe depression after that. Although a psychiatrist I saw for a short time (I don't believe in therapy. I've analyzed myself for decades.) determined I had been depressed probably since childhood when my parents divorced. He wasn't wrong. The problem is, I haven't found any meds that help. I've been in chronic pain that has impacted my life severely for almost 40 years. It has cost me friends, jobs, and relationships. Sorry for the long post, but just wanted to say that it's about time someone recognized the relationship between pain & depression. I'm glad there's a scientific explanation for it. I will share with family, because they don't believe me.

This video and anyone who comments that they are the same, would I hope help you to feel less alone. It can make SUCH a difference, not necessarily to the pain or initial conditions themselves, but to feeling heard, respected, validated, understood, cared for, and believed. 🥰

There don't believe me when I say it's mental and Physical. As I got older it's more physical pain that makes me not do much. The only time I was able to do things and move around feeling good lots of energy was when I had a bad cold and was on prednisone for my asthma. Wow I was not depressed, I was able to finish things, did not sleep as much. I found a post and so many people experienced the same thing. I believe they did research and said 7.5 daily is safe. My Dr is against it. I was so sad 😭 when I heard that. Hope this helps someone.

@@mbecky7414 So interesting that you say that! I was prescribed prednisone this summer when I had an allergic reaction to a wasp sting. I couldn't believe how much better I felt! I asked my doctor for a refill to take prednisone instead of Meloxicam for my osteoarthritis. She's against it & says I'll develop osteoporosis. But I already have that. The way I see it is this: I'm almost 69, I don't want to live past 80, being crippled by pain, so give me what I need to get the best out of my last few years. I know the risks & it's my choice.

Dr. Marks, please make a video on anxiety and exhaustion. My anxiety level is constant and usually very high. I feel exhausted most of the time and have a hard time getting out of bed in the morning. I take many days off to just lay in bed all day.

I'm similar. I think it's a combination of using up so much mental, emotional and physical energy on our anxiety that we end up exhausted, plus possibly not getting good quality sleep so feeling extra sleepy or fatigued during the day, plus feeling a relief from being asleep...because it's the only time we get a reprieve from the anxiety...plus (for me anyway), I always have a feeling of impending doom, so sometimes even the thought of doing anything just makes me feel physically sick with stress and fear, so I find that staying in bed is like hiding or withdrawing from that fear, and lastly, chronic anxiety for me has led to chronic depression, which tends to make it harder to get out of bed...especially when I don't have anything I want to do thanks to being too anxious to even try to enjoy anything. Ugh!

Whenever I m depressed I get anxiety which in turn results in body pain, I feel heat... Also stomach issues.. And stomach issues I get probably due to fear of getting them... How to deal all this... I can't do normal things like before!

I have a friend who has not been doing very well. About 2 years ago she started to self-harm. Less than a year ago she decided to tell her parents and they didn't respond very well. They haven't given her the help she needs. I just wanted to know if self-harm is always an indication of a mental illness and also what I can do when she finds the need to hurt herself.

I have MDD and GAD and I mutilate myself frequently. The problem is that I like it, it calms me down when I'm having a panic attack or something. I read somewhere that when someone cuts themselves, the brain produces endorphins / dopamine to relieve pain and cause a good feeling in the body. So what exactly is this relationship between self-mutilation and endorphins / dopamine?

I’ve been in chronic pain for many years. I used to live to ride my beloved horses, had many hobbies, my children, but now, they’re all gone and I’m unable to get them back. I’m totally alone and moved to a place I never wanted to be for my daughter so she could pursue her education on my dime. She proceeded to clean out my bank account after we moved here, she started a nowhere relationship with a dirty cop who hates me fo some reason. He stole my Serval cat while I was in the hospital. I’m 68 years old and I’m at the end of my rope. I can’t wait for this to all be over. I no longer trust in doctors, (all they do is prescribe an array of medication that makes me worse), and I’m stuck in my head with really bad thoughts. I’m planning on ending this suffering because I can’t take more. I don’t have any advice for those who are depressed. I guess it just happens. I’m sorry.

The broken back caused insomnia. Insomnia caused a very deep depression. I can't work off my natural energy so i was awake for a year. My brain isn't designed for this. Now, I'm broke and insane. All I wanted to to finally go to college. Another lost hope. It all adds up. Yet, I'm crazy? No, I'm a fighter but I'm worn the F out. Where's the help? Don't matter if you are correct. My body is failing more and more each year now. Where's the help?

I'm sorry for what you're going through. Did the drs recommend surgery to try and fix the back? Have u seen a psychiatrist for the insomnia? If u start sleep aids please keep a notebook with the drug name, miligrams, side effects, and the dates u used it. Because if u switch to a different dr later they will start u from the beginning all over again. I hope u find peace and are able to sleep. Without sleep, the body begins to wear out.

Ive tried hypnosis and can't be hypnotized. I never relax. I think the only time i was truly relaxed was when i prayed for God to hold me. But I've tried different types of hypnosis to ease the anxiety and it just didn't work.

@@JadeDRail thanks. I have tried hypnosis with different people just to make sure, but NO dice. Lol somehow my mind has put a double padlock and is blocking some serious stuff because i can remember my dad beating my mom and i can recall being locked in a coat closet in the dark , being molested by a family member and yet my mind is locked harder than fortknox.. 😶

Hi Tracey, Thank you for this video. I was on antidepressant for a while. After watching video, I got the doubt that pain in my anus might be causing the depression. I went to hospital and confirmed that I had anal fissure. Right after getting the treatment, the depression disappeared within few weeks. Now I'm completely free from antidepressant medication. I'm feeling a lot better now without any antidepressant. Your videos are indirectly saving lives across the globe. Once again a big Thank You from India.

I'm in this cycle of chronic pain and depression, I have back pain and my doctor tells me that my injury shouldn't cause that much pain (like he knows 🙄) I think I had depression then the pain got worse. I changed my psychiatric last year and to my surprise he diagnosed me with ADHD 😳 but honestly that the best thing happened to me it explains everything, my only regret is I wish I changed my psychiatric sooner than later i could avoid to many problems☹

I have read that sodium channel blockade is part of the reason anti-depressants, especially the tricyclic antidepressants, are effective against pain. Sodium channel blockade is also the mechanism of action of novocaine and benzocaine anesthetics.

Dr. Tracey, I didn't ask the question but I REALLY appreciate this video! I will be 60 soon and I have a significant trauma history from childhood on. In addition, (and I've had two professionals tell me it's related to the trauma) I struggle with morbid obesity, untreated sleep apnea, hyperparathyroidism, fibromyalgia, PTSD, depression, and most recently, diabetes. My primary care nurse practitioner diagnosed the fibro about 8 years ago but i have a hard time believing it. I have NEVER had pain like this! I hurt 24/7, am on venlafaxine which isn't doing much of anything. Can fibro cause so much pain it affects mobility? I feel pain in my lower back and upper buttocks, both hips, my knees, my shoulders and neck and sometimes between the shoulder blades. I have been in physical therapy for 6 months. Any ideas or suggestions? This is REALLY getting to me emotionally! Thanks so much! Oh, I AM in therapy twice monthly by phone

This is perfect timing. I have mdd and chronic pain. I have a pain management dr due to the pain. It has gotten stronger as years go by. My pain dr has helped me try to function but the pain is severe. What's the difference between chronic pain and fibromyalgia? I have fibromyalgia but call it chronic pain because it doesn't stop. Can u tell me the difference? P.s. you are an excellent speaker and teacher!! Thanks for this video

I've been in chronic pain since July 2019 due to two herniated disc. I feel like I am going crazy. Sleep is terrible, I suck at work. I don't know how much longer I can keep it together.

Dr. any insite on why xyrem (ghb) when used for sleep is reported to be extremely effective for pain. In fact many Drs used it off lable for treatment fibromyalgia /CFS. Too bad this drug is highly controlled and prohibitively expensive

Hmmm, I've always very, very tolerant to pain. But when depressed I don't feel more pain in the same situations, but I get very irritated with the pain when depressed.

Hi Dr Tracey Marks. I'd like to know what is the best approach when you have treatment resistant depression and other comorbidities. I'm getting treated for low thyroid and cortisol levels, and shifted antidepressants for the fourth time and it still hasn't helped me to get better, though I tried psychotherapy and self care. What should I try next?

There isn’t one thing to recommend. That answer really requires a deeper dive into your history and comorbid issues. I’m sorry you’ve had a hard time getting something to work for you.

This makes sense, i get back pain and headaches when i'm in an episode. I struggle to define the difference between being depressed or if i'm sick. By the time i go to a doctor, whatever the illness/bacteria is, is much worse which i could have avoided if i went early. Do you have any advice on defining whether i'm sick or depressed? Hey, Dr. Marks. A little over 2y ago I was diagnosed with bipolar 2 disorder after a long depressive episode + SSRI's = hypomania. Since then I've been on Lithium + Lamictal and life's been great. I've had 2 depressive episodes post my diagnosis but no hypomania whatsoever. Can someone have bipolar 2 with just one antidepressant-induced hypomanic episode?

In my psychology class today it was mentioned by my professor that, in the states, sadness (not depression or prolonged sadness) may be considered to be added to the dsm (or some other diagnostic manual for psychiatrists...?) I find it extremely hard to believe and a very disturbing IF that were true.. What are your thoughts on this? Is it something that could be done? What may be the consequences of that?

Since i have the chemical imbalance in my brain would the covid vaccine give a big impact in my body? I feel dizzy mostly everyday since i stop taking anti depressant. I am scared to take the vaccine that it may make me more dizzy and anxious.

Hi Doctor good video. I am tapering off benzodiazepine. The withdrawal symptoms make feel depressed along with other symptoms. Can I take an antidepressant while I transition from benzos. I take lorazepam and it's scarce where I am. Cannot properly do the tampering. Thanks so much for your help.

Most people who have chronic pain especially somatoform pain disorders have alexythmia because pain impairs the brain biologically to feel emotions(emotional blindness and not emotional numbness).This means most people who are in chronic pain can't feel emotions , hence they can't even feel depressed since they feel neutral all the time.and still cbt is used for treating pain disorders for people who are not even familiar with how stress feels like.quite an irony.thoughts?

Thanks Navjot for this question. A therapist should be able to recognize the alexithymia. Part of the treatment could include helping the person connect with their emotions and help them name the emotions. 6:32 - You listed only one tricyclic antidepressant and it was NOT one of the secondary amines, it's a tertiary amine (amitriptyline, Elavil). The secondary amine TCAs, for instance desipramine and nortriptyline, have a stronger relative preference for NET, the norepinephrine transporter, relative to SERT, the serotonin transporter. As a result, the secondary amine TCAs cause more norepinephrine reuptake inhibition and raise NE levels more. Increased levels of NE seem more directly related to pain relief, at least based on studies using intrathecal drug administration. Of note, nortriptyline is a major active metabolite of amitriptyline. Desipramine is a major active metabolite of imipramine (Tofranil), which was the first TCA to be marketed. Amitriptyline has gotten a lot of attention and remains familiar to doctors possibly because it is often used for migraine prophylaxis, whereas most other TCAs are second- or third-line for everything, due to SSRIs being generally awesome. SSRIs are safer in overdose, they aren't anticholinergics (which also cause constipation), they have more predictable side effects, they are often less sedating, and they are less likely to cause cardiac arrhythmia - at least in general. But desipramine and nortriptyline deserve attention. Also, you didn't list MAOIs, but the lack of opioids is really far more heinous. You also didn't list gabapentin or pregabalin, those help neuropathic pain and have some mood effects. Ketamine, an NMDA antagonist, is most often considered a general anesthetic, but low doses are as effective as morphine for pre-hospital pain relief (given in the ambulance), and ketamine is also attracting attention as an antidepressant with very strange kinetics and dose schedule. We could also talk about memantine, another NMDA antagonist approved only for Alzheimer's disease (unless I'm mistaken), but it has interesting mood effects - I suspect it dulls pain, but I have not seen or heard that. NK1 antagonists like aprepitant might also relate to both mood and pain - although aprepitant is approved as an antiemetic, so yet another class of symptoms and diseases.

Thanks for explaining this. I have a lot of pain and a lot of weird illnesses. I was exposed to toxic chemicals in the military. My problem is that I have treatment resistant depression. My doctor recently prescribed blood pressure medication and nortiptyline along with my venelifaxine. I don’t have high blood pressure. I’m not taking it! I feel like I’m a Guinea pig. I ran out of the venelifaxine because it’s on back order through the VA. This brought on withdrawals, so I had some leftover Paxil. I’m taking 40 mgs a day for two weeks then I’m stepping down to 20 mgs for a week, then nothing. They want me to try ketamine or ECT. Not doing that either. I’m considering going to a state that has easier access to psilocybin and trying that out. If that doesn’t work, maybe I’ll eat my gun…

All praise and thanks to our Creator, the Quran and lifestyle of Prophet Muhammad, peace be upon him and upon Jesus, Noah, Abraham, Moses, Jonah... the body pains come and go mostly go quickly now. Alhamdulillah (praise and thanks to the One Creator) and Glorified and Highly Exalted is the One (Subhaanallah!) . Try reading/listening to Quran in arabic and english and ask the One Creator for healing and guidance. 20 yrs ago that's what I did. I want to teach my grands /great grands this. I already told my children. LOOK KIDS, NO PILLS, LOTS OF PRAY AND PRAISE AND GLORY...FOR OUR CREATOR! I am happy with the One Creator as my Lord, with Submission to the One Creator as my way of life and with Muhammad, peace be upon him and upon Jesus, Noah, Abraham, Moses, Jonah... as prophets and Messengers of the One Creator Almighty, aameen (amen)

Many people report significant improvement in pain after using xyrem(GHB). MANY Drs in the past uused it as an effective off lable treatment for your condition. Too bad its controlled and prohibitively expensive

When you're manic do you feel naturally warmer, or do you still technically feel that it's cold but it just doesn't make you feel cold within yourself? I wonder if it's got any kind of relation to those people who walk over burning hot coals or who can lift cars above their heads, from increased adrenaline or something.
