ONLY started noticing hair growth after doubling the dosage of minoxidil. 1ml is ridiculously low. It barely covers your scalp and most gets on your hair. I use a minimum of two FULL droppers worth twice a day along with derma rolling once a week. I was using the two-bit four drops of minoxidil for 8 months with no effects. Three months after doubling up, I had great gains. No side effects at all. Most side effects are all psychosomatic and guys getting so worried about the non-existent side effects due to the power of suggestion. So don't worry about it. Trust me.

Might want to consider microneedling every 2 weeks at an absolute minimum. This isn’t only for recovery but important growth factors

Have you seen the recent results about Verteporfin? Have you mentioned or discussed it with colleagues since its seems it will help limit scarring in the extraction regions and is seemingly safe in terms of side effects

Thanks sm for this video! It took my about 6 months to get out of this rabbithole…when you are constantly looking at your hairline and so on….hairloss can really be a terrible thing. I will be getting my hair transplant in switzerland in summer :) But I think I will go for the FUE because I always wear a fade and I think this would not be possible with the FUT scar… what do you think? I am 25, on minox and fin.

What is the survival rate for FUE vs FUT in Drs Feller and Bloxham's practice? And can you possibly speak to why Hasson & Wong claim they can get comparable, if not better, survival rates with FUE? Perhaps Drs Feller and Bloxham could collaborate with them to learn some of their tricks for improving FUE results? I say that because there really are some benefits to not having the FUT scar, like not having that noticeable dent in the hair in the back where the layers fall over the scar.

I’ve noticed that in many severe cases where men bald very early, where their case is more rare than the balding man in his 30’s, they need Dutasteride over Finasteride. Though I have seen Finasteride work very well in those men also, it does seem that for those whom Finasteride is not enough, they tend to fall in that category more than not.

There is no specific science with androgenic alopecia. Some men can stop it with low dose finasteride, some with full dose, and some will need dutasteride. And of course some have aggressive genetics and nothing will help them. All we can do is try.

I had 2 FUT transplants but had poor growth. The problem with FUT is cutting away and sewing back a strip at the back will tighten your scalp. My scalp is still tight after many years after the op and losing the transplanted hairs

Hello Doc, Thank you first of all for this informative video. Did you find out before the operation what is the diameter of your hair on average? I wonder actually the thinkness of your hair. For example, I have thin hair and I wonder if I can get a great result like you with 2000-2500 grafts and my bald area is just like yours before you got hair transplant. Accordingly, I will know how much area 2,300 grafts can cover. Kind regards...

Hello! I do believe I have more wider diameter hairs you are right. However, patients with lower diameter hair tend to have MORE hair follicles in their SDA as more can be packed in anatomically. This means we still generally achieve great results with these patients as often we can harvest more grafts to compensate for the low diameter hairs.

@@DrParissis Thank you very much for the feedback. I have one last question that I can't find an answer to anywhere else, I hope you will answer it. Can a daily shedding rate of 100 to 150 hairs be considered normal for everyone? For example, if a person with healthy hair has an average of 100 thousand hairs on their head and sheds 100 hairs per day, we can consider this normal. But for a person who has experienced hair loss until the age of 30 and has lost a certain amount of hair, is 100 hairs a day still normal? For example, I don't think I have as much hair on my head as everyone else. I am 33 years old and the volume of my hair has decreased until this age. Let's say I have 50 thousand hairs on my head, is my normal still 100 hairs or half of them?

Hey man, cool video. I was thinking of having an FUT procedure as well instead of FUE. I was told FUT has a higher chance of being successful and having that strong healthy restored look. What's your opinion on that ? Would you still pick FUT over FUE today if you were me ? (I'm in the same position as you, need to fill my temples mostly)

The research evidence for growth yield differences between the two is mixed - although FUT does slightly edge FUE out. However, in our practice we note much better growth yield with FUT in the AVERAGE patient that comes in (ie of the patients that get FUT, more of them have good growth compared to that of FUE) - however note that this is just my anecdotal opinion (the lowest form of evidence). We are working to add to the research in the future by doing some side by side studies. The biggest argument for FUT over FUE is currently the fact that you get more grafts using FUT first over FUE - specially the average patients gets about 3,000 more grafts over lifetime - enough to solidify half of the top of the scalp!

the hair transplant is what gained him all of the ground back on the temple but remember the important part is he started taking finasteride to prevent further loss. You could argue dutasteride, the stronger 5ar inhibitor, would be better to prevent further loss in the future in severe MPB cases like this. 21 is really early to see balding that severe. if you get a hair transplant and dont take any major 5ar inhibitor life finasteride or dutasteride. you are just going to continue balding and thinning at the same rate as before. the transplanted hair is resistant to dht and wont bald like the normal hairs on top of your head so long as the donor area is quality but the hair behind the donor hair is going to continue thinning and falling out. and its not like you can just keep getting hair transplants to catch up because eventually you will run out of donor hair, and that would be really expensive anyway to keep going back and doesnt make sense when you can just take the medication.

The only issue I have with the hair transplant, is that not everyone has 10k US dollars or more to spend on that. Especially with it not being a 100 percent guaranteed return.

Strip surgery is removal of a strip of skin with hair grafts in it - and then removing the grafts and implanting them into the desired region!

It depends. Some people could get away with doses as low as 0.05mg, and others will need to take much more. It’s very individual

I’m trying 0.25 twice a week, maybe go up to 0.5 a week which I’ve read can be quite effective if you don’t have aggressive loss

@@52yamahar1 Finasteride has a half life of 5-6 hours in your blood before it gets excreted. Taking it any other way than daily is counterproductive. The studies clearly suggest 1mg is optimal.

3:25 now hear me out. My father is 65 and has great hair still. Though I can see some bald spot and thinning. But still not that noticeable.And here am just 23 & my hair condition is already worse than his. I never expected this to me. Am such an unlucky person 😢. (& the most concerning part is I haven’t found my soulmate yet) am going crazy.

Hey man. I am same age (year younger actually) and in the same situation, I also never had gf and consider myself unlucky and I am also scared (this is the most calm I've been in last 3 months). I just wanna say that we need to have some hope. For now lets enjoy what's left and save up some money for hair transplant. Lets be strong for at least a year or two. And if we fail lets help our surroundings but until that we hold our heads high. I am exhausted because of it but we have to make it through. I wish us both full head of hair and may god be on our side. Might update every next month

Idk bro minoxidil grew my hair back in 6 months but yeah I was doubling the dosage 2ml 2x a day to cover more areas no side effects now just using it daily just as I would brush my teeth daily also mix it with rosemary oil 2x daily while taking DHT pills with biotin daily

Have a blood test to rule out anything other than alopecia. If the blood test comes clear, see a dermatologist to confirm you suffer from alopecia. Oral finasteride and topical Minoxidil is what you will most likely be recommended

⁠@@lt7099The occurrence of side effects is only a 2% chance The most important thing is to not stress and be mindful while taking it. Most of the side effects come from placebo, meaning your mind will play tricks on you if you worry too much!

I`m not an expert...but ins`t this a bit outdated because of the results you can archieve by minox, topical finasteride and eventually Tretinoin as an Minox enhancer? No? Just saw videos with some amazing results...and there seem to be studys backing that up....but maybe some people prefer the transplant? That`s whats going through my mind right now....not meant to be criticism....

Ok, a summery: - You experienced _agressive_ hairloss and at a _young age and_ have a mostly bald father - Your hair loss is apparentely so agressive that even with finasteride AND rogaine, your hair loss _still_ clearly progresses extensily, yet at a slower rate - You use words like 'I wanted a pernament solution" and 'a solution so I was not bald for the rest of my life' when you speak about a hair transplant when relatively young, yet you seem to have all the odds against you (apart from having thick hair). So your solution that you now have is not likely pernament at all. It's a solution that might _feel_ pernament now, but it not reasonably seem a pernament solution at all as hair loss typically does not randomly just stop? Sure, you can take another hair transplant. But reasonably, you will likely run out of donor hair, as it's quite hard to cover up your head when you only have the original sides left. ALso it's not optimal to shave it off either, since you have a scar. I do not want to be the person to make you feel bad, but the way you speak about the matter seemy very naive, like you're not understanding the big picture. Just because have a good hair transplant doctor, does not mean he can magically increase your amount of donor hair you have, as the amount of hair on the sides is a finite resource. Only if you know you have both thicker and more hairs than average, then I understand your almost naive seeming optimism. However, maybe I am missing something here and in that case you can ignore my comment of course..

If we're gonna talk about the "big picture" as you say, then timelines are also important. By the time what you describe comes to pass, it's very unlikely that tools like verteporfin won't be involved. The way things are looking right now, it wouldn't surprise me if verteprofin turns out to be useful without hair transplantation. Also, even putting hair transplantation aside, finasteride + rogaine is absolutely not the ultimate hair loss stack so even on the medication side, he's far... FAR from having exhausted his options

​@@maloxi1472 We _do not know_ that anything like that will even come out and even work on humans as intended and without too many side effects. So it _might_ solve his future problems and it _might not_ and should thus not be treated as uncertainly, because we objectively know for sure it's really not a certainty. Also, if he is 'far from "exhausting" his options, then I am curious what you think his options are in your idea that he has not tried yet. He has tried Fin, Min and hair transplants and he has rather poor results with trying _both_ Fin with 2 of them. If you think he is "FAR from having exhausted his options", then I kindly ask you to mention this list his other options . As you mentioned he is _FAR exhausting his options_ with capital letters, it does suggest this will be quite a big list

I switched from finasteride and rogaine to onion juice and saw palmetto and the results have been absolutely staggering. The transition was absolutely seamless and my results have actually been better. 3 months ago I added rosemary oil to my routine and it only enhanced my results

You won’t have as much blockage as every day, but if that’s the amount that you don’t get side effects from, then there’s no safety risks of taking 1mg every other day.

@@jojobrooklyn5285 No problem. So according to the dose curve, on the day you take it you block 66% of DHT on the day you don’t that dose from the day before blocks 48% of DHT. Long term you’re probably blocking about 57% of DHT instead of 66% - not much of a difference. But then again if you are super sensitive to DHT, it could make the difference.

@@jojobrooklyn5285 starting off on a lower dose is always best and building it up . But for long term prevention , depending on your degree of hair loss, 0.5mg every other day may not decrease DHT levels enough.

Actually he lie because 2018 I got fue hair transplant!and several years after my hair shedding and thinning but ı started micro nidle and minoxidil and almost all my hairs come back and thicker

What worked for me was onion juice, well consistensy is the key...washing my hair everyday wasn't an option as I have 2c hair and it's pretty dry so it didn't work when i used it on my wash days only...then I tried something different...appling onion juice with aloe juice on my scalp with adding few drops of lemongrass:(to mask the smell), rosemary essential oil and castor oil (only3drops) and didn't wash off. I did it at every 2 days interval, now I do every other day and trust me it really works. I have a lot of baby hairs, you just have to be consistent and patient

I would still use rosemary every day. It takes about 6 months to kick in but it does work

"Going up the treatment ladder" is a myth that needs to be busted when it comes to hair loss and I wish you had put more emphasis on that point. Oils and diet changes won't do anything for androgenic alopecia, even topical Minoxidil on its own will only mask the problem at best. You need to get on oral or topical Finasteride the moment you notice hair loss and even more so if its aggressive.

​@@sapujapu6323 Well things like Saw Palmetto and Pumpkin Seed Oil are AR-5 inhibitors anyway, so you might as well just use a proper medical therapy to begin with and not risk 6 months or more of lost ground because you were in the 50-70% of non-responders (assuming the studies you refer to are actually good quality). People can also start with topicals to reduce their risk of sides if that is a concern. The "treatment ladder" should really be inverted for hair loss because all the time you spend trying other approaches you are potentially losing ground that could be the difference between needing a transplant or not, especially if it's aggressive loss.

@@DrParissis and zero sides with fin? I'm like you, I started to lose hair at 20-21yrs old and I'm now 24. Due to my hairstyle I can cover my shitty hairline atm. I'm not super enthusiastic to take finasteride for the rest of my life and potentially get more than one hair-transplant. Don't you reckon you'll have to get more hair-transplants down the line? What kinda of sides was Dutasteride giving you if I may ask? I'm currently taking only saw palemtto and other natural remedies that seem to have at least slowed down the inevitable process. Do you have any thoughts on natural dht blockers like saw palmetto? What about post finasteride syndrome? That's kinda scary. Thanks man :)
