What is the best treatment for lichen planus of the vulva please. I am suffering so much . Please help me. I’m going crazy with it . 🙏

I only diagnose it; I do not treat it. So I am not an expert in treatment. But my basic understanding is that it is often treated with strong topical steroids. But these require a prescription and can cause permanent damage to the skin if used incorrectly. It is important to see your dermatologist to make sure the diagnosis is confirmed and the proper treatment is prescribed. Gynecologists can also treat this but I think that dermatologists in general have more experience in dealing with it and other skin diseases of the vulva. But either way, please see your physician for evaluation and treatment recommendations. I am sorry to hear that you are suffering and i hope you get better soon. I wish you healing and good health.

Dr Gardner, Does this Lichen Planus occur on the lip, on the side of the oral mucosa? I know that you are showing an example in the leg, I think I have seen this band of lymphocytes, and plasma cells " attack and “chew up” and destroying the basal layer keratinocytes, in a 85 YOA female. This I believe causes vacuolarization/ liquifactive necrotic changes. Leaving the “pink blobs” of dying keratinocytes at the dermoepiderminal junction that you called here cytoid bodies or Civatte bodies? The major difference in the case that I saw, there was there also was a greater than 3 mitotic figures in three HPF. Is this consistent with Lichen planus? Is it possible to tell a keratinocyte from a melanocytes if we are in the mitosis phase of the cycle? You rules or hints for interface cells have help me greatly. Thank you.

That’s incredibly hard differential for me still! I’m not sure how to teach that yet but I’ll try to do some verrucous things in future.

I've been suffering from this for 20 years. Will cosmetic procedures work in removing those areas?

I’m sorry I know this can be a very frustrating condition especially for patients with darker skin types because it often results in hyper pigmented areas. I diagnose this condition as a pathologist but since I’m not a dermatologist I don’t treat it so I’m no expert at treatment. My understanding is that it can be difficult to treat. I have heard there are cosmetic treatments to help with hyperpigmentation but my impression is that this is a difficult problem to fix unfortunately. Please see your dermatologist to discuss all of this and ask them what treatment options they can offer you. Best wishes for good health.
