There are around a million simulator games made by German game studios (or targeted to a German audience) about basically everything. Farming Simulator is just the peak of the iceberg. I'd recommend something like Emergency Call 112, which is a Fire Fighters game. But there are so many and you'd be brilliant in all of them.

I get the feeling the game's creators were thinking of 19th century Wild West taverns (which still didn't have indoor bathrooms) but, you know what, why not. Not to mention there were no Health Departments or Ministeries of National Health around in feudal medieval times, predating as it were the invention of the nation state and concepts of hygiene in medicine. But, you know.... it must be time travel...
