I think you should pop all pills as long as your build can accept a tears down. You're more than likely to get two of the same pill, so comparing getting a range up to a range down: If it's a range up, you take the second range up. If it's a range down, you ignore the second. The upsides way outway the downside on average. (tears down with low tears can outright kill a run though)

Remember the mod that you made lazy mattman? I had an idea for an item. It’s called birth certificate. How you get it is by using death certificate in a room with birthright in it and it will transform the birthright into birth certificate. What it does is it takes the natural innate abilities of characters and puts them on steroids. So here is an example for you, tainted Isaac will get 24 item slots and have a mini glitched crown which will cycle between 4 items. For the forgotten, you play as the soul and the skeleton at the same time like both characters will be active and depending on who is in control will act independently while having the same items you picked up like let’s say you have brimstone. If you are playing the skeleton, the soul will find an enemy and shoot them with their own brimstone. For tainted lost, holy cards will spawn every room that you have cleared, it adds extra lives back into item pools, and it makes it to where every item you pick up give you a random stat up even if it does affect your stats, like Polyphemus, it will give you the large damage up yet it will give you an upgrade to a random stat like range or speed or tears.
