In this episode, Dr. Douglas Lucas discusses the crucial role Estradiol levels play in bone health. Many doctors do not test estradiol levels, which can lead to inadequate protection of bones. Different types of testing, such as urine and saliva testing, can yield different results, but blood testing is the gold standard. He also delves into how compounded creams and commercial patches that can be used to achieve the desired estradiol levels. More is not necessarily better, and hitting the threshold is sufficient for bone health!

I’m 65 yo and was denied hrt after having ovaries removed and recently received an osteoporosis diagnosis. I’m seeing an obgyn (toronto Canada), this afternoon , who provides bioidenticals. Wish me luck. I’m outside of the 10 year window for greatest benefits of hrt but according to Dr Menn , an obgyn menopause care specialist in New York, we do not turn into pumpkins after 65 yo and still can benefit. Thank you Dr Lucas for this remarkable bone health program. It is truly one of a kind.

I too am 65 and was denied hormones (in British Columbia). I found out I had osteo when I fractured my T7 six months ago. I am curious to know if you manage to get hormones or not.

In the UK, we measure oestradiol in pmol/L rather than picograms. So 60pg/ml is 221 pmol/L. (220.26pmol ) Mine is consistently lower than this. NHS doctors make a fuss about patients asking for an E2 blood test, 'no need' , or, ' we don't do that '. I've had to stand my ground to get it tested annually, and that's the only reason I know, and that they know, my levels are inadequate and have been for years now. Then there's another big fuss about asking for a higher dose of oestradiol. It's such a faff to go through this power play nonsense, and yet it's so important for patients to get what they need. Especially for we patients at higher risk for osteoporosis, and not just because of menopause. Thank you so much, I will remember the information from this video for my next battle 💛🤗xx

A heartfelt thank you Dr. Doug from Canada. The research and presentations provided are very much appreciated. I’ve been following your presentations for several months, since first diagnosed with osteoporosis. Your work educating women like myself with knowledge and science based research has empowered me to question the standard medical, drug based approach. Like others who have posted here, I’ve to had to push through and keep asking about a hormone alternative for osteoporosis, which can be an onerous process in the Canadian medical model. I’m determined though, to resist using osteoporosis drugs for as long as possible. My intent is by accessing accurate knowledge, lifestyle enhancements and HRT, I’ll continue to live a vibrant, healthy life into old age.

I’m an Ob/Gyn focusing my approach on optimizing diet/lifestyle/mindset +/- HRT and giving lots of physiological Testosterone. There are no studies looking at brain, bone and body optimal health for women when we replace T as well. I aim for 70pg/ml E2 for women on no Test replacement and 30-40 pg/ml when giving concomitant Test. Do you have an opinion regarding optimizing hormones other than E2? I treat with Oxytocin if there is evidence of prolonged grief (like divorce or death of a spouse) or complex PTSD. Thank you for the courage to have these conversations regarding HRT and what is optimal for ‘humanity’. With woman commonly taking on post menopausal ‘rules, roles, and responsibilities’ that were uncommon 50 years ago we need to rethink what is optimal.

I think SBHG also plays a role in estrogen levels/testosterone levels and availability. blood sugar levels, inflammation plays a role. so does thyroid and T3 to T4 conversion

Thank you for providing links to the studies! I will bring them to my OBGYN for my well-woman visit. I have to argue with her (and bring studies) to up my estradiol every time I see her. She keeps quoting "the lowest dose, the shortest amount of time" crap. I also have to push her to get lab testing for hormones. I have been on corticosteroids for asthma for over 20 years so HRT is so essential for me and my bones. Great video!

I’ve just started taking HRT at 64, after seeing a different Consultant. She asked why I wasn’t taking it and wasn’t impressed when I said I was told I was too old by the previous Consultant who just wanted me to take drugs. She assured me it was one of the best defences against bone loss and for my heart health.

@mandysmith7812 Thanks, good to know. Just last week, I ran into a GYN acquaintance (not my Dr) and told him about Dr Doug. I commented about a recent bone density test and that I'd seen him once several years ago about Bioidentical HRT, and he told me I wouldn't benefit from it since I was post M. He'd recently closed his OB/delivery practice and changed his focus, so maybe he wasn't as well versed at the time as I thought he'd be, or just thinking about symptoms other than bone. I asked last week if I could take them for my bones and he said to come see him.... only I don't know if I want to now.... how much confidence I'd have in him. I also asked if he was familiar with the REMS testing, and he wasn't so I briefly told him about that. He's independent from the clinic he was part of, so he isn't bound to a corp or big pharma any longer. Hopefully, I can take some good notes from listening to Dr Doug to talk to this Dr or my GYN. Mine is part of the clinic the other Dr left. I'm about exhausted from trying to put this all together, and crossing my fingers that I don't end up in a wheelchair one day.

​@@mandysmith7812yeah especially the patches don't increase the risk of breast cancer and I think stroke is less too. Take your stroke preventative foods as well to minimise stroke risk you should be fine. Osteoporosis is pretty harsh and the drugs they give for it also harsh and risky so anything that can stave off that problem seems like its worth doing. The drugs for osteoporosis are also risky and less beneficial so it kinda seems crazy they don't offer it as long as someone is benifiting and doesn't have a problem related to it.

See Dr Heather Hirsch... If you have a uterus, you need progesterone to keep the lining of your uterus from thickening and protect you from risk of uterine cancer.

If you have a uterus, you should take progesterone with estrogen/estradiol. Progesterone reduces the risk of uterine cancer.

Please can you clarify the European equivalent. Eg a result of 83 pomI/L and the doctors in the uk recommend 200 for bone health. How does that compare to the 60 you mention?

In the graph where both estrogen and endometrial stimulation is displayed, it looks like the optimal level of estrogen is just shy of stimulating endometrial tissue? Can you explain a little why that comparison is important? The take away for me is bone health good vs endometrial stimulation bad?

You should ask for bio identical, horse estrogen isn't good for humans...not to mention for the poor horses....best of luck

I used these for years from a pharmacy in Eugene, OR. The pharmacist was very helpful anytime I had questions.The compounding pharmacist is a good place to start.
