I've seriously fought trt for years. I'm 40. I have been drinking 2 years. I don't smoke. I don't even smoke weed. I have maybe two cups of coffee a day. I exercise 4-5 times a week for years. Sleep 6-7; hours a night No libido, no motivation. Tired. Bouts of depression or the blues. I got blood work done last week. My overall t is 91 and my free t is 1.6. I feel so much relief knowing what's been going on with me

So the one thing you’ll have to define for yourself is what results you expect. For me, because I have been training hard for so long (since ‘91) the TRT didn’t do a lot for physique (except when my levels briefly got too high). The results I was hoping for were centered around energy. I was really dragging at the time and that’s where the TRT made a huge difference.

I am very impressed for being 185lb the size you carry. I'm on trts well for medical reasons for hyperlipedimia. I've had this for over 20 years! In 1 session, it was back to normal. TRT has been a game changer for me all around. I've been given a second chance. I've stopped drinking, smoking, etc.. TRT set back the clock at least 15 years.

I am in the same boat as James, although not near the lean muscle. I am a software developer and the last 2 years my Testosterone went from 605 to 450 and my ability to concentrate and motivation is in the toilet. I can sit at home on a Saturday and get little to nothing done. I am an active Crossfitter and I do enjoy working out and plenty strong and great body composition for a 56 yr old. I really want to have some giddy up as my first priority, Libido and muscle growth fat loss in that order. Thanks for being transparent, very helpful

I wish there were more doctors like Dr Osborn and easier for people to access them. I am 42 and energy levels suck most of the time. Its just expensive and insurance does nothing. I'm not trying to become jacked. Just want to feel energetic again without going broke

A man with a Testosterone level of over 500 given 200 mg injections of Testosterone a week so he can get his levels over 1500. He will have to phlebotomize because his HCT is going to be dangerously high. Do u agree with 200 mg of Test a week in this case.

@@Patrick11223 Yes, your Hct and Hgb count will go through the roof. Increased RBC count will increased risk of stroke and blood clots. You have to give blood at that point or risk self harm. I wouldn’t ever want my Test level to be higher than 900 for that reason. If body builders are doing it they probably have their labs being strictly monitored.

James - and anyone that reads this - I’m 49 now (2022/March) and I have been feeling like chit since I was 38. For the next 5-7 years I tried to find out what was wrong with me and all my doctors that I visited said I was good. I didn’t have drive and could sleep for hours only to wake up to get back to sleep. After 45 years old I ran into issues like my appendix then COVID and just now - after all this time I get some answers to my questions thanks to you James. My man , I don’t think I will go on TRT but will be trying to maximize on what I have for this year but I will do a follow- up post in the future. 🙏🙏👌💪

Dude these 3 videos with dr osbourne have been exactly what i was looking for. Thank you and him very much. I just turned 30 and had my blood work recently done. i have a 357 Total T but other metrics are normal. Been feeling very groggy and unmotivated and i suspect thats why. I know i have a few things a in my life i have to optimize before even considering Trt but This is the knowledge i was searching for. my doctor said because said that because i fell in that 350-1000 range that im totally normal but i wasnt really buyin that since i was borderline. 7:35 mind you the average in 1984-85 in men ages 22 to 32 was 1200. That range is something newer that the medical community adopted within the last 15 years or so. I take 170 mg every Wednesday and my resting T is 780ish. My medical team started at 50 mg and increased the dosing slowly on a monthly basis.

Important Side Note: One thing that didn't get covered in this video is that my first labs were done in Jan. of 2016. The strategies and philosophies have changed a bit from that time, especially as it pertains to using an anti-aromatase. If you listen through the video you'll hear us discuss it.

You all make TRT sound complicated. If you are on a small dose of say, 100mg of test per week, if will have minimal effects on fertility. If it does, simply take HCG for 4 weeks and you are good.

It gives people who have tried everything else an opportunity to regain some youth. When my libido drops and my energy levels drop due to age I’m heading straight to the clinic because I’ll be 48 this year and I know my time is short. Sylvester Stallone is 75 and still doing weighted pull-ups because he’s on the good stuff.

I month ago Started Nebio 1000 mg for every 12 weeks. I just had a blood test today, but need to wait for result a couple days. Sleep improved a lot, strength improved, libido is same...but testis are sore..hence blood test...😁 I really appreciate this info what you put out for us. 👍👍

Thanks James, very intesting. One question though: in your first 2 results (at 5:30 and 8:00), the Free T reference range is 6.8 to 21.5 pg/ml, while in the third result (at 12:40), it is 35 to 155, still pg/ml. What is this incoherence ? As a side note, if the free T is to be 2% of total T, only the last range can be correct. I saw in another comment from a pharmacist that the difference in ranges doesn't really matter, and we should just look at where we position relative to whatever range our lab has established. But still, this is disturbing to get a result which means our free T is 0.2% of total T...

If I hadn't worked out hard and consistently for the past 30+ years and didn't always watch what I eat, what would I look like - and would anyone ever say that I had good genetics? It's hard to say. There's a chance that I may look slightly better than someone else with the same lifestyle habits but probably not much different. Genetics is often an easy scapegoat for lifelong lifestyle habits.

Im 25. I got tested and was at 520. 7 months later, im in the best shape of my life (so far) , but it dropped to 465. My diet, etc everything is on point finally, but showing all the symptoms of low T. Dick issues, libido issues, sleep issues, energy issues, it goes on and on. My NP prescribed me Test-C after my GP prescribed me antidepressants which i nearly killed myself on because it made everything 100x worse. I hope this gets me back to feeling normal. I shouldn't feel incredibly worse, when physically my health is amazing compared to back then. Im so tired of feeling how i feel 😞

I’m 46, and this morning have started TRT per doc direction. I’m nervous because I domes line to put chemicals in my body…. But I have zero drive, ambition or libido, no energy ever… I hope this works because I’m tired of getting like garbage…. I think food made in America has a lot to do with why I feel like garbage…. As soon as I got my house in Costa Rica…. The food there is real and not processed or given hormones…. Chicken breasts are NOT supposed to be the size of your human thigh. My Costa Rica diet is impossible to follow in the US…. No fresh food, seafood or fruits…. Everything is processed foods in the US

Not since Rich Piana have I seen such a honest,to the point, break down of a hormone adjustment. You're much better than me Gage. If I shot T levels up, like yours did, i'd freak if I had to give blood to start over again. lol I'm a guy who doesn't do needles,vaccines, or even aspirin,well I took Tylenol all 4 times I had corona. That's it. 64 and probably negative T in my body. But I just keep fighting thru. i'm a candidate for sure. So a video like this, is certainly inevitable if if continue bow hunting with an 80 pound bow the next several years if I stay healthy. i'm gonna need a boost. 200 Mg was too much,so maybe like 25 mg,just a little boost. Might work for a guy like me who isn't into taking meds. BTW love your content,and think you're fairly local to me in Palm Beach-Dade ? When I started back up in the gym when corona went down, I did the 20 reps thing to get range of motion,and restrengthen tendons etc. Got the tendonitis and your videos on it helped me immensely,the Doctor from the Dolphins ? Warm the body part up first before stretching it ? Turned my life around !! No bullshit. Shoulders were killing me. I bow hunt and shoot a ton of archery. It was the BICEP the whole time, not the rotator or labrum. I warm up good, and stretch,do a set, stretch again, and i'm good. I know you don't like doing it, but i'd like to see how you handle and workout using heavier weight maxing @ 12 reps or so ? Seems many guys like John Means and even Jay Cutler, feel 12 is a good way to work the muscle ? 20-16-12 is sorta what i've started doing. When I was your age, I maxxed @ 8 reps,,,,,lol 8 reps was lite weight.

Excellent video. This doctor definitely cares about his patients. I like many others enjoyed and reaped the benefits of TRT when I first started. That was short lived. Now I feel absolutely no benefits and I feel just as bad as I did before starting TRT. I've tried every protocol out there. Nothing has worked. Libido dead. Energy dead. Drive dead. Horrible. Tried countless endos and doctors. No one can fix it. Enjoy the honeymoon phase my friends, because after that, its gone forever.

I just switched to one dose a week IM. Second week of this. I feel a little better but still no where near where I was originally. I’m not giving up.

Dr has referred me to a urologist, but I worry they will try to tell me its normal and I have to accept the depression like state im in.

Its 700 on the lowest stage on TRT, for a 7 day pinning. Curious what it is at its peak. Usually 48 hours after pinning then gradually descending. I pin EOD because at once a week, I was crashing after day 6.

Yeah, It's hard to say becasue if you look at it it seems clear that its a peak and valley effect, but..after several weeks the wave pattern does get flattened out. If you were at zero on last day, then you would just get the same wave pattern, but you get more and more overlap as weeks go by.

@@mikearonson1071 I do 0.25 every other day. Tried once a week. Didn't like the crash. Did twice a week 0.4 and as well, but notice the trend is microdosing eod. Less peaks and troughs. Everyones different though. Need to find what works best for you.

Not sure if I will get a response since it been some time but I really could use a physician like this in the Florida area (i live in miaimi). My levels: 8.5 Free 450 Total I am 26 years old, I weigh 180 pounds, I heat very healthy (weigh all my foods etc). I lift 5-6 times a week. I am sore more than often and I refuse to say I am depressed but my motivation is seriously lacking. I really need some help but I don't trust doctors and am so worried I will go into the wrong Endo and get someone who just prescribed for the symptoms rather than actually helps me address the problem and if worse comes to worse prescribes me TRT but forgets about maintaining my sperm count etc. Really could use and all advice. Lacking a lot of direction but I refuse to give up.

Appreciate it my friend. Yes, cypionate is the only thing I take. I don't take growth hormone but I'm sure there are plenty of doctors that would give the thumbs up if it was done the right way (to optimize healthy normal levels).

@@JamesGrage - I had a consultation with Evolve and they recommended HCG to keep the nuts hanging and CJC 1295 to stimulate HGH secretion. I hadn’t had any blood work done, but that was the basic protocol.

Great video. I do think you missed talking about how this is affecting your lipids, especially Apo-b and hdl. These inevitably can turn into bad values.

I just started trt. My 1st injection of just 1ml every 2 weeks. My doc said my free testosterone was ONE!! I was told i almost had enough testosterone to be a 90 year old woman. Im a 47 year old man. But ive also been a quadriplegic for 30 years and in pain management for 10 years. Opioids can destroy your testosterone ive heard. So i hope this helps because i had all the symptoms of Low T

While on trt my gyno went crazy, my estradiol was 60. I used to to cycle tamoxifen to manage the gyno, but I saw one comment one day of a doctor that said to take 0.5 milligrams of anaztrasol per week. That’s a super low dose and I started taking 1 mg per week. Never had issue with gyno anymore. I’ll probably lower to 0.5 mg

On .5ml 100mg test cyp I’m at 1160total and 58 on free test. I metabolize it slowly. Get blood draw every 60 days to keep hemoglobin good level.

bro that does not mean your 1500 total test. you wouldn't land on an even number like that. the bigger then sign means 1500 is the limit on that lab test so you are somewhere higher and you have no idea how much higher fyi

9:00 can you take baby dose of aspirin to thinner your blood, is that enough? I used to eat those until someone said that dont eat those without a reason or you may get cerebral hemorrage (blood is too thing in this case). So, how can we know when the blood is not too thin or thick + there are genetical factors e.g. "too thick" blood may safe your life since with "normal blood" you would have got a hemorrage..... very complicated and I dont know if anyone has an answer to this even with full examination science knows today.

Why would I block estradiol? It’s cardiac protective. I didn’t block it in my teens and 20’s. There is no study showing a phlebotomy helps. Again, we didn’t do that when we were at our highest levels naturally. Is there a study showing these 2 issues?

our bloods look very similar , I had issues with my H/H , Learned to do things smarter.. We have exact MCV MCHC , RDW Platelets, ect... One thing i was curious on was your liver enzymes were a little high i believe? I may have got confused or got distracted. What was the reason for that to go up? And just to share my last blood test, i tried to lower my hematocrit and red bloods by taking a month of TRT.. Well I dont advise ever just stopping.. I felt great for the 1st 3 wks but by week 4, very tired, pale got sick with fever and just in a catabolic state.. My total test was 35 free was 0.1 and i thoght i ws reading the test wrong.. It def lowered my bloods into the 16 ng/dl for hemaglobin. but i did that on my own and when i went to the doc and we went over the bloods he was happy to see them lower.. and even said maybe stay off test and let the body come back.. Problem is , im 45 and the reason i went to him in the first place is cus my testosterone levels were low in the low 300s and Nope i wasnt messing with Gear prior. Its not an Looks thing for me either,its feeling great and being optimal im looking for.. So anyway im writing and doing 5 other things at once , so some of this may come out scattered, but this was a great video, thank you for sharing your blood work..

FYI - the ranges between labs don’t matter as much as some people think. The lab sets their “normal” ranges so that the result can be Interpreted correctly. So as long as you’re interpreting the lab result based on the normal range reported from that same company at the same time you got the test done then you’re fine. In other words, the absolute numbers don’t matter as much as the result’s relation to the range. For context I’m a clinical pharmacist and look at/interpret labs all day, every day so I know more than the average bear
