Pernicious anemia is an autoimmune condition involving production of autoantibodies against the parietal cells of the stomach, which produce intrinsic factor that is required for absorption of vitamin b12. In this lesson, we discuss the risk factors, the causes, and the signs and symptoms of pernicious anemia, along with how it is diagnosed and how it is treated.

It's very hard for doctors to diagnose. NONE of my 5 specialists could, since none of them ordered MMA or Hyc. My Endocrinologist is the one who gave me the clue, so I could do it myself. As we were ending a regular check up, he said that my MMA was slightly elevated. Like I stated, it was never checked. It was a Freudian Slip. He meant that my blood markers indicated low B12. I found this odd, since I had been complaining that I had EVERY low B12 symptom but I had been injecting B12 for over 10 years and I knew my serum levels were normal. This was enough to get me learning about metabolic pathways. In my case it was functional B12 deficiency due to MTHFR, DHFR and NAMPT SNPs and YES I figured that out too. They had me on Methatrexate, Sulfasalazine and Folic Acid. (NOT GOOD!). Since "HMT" requires 5-Methylfolate, a lack of Folate causes decreased activity of HMT. This decreased activity can actually drive serum B12 up, since it's not getting used. This is where doctors and patients often get confused and why MMA and Hyc is so useful. Since MTHFR is B2 dependant, Riboflavin deficiency can also lead to the same set of symptoms. Even B3 (Niacin) can lead to these symptoms because DHFR and MTHFR, both require NADPH, a metabolite of NAD. One day, they will standardize at test for SAM/SAH ratio. (s-adenosylmethionine/s-adenosylhomocysteine). This will prove to be very useful.

I injected Cyanocobalamin for over 10 years, until I learned about my Methylation problems and how these problems were affecting my liver. Purely theoretical but I found it curious that people supplemented with a form of B12, for energy, that is formed with Cyanide, when Cyanide is known to inhibit Complex 4 of the Electrol Transport Chain, the very process responsible for energy production. It also has to be filtered out by the liver, the "Power House" of the body. Once I learned that, I switched to Hydroxocobalamin, until it got harder to find. Now, I inject Methylcobalamin.

I'm not a doctor or even an expert but that's the first time I've heard Methionine Synthase refered to as HMT. I've seen MS and MTR but never HMT. I guess you learn something new every day.

Many of my female relatives on my mother's side had PA, several died young, before knowing what it was. They didn't know what PA was until my grandmother's generation. Thank goodness for science.

I was diagnosed when I was 30. I always wondered if my grandmother had PA as well and didn’t know it. She passed away at 38 years young, and the cause was said to be malnutrition. This was in the 50’s. It seems to make sense to me.

My grandmother was diagnosed with PA at Mayo Clinic in the early 1940’s .Although none of her two daughters inherited that particular disease they did have celiac disease and RA in their early 50’s. Me , my sister , and my oldest daughter all have Celiac disease and I was the lucky one to get RA as well. To date none of us have PA .

My paternal grandmother suffered from Pernicious Anemia for a very long time but in the end she died from Leukemia. Is there a link between the 2 diseases?

MCV isn't necessarily high if there are other conditions such as iron deficiency.

Thanks for the video. this is really very well explained. My question is, given the Schilling test is now obsolete, after determining it is Vit. B12 def. anemia, how can the cause be identified? (autoimmune (impaired absorption) vs nutritional deficiency)?

If you take methylB12 and it doesn't work, you know it's due to intrinsic factor not being correct.

See Peter Osborne yt Hes very knowledgeable about nutrition. I believe a cause is Non Celiac Gluten Sensitivity....I figured out , on my own I have this. My aunt had pernicious anemia then I realized that my Mother had NCGS too. The villi become harmed from gluten, then one is not able to absorb nutrients properly. Be blessed❤

I jyave Hereditary Hemochromatosis and Macrocytosis Anemia. My B12 was normal but my folate was low. I get weekly phlebotomy and since my treatment started, I've been having pain in my sleen. Im starting to understand now that the large blood cells are putting lots of stress on my spleen. Its interesting to know i can still be deficient in b12 with normal levels.

I have methylation issues with b vitamins. Just diagnosed with pernicious anemia which I suspect I’ve had for years based on markers. I have other autoimmune conditions and type a blood.

THIS GUY IS 😮OTT I HAVE SOME MEDICAL TRAINING, 😂GOD HE HAD ME BAMBOOZLED.😮😮😮😮 I HAVE F.A. OR B12 DEFICIENCY. HIGH MCV. ETC. MY DOCTOR JUST GASLIGHTS ME ON IT ALL. DIFFERENT DOCTOR, PUT ME ON 3 MONTHS OF FOLIC ACID TREATMENT. BUT, DIDN'T FOLLOW UP ON ANYTHING 😢

I have celiac disease, polymorphism in the mthfr pathway, eosinophilis hypothyroidism, EoE, previously I had moderate homocysteine with high mcv (104), Mch, metabolic syndrome ( triggered with the gluten free junk food), bile acid malabsortion. Can you have Pernicious anemia with Vitamin B12 at 300 based on my list of chronic conditions ?

I think I might be dealing with hymoletic and pernicious anemia. I have one on my Mum's side and one on my Dad's side. I've been struggling with this since I had my son 20-years ago. My hemoglobin dropped to 50 and they were stumped as I did not lose any more blood than a regular delivery, they claimed (I had a c-section). Since then, Ive been diagnosed with borderline hypothyroidism, B12 deficiency and now I've discovered this G6P deficiency from researching why my body can't handle a henna hair application. My urine was the colour of black tea last night and I had an extreme headache, dizziness, and nausea. How on earth can I sort this all out.

If I were you I’d call my pcp and talk to the nurse. They may want you to get some lab work and get you in right away. If they aren’t available, go to urgent care. If you’re taking care of a small child you shouldn’t be experiencing dizziness, so make sure they know. Good luck!

I knew my b12 was low, but I never really got serious about taking a supplement. Well I have all of these symptoms, so I will be taking better care of me from now on 😅

Brilliant! now I understand my condition much more. I depend on one injection every three months... I have a question, normally I was receiving Hydroxocobalamin in the UK and since I'm in Asia I have received one injection of Cyanocobalamin is this okay or should I only receive Hydroxocobalamin? Thanks!

But necessary as Doctors who watch this will know and need to know and understand this. I'm sure there are more simple videos out there for people who don't have a medical background. This was over my head as well in many areas but you can at least write the tests down that he says will help get a diagnosis and request them. Knowledge is power.
