I think you must view your question from the perspective that OUR Royal Family do not represent the US in any way whatsoever, either culturally, socially or heritage wise and are therefore reported upon as a foreign news story that they have no part in. In the UK the Press (and the ordinary people) also refer to her by a loving name (a "nick-name") but the important point here is that the intention in NOT meant to be disrespectful, whereas this is not always (and generally is) the case in the US Media.
