In this episode, I explain how ketamine causes rewiring of brain circuits and dissociative states to relieve symptoms of depression and post-traumatic stress disorder (PTSD). I explain how ketamine impacts both the brain’s glutamate and its endogenous opioid pathways, which together regulate mood and well-being. I discuss how ketamine therapy is used clinically to treat major depression, bipolar depression, obsessive-compulsive disorder (OCD), suicidality and other psychiatric challenges. I also describe how ketamine causes the subjective effects of dissociation and euphoria and, at higher doses, is an anesthetic. I compare the different routes of ketamine administration, dosages and forms of ketamine, and if micro-dosing ketamine is effective. I also highlight the potential risks of recreational ketamine use (and the colloquial term ‘K-holes’). Lsd, ketamine, dmt and psilocybin containing mushroom are amazing. I was diagnosed with Cptsd 20 years ago as a teenage. Spent my whole life fighting Cptsd. I suffered severe depression and mental disorder. Not until my mom recommended me to psilocybin mushrooms treatment. Psilocybin treatment saved my life honestly. 8 years totally clean. Never thought I would be saying this about mushrooms.

Can you help me with the reliable source 🙏. I'm 56 and have suffered for years with addiction, anxiety and severe ptsd, I got my panic attacks under control myself years ago and they have come back with a vengeance, I'm constantly trying to take full breaths but can't get the full satisfying breath out, it's absolutely crippling me, i live in Germany. I don't know much about these mushrooms. Really need a reliable source!! Can't wait to get them

100% agree I used to have Psychosis and paranoid thoughts like "people thinking about me talking about me etc. Very odd behavior after getting off Adderall from 7-16. Antidepressants at 18-29. 31 now. I took way to much, but took about 20g of Gold caps (Psilocybin containing mushroom) I analyzed my entire life. The emotions that came out helped me understand behavior etc more. Wont ever need to do it again because I'm happy and contempt forever, but I wish more people did this to alter their perception of reality. Would help with healing much trauma

7 ketamine sessions, ptsd and suicidal ideation free for over 6 months now. My anxiety is also reduced by 70%. No more smoking. Sleep is finally getting better too. I finally started to exercise 3 times a week which further contributes to the improvement of my symptoms. What ketamine did for me was to make my brain unstuck on trauma which ptsd victims experience and gave me the ability to move on with my life and not relive the past over and over.

After decades of depression and failed psychotherapy and pharmaceutical treatments and a LOT of wasted healthcare dollars, I got complete relief from Ketamine and have been depression free for 3 years with NO added medications or psychotherapy. congratulations :) I have spent half my life trying psych meds for depression and anxiety and doing therapy (ages 13-25 now) and can't afford IV ketamine because insurance won't cover it but am able to afford the psycholytic small dose from a compounding pharmacy. do you think I can expect similar changes? did you do IV treatments?

I had chronic pain for years and was in a deep melancholic depression. I was not getting out of bed, crying throughout the day, not eating, unable to sleep or sleeping too much. I had my first Ketamine IV infusion earlier this week at the hospital and within hours was eating again, have slept well, began to run again, began doing pilates and restarting my doctoral program again later this month (was going to take time off due to my pain and mood). This was an extraordinary experience for me and beautiful for me. I know that I am not the only one. No more pain! No more tears! I feel reborn.

My mom did ketamine treatment earlier this year before she passed away from cancer. It helped her so much with the depression and fear of dying. Unfortunately, she was only here long enough to do two sessions.

Don't be discouraged if you feel the need to have boosters. It's really common and is universally accepted that the treatment doesn't last forever without more treatment

Jesus is my cure, something that is spiritual must overcome something so traumatic one experiences in life. That's my personal belief. That we must match death with life and only God fulfills like that. It solves why we are here and why not dead. Obviously, we don't have all the answers, but it's comforting, and it works for me.

My daughter teen was struggling with suicidal ideation and self-harm, and the children's hospital was doing a trial and used it on her only one time! she hasn't had any of those behaviors since! I'd like to add my daughter has struggled with PDA Autism and adhd her whole life.

I was treated with a single dose back in 2018 for treatment resistant depression and extreme anxiety that began in my teenage years. I've been well ever since. Last year I was formally diagnosed as autistic. At age 57. Has your daughter been screened for autism? It's often missed in people who are not intellectually disabled. Especially in girls, where it presents very differently than in boys. I ask because it was the stress of dealing with (masking) undiagnosed autism that led to my severe anxiety and resulting depression and shut-downs. My autism was missed by dozens of psychiatrists and psychologists. It took an autism diagnosis specialist at Kaiser and a 4 hour interview and and exhaustive history starting with childhood to make the diagnosis. Autistic people are known to react differently than ordinary people to a variety of drugs. Your daughters 1 treatment cure mirrors my own. My journey towards formal diagnosis began with a self adminstered online screening test that took 10 minutes to complete. It suggested I seek further evaluation as I scored high on autistic traits. Beware though - most of these screening tests are outdated and gender biased towards detecting male autistic behaviors, interests and traits. Either way, I'm glad to hear about another person having the same miraculous expereince with the drug as I had =)

I broke my leg last year & had to have surgery. When I came out of surgery & was waking-up. I was in Extremely terrible Pain. I heard the nurses talking about the amount of opiates id been given & I wasn't able to have anymore. Yet I was still in Horrible pain tho. I heard them mention that maybe they should try & give me Ketamine. A short time later I recall feeling like my Leg was across the room. Then out in the hall & then it was outside the Hospital. The pain fading away the farther away my leg seemed to be to me. Ketamine worked wonders for the pain I was experiencing for sure. I just Figured I'd share my Ketamine experience with everyone. I've been extremely depressed since I was probably about 8 years old and throughout my life I've attempted suicide., I'm 28 now and over the course of the past 15 years I've been on every antidepressant you could possibly think of, late last year I did ketamine under medical supervision and just that five rounds of ketamine has done more than 15 years of antidepressants ever did. I feel like ketamine gave me a second lease on life, I'm happier now than I've ever been.

@@bryanutility9609dude, the people I meet in the psych hospital would be doing far more exercise, meditation making their bed, walking in nature then you so it’s no cure. If positive thinking, healthy eating and active discipline was the cure these people wouldn’t be killing themselves.

I wish they were readily available in my place. Microdosing was my next plan of care for my husband. He is 59 & has so many mental health issues plus probable CTE & a TBI that left him in a coma 8 days. It's too late now I had to get a TPO as he's 6'6 300+ pound homicidal maniac. He's constantly talking about killing someone. He's violent. Anyone reading this Familiar w/ BPD know if it is common for an obsession with violence.

Thank you for your work! I'd love to see an episode on Depersonalization and Derealization Disorder or dissociation in general. I work as a therapist and find this to be more common than one might suspect and little understanding of dissociation states that are not due to medications/illegal drugs.

Hi! I'm super curious about your opinion on intra nasal ketamine. I've been doing Spravato for a couple of months but am looking to switch to at-home compounded format. But I'm reading some reddit posts saying that people don't usually like the nasal at-home version for some reason or that it's not as effective. I really don't want to do the oral version unless absolutely necessary...

Great question. It is very similar (the esketamine in Spravato is part of the nasal spray from a compounding pharmacy. It all depends on dose and directions from your provider. Make sure the compounding pharmacy is legitimate too! Easy way is to see if they are PCAB accredited. @@deprofundis3293

I tried 2 courses of ketamine infusions. It was a great experience and lifted me a bit mentally, it really felt like it was sorting through the hard drive of my brain, but it turned out my depression was caused by undiagnosed inattentive adhd and executive dysfunction problems. Now medicated for that I’m not depressed anymore.

+1. Please, share which meds are helping you. I started elvanse and felt a huge improvement in my depression, bofore seeing any improvement in my focus. But after 3 months I'm starting to see the depression slightly increase again. Not sure yet if it's related or not.

Hi Dr. Huberman-fellow Stanford faculty here. I am a pain psychologist in the Stanford Pain Management Clinic. We have many patients with comorbid chronic pain and depression. We offer inpatient ketamine infusions for pain and many patients are under the impression this will also help their depression. Thank you for this deep dive on the topic. Ketamin therapy has changed my life and patterns of thinking. I no longer go down negative ruminating loops in my mind. Neuroplasticity at it's finest! I completed 6 sessions over 15 days back in September and have not felt like I needed to go in for a "maintenance session" and it's almost been a year. Huge success for me! @iceshawty9751 the clinic I went to administered the ketamine intramuscular, so just a shot in the arm. They give one shot which sends you out there then they give another shot about 15 minutes later to keep the effects going for about an hour. I don't remember the dosages but I do know they increased it each session because you do gain a tolerance to it a bit but that and your initial dosage will all depend on how quickly your body metabolizes the ketamine, which is different for every person. I’ve also seen it explained as AMPA is the friend who is always willing to go out but he’s kind of boring. NMDA is the one who takes a lot of convincing but when he comes he’s really exciting. I have been doing IM Ketamine assisted therapy for 1 year now. It has been absolutely transformative. PTSD thoughts and flashbacks I don’t think about anymore, it’s not at the front of my mind . I haven’t forgotten about them but instead it’s like previously I was hauling a heavy suitcase and now it’s like wearing a backpack. It’s easier to carry. Ketamine therapy has also heavily helped my depression, panic attacks, and job retention. I’m manic depressive bipolar type 2 and through the last year I have stopped having panic attacks before work. I can get myself to work, stay at work and feel less depressive overall. I appreciate the little things in life. I have a higher sense of spirituality and a clearer view of what I enjoy about life. It feels as if ketamine is creating new positive tracks in the road. Ridding the old path and creating and deepening a new one. One of the most amazing experiences I’ve been lucky enough to experience.

No mention of severe bladder damage as a risk from prolonged Ketamine use? Anecdotal wisdom on the internet is that consuming green tea or NAC before taking Ketamine is supposed to mitigate bladder damage. Would be interested to hear what the science says on that.

Great question. I take NAC and am considering ketamine therapy for depression, etc. I had one session with a professional-a lovely man, btw-but did not experience any relief from the depression. It cost me $2000.00 😩I may try again.

As a ketamine provider, I greatly appreciate this data driven approach to ketamine education in the treatment of mental health disorders. We need more science-based education in our field. The misinformation from "psychedelic enthusiasts" has refocused the patient's attention from improving lifestyle practices outside of the administration setting instead focus on some experiential or transformational ketamine induced realization. While mindset matters, as you stated, the long term effectiveness and viability as a treatment appear days to weeks after the ketamine administration and they are compounded over a series to get maximum longevity. Thank you again for this clear explanation.

Andrew, this was so well done, and very well timed for me! I am on my last weeks of a six week at home assisted oral ketamine treatment, and so much of this rings true! The treatment has been incredible especially because I was prepared, mentally and emotionally to make some big changes. Better living through chemistry still requires better living… SO TRUE. When I finished watching this, I immediately went and listened to Tim Ferris's podcast on ketamine and am so excited about all of the science and research happening in the psychedelic realm. I've had good results combatting depression with psilocybin but am very curious of the therapeutic effects of ketamine now. Cheers to the psychedelic revolution 😁

Ketamine is incredible for heroin withdrawal. I came off methadone and it was out of my system for a couple of weeks, but I felt depressed and like I was constantly in withdrawal. I did a pretty large amount of K and all the depression and withdrawal feeling completely disappeared.

As always a well rounded and informative episode! In the final 10 minutes of the episode with Maya Shankar she mentions ways to keep yourself motivated while pursuing goals. One strategy was to incorporate a pleasurable experience, such as music when completing a task that is otherwise difficult to stay motivated for. At first this sounded wrong, where on a different episode about the dopamine system you spoke out against using rewards for hard tasks. Is the difference the timing of the positive stimuli, meaning in the first case it’s concurrent while in the latter it’s after the fact?

I had decades of all sorts of anti depression meds with absolutely no benefit. My ketamine (spravato) treatments provided a cure. I have been depression free for 3 years. But…with me…I incorporated meditation with ketamine. I do not know if ketamine alone was the cure. I just googled meditation and got an app…no professional training.

I did ketamine assisted therapy for cptsd and no longer wanting to be here back inJanuary. My life has changed. It was the best thing I’ve ever done for myself. I also really committed myself to doing the integration and I do it daily because even now in august I see daily learning moments, tools, moments to heal something even if it’s a tiny little moment that triggers me.

can I ask if you did IV (psychedelic) or sublingual or intranasal (psycholytic) dose for this process? I am unable to afford the cost of IV ketamine therapy despite being a strong candidate having tried all other antidepressants with no avail - I have "treatment resistant depression", c-ptsd big time and have been doing talk therapy in various modalities since age 14 (now am 25). I am starting low dose psycholytic at home ketamine treatment and am wondering if taking it during therapy sessions would be helpful for increasing the likelihood of my brain accepting new ideas/making new connections. Thanks!

Hi Dr Huberman, longtime listener first time commenter, from here in the UK. Thank you so much for yet another incredibly informative podcast episode. I was surprised you didn't touch on "Ketamine Bladder". Here in the UK we have the highest per capita recreational Ket use in the world and irreversible bladder damage is seen by both the medical and user community as a pretty widespread side effect. Do you have any thoughts on this e.g. if there is sufficient/insufficient evidence or if different modes of delivery are more or less risky. I'm aware of journalism by Chris Hamby in the NYT raising concerns oral administration is riskier than IV in regard to US home prescriptions but of course an NYT investigation isn't exactly a scientific paper!

I felt like he touched on ketamine addiction almost like it was porn addiction, meanwhile this is as serious as it gets, from cognitive impairment to permanent body damage, infamously the bladder but also leads to kidneys, liver, gastrointestinal issues and even to the respiratory tract.

best roa for minimisation is iv, its about the least amount of ketamine passing through your body, it all goes through your bladder so the less you have to take for an effective dose the better

@@Letstalkitoutnow this is only true with oral tho, intranasal, intravenous both work too. rectal is a very nice way to take it, but however its very hard to get a good bioavailability. if administering at home its maybe about as good as intranasal, but if in an infusion setting, you are just risking unnecessary bladder problems because you choose not to IV

Thks for your work. Following you from France. I've started using ketamine a year ago for recreational purpose. But after being experiencing hard and drug resistant depression I manage to get out of 15 years of depression, start going back to the gym. Still buying out of any medical control but I've been so close to end my life for years that I can say it's been a life changer for me. Wish there are more open minded doctor's like u in France. But still it's good to see that I'm not crazy seeing it as a therapeutic drug, not a recreational one. Again thanks for your work. Best regards

As someone who suffered greatly from anxiety and depression, ketamine really saved my life. The effects for me were instantaneous, I'm not religious but I experienced a closeness with the universe and, dare I say, God. I've only used the substance a couple of times when things got particularly bad, however, I had profound experiences that were long lasting in that they offered "hope." Not hope that I could just begin relying on the drug, but that my old perspective and feelings weren't the only possible experience. Remembering how the relief felt gave me a mental snapshot i still go back to now when I'm suffering. I understand that the drug has possibility for abuse, but I think the stigma around it is horrible. Especially, for something that did so much for me when I was in desperate need.

You are so good at explaining "the science." Your recordings may be long but it's because they're thorough. I just got a job with a ketamine infusion center. This job was divinely brought to me and now (after decades of effort to heal trauma) I am starting ketamine infusions on Monday. SO LOOKING FORWARD to a change in brain chemistry/function that does not take months and months of effort and repeated experience to solidify the change. 🎉🥳🤸‍♀️🙏

let me start off by saying thank you for doing what you do. I've completely changed my life in all aspects because of the knowledge I've learned from your podcast! I currently take adderall for my ADD and i have been taking 25m seroquel to help go to sleep. it seems to work wonderfully. is it keeping me from hitting all sleep stages though? I was wondering if you could do an episode on seroquel? thank you so much for what you do!

Neurons are like another universe, so interesting. I am one of those who wellbutrin don't work so I just quit. I was scared of ketamine due to the addictive part. I now use gym, nutrition, talk therapy, proper sleep & supplements to alleviate the depression. It works as long as I don't get a trigger which puts me in a hole once in a while. I would like to treat it completely. Thank you Andrew for this, the knowledge helps.

Used clinically it's not likely for someone to become addicted. Addiction would come with a pattern of habitual use. Which isn't how it's administered clinically.

Excellent episode. As an avid (past) user of ketamine it definitely answered some of the mechanical questions I had about what my brain’s been through. I would have liked to hear a little more about the recreational side of things - it’s hard to find a non-biased report on side effects, usually it’s coming from a “rehab perspective” which basically makes it sound like psychedelic meth. I’ll share one thing: if you snort ketamine, spit the drip (the bit that goes down the back of your throat). To my understanding (and someone please correct this if it’s not or not exactly true) once swallowed and processed by your organs, it crystallizes in the bladder and can cause lesions and ulcers and mess up the urethra. A friend (long time user) recently had to have surgery for this and it sounds awful. If you’re going to party, try and stay out of the hospital;)

as far as i know ingesting ketamine in any way can form crystals in your bladder so spitting the drip isnt going to do a great deal, its the main reason to have caution doing it recreationally and not try not to abuse it. i know of people in their early 30s who had their bladder removed from only a few years of heavy use.

I have been doing ketamine infusions with lidocaine for a few years now at a very high dose 500-700 an hour for 4 hours this is done in a pain clinic for a very aggressive form of CRPS that started in my hand and spread through out my entire body it is very rare but I was the luck one. Ketamine saved my life it breaks the pain cycle and is the only thing that does but the treatment.

Glad for you, I'm trying it soon, had 14 back surgeries and am at wits end dealing with all the pain. Around here they mix the 2 together but it's once a week for 6 total but not 4 hours long. Where do they do it how you said, I'm in PA

This comes from ingestion of troche crystals that irritate the urethra if you do 2x a wk. If you cycle or dont swallow fully, it helps too

It's a complex mechanism related to the NDMA receptor antagonism (which happens systemically). Be careful and let a trained physician assess the risk for you if you use it. Cheers

@@LeslieTaylorFit Look, whatever you've been told about the troches being responsible for ketamine-induced cystitis isn't backed by scientific evidence, at least none that I'm aware of. I'd be glad to read any material you're using as a source to claim this. If the basis is merely anecdotal or opinion-based, it doesn't hold up against more solid scientific evidence. Regarding the risk, obviously, the higher the dose, frequency, and duration of use, the greater the risks are (individual variations in response to medication should also be considered). However, a statistically defined safety threshold hasn't been established yet. So the recommendation is always to use the lowest effective dose for the shortest possible time. This is the case with many, if not all, of the medications we take. Always seek the most qualified professional, and get second and third opinions if necessary. Wish you the best

This is a terrific episode from Dr. Andrew Huberman as usual he delivers powerful information. I would like to know how a regular person could find a clinic/psiquiatric who would precribe and do sessions for ketamine. Medical system is sometimes a laberint where you could get lost specially in mental health, and I do not want to depend on antidepressants only. I would love to have answers from anybody that have tips or experiences. Thanks Dr. Huberman :)

I have PTSD and am antidepressant resistant..have been on the sublingual troches at 80 mgs for about 2 months. It is no miracle drug but in the way it has treated my depression etc. It has worked wonders and has absolutely changed my life in a positive way. I as always appreciate your amazing podcast..I hope you can one day do an episode or if not have one already on the benefits and negative effects of kratom..there is a lot of controversy as of recent I'm the news Was hoping someone like you can break it down once and for all.🙏 Thank you brother

I am one month in on the same dose. So far so good for me, complex ptsd from childhood trauma and some depression/anxiety. I have felt more “space” and less emotionally reactive. Less extremes.

@@pacattack6772 Hi, I started with a small dose and was moved up to 80 over a couple weeks. I use Joyous, I have had a great experience with them. I take mine and then usually lay down and relax with music or a guided meditation on my lunch break. It is not psychedelic for me, but mildly dissociative and very relaxing. I have never experienced a large dose of ketamine.

Depression haunted my life from a very young age, and I was put on a bunch of SSRIs as a child in an attempt to deal with it. None worked, and I just had to live suffering from MDD for over a decade before the possibility of Ketamine was bought to my attention. It was the first thing that actually had real effects, and while I get to enjoy the repercussions of most of my life spent severely depressed, at least now I have the opportunity to try and turn my life around. This was a fascinating look into the mechanics of the drug that has provided me a chance to actually live, rather than simply trying not to give into despair, and I thank you for the excellent lecture and extensive evaluation of the drug, Dr. Huberman. I'm unsure if floating ideas for episodes in comments like this is correct, but I'd be really interested in your understanding of the limits of therapy versus pharmacology (and it is possible you have already discussed this topic and I haven't yet encountered your video on it.) From my understanding both often need to work in tandem, but there are so many different schools of therapy, and figuring out which ones are effective, and for which kinds of problems, is often quite difficult in my experience. Never mind determining which kinds of medications might assist in dealing with specific issues, which are worth the downsides, etc.

From personal experience, MDMA assisted therapy is even MORE helpful and beneficial than K...totally different neural pathways and everyone's system is different of course, but persistent MDD for me and that's BY FAR the best I've found✌🏼

@@bryanutility9609 but if u have thought and lived a certain way for very long time even if u figure out a lot of things are lies depression lies and or anxiety lies and figure some things you thought were bad ways of thinking of things and u know more the things u need to do in life but u suck at applying it like simple things u feel like it is impossible . Certain things I thought were depression or I couldn't do because of depression I felt less depressed and I can do things now and oh boy I'm gonna get this place tidied and organised . Boy, did I ever not be able to do some simple life things like organising. I kinda lost it , it made me anxious and it seemed utterly impossible to get it all done. I was way too upset and idk why just trying to be organised could be such a disaster . was even more disorganised with piles in other places lol. I can laugh now but the level of angst? Rage! Despair I felt cause I couldn't organise was over the top. I just felt I should be able to do this and it was not that way at all! I understand being so messed up u don't even care or have the energy or any motivation for life stuff like showering getting out I f bed and being really just really super non functioning. I get it . But I don't get , even now, why such a seemingly simple thing as organising cash be so difficult ? I do know if I focus on a little areas or just certain things i actually can organise lol cause I'm no where near good yet hut
