He used to get busted for flipping teachers off all the time, so it makes sense that he would want to be friends with the kid that can get away with it. Tourettes is an illness that goes on a spectrum! Some people just make involuntary sounds but do not cuss. Some people have involuntary motions but don't make much or any sounds at all. It's actually pretty rare for somebody with a tic disorder to have Coprolalia (ticing of curse words and rude phrases) and some people with Coprolalia also have rude gestures, like making jerk-off motions and flipping people off. Tourettes can often be worsened by stress, which is why a person shopping might have steady tics and suddenly get worse when people start to look around and stare. It can also be triggered by other people ticing or repeating your tics after you do them. Tic disorders have been shown to vary day by day and week by week. Somebody might have a really bad week where they tic nonstop and then a whole month after where they barely tic at all. Tic disorders also lead into Tic Attacks, which is when a person starts to tic nonstop. Your body jerking in a motion over and over for an hour is a known cause of injury for people. I have a tic where I turn my head and during a tic attack I have pulled a muscle in my neck a few times now. Tic disorders can be late onset and a person who never had tics/tourettes might wake up one day to find that they have developed the condition. Sometimes it's triggered by certain medications or traumatic events. I only learned after going to a neurologist that while this is uncommon, it happens all over the world. Making a scene out of yourself in public is no joke. Especially since so many people know very little about what it means to have or develop a tic disorder.

This is why I love South Park. They make it openly clear they poke fun at everything and everyone under the yellow sun yet they still get fans from the very thing they made fun of. Like yourself and a Jewish friend of mine his favorite character is Cartman. It's so amazing that people can learn to laugh at themselves. And south park doesn't pick sides either they make fun of trump in one episode than point out flaws in Obama in the other. It's genius. Yes there's some lines they shouldn't cross but I don't think matt and Trey care.
