In liver cirrhosis, there's under production of SHBG. This leads to elevated free androgens as androgens are bound to SHBG. Free androgens are converted to estraone Which causes gynaecomastia

Gynecomastia in cirrhosis due decreased metabolism (excretion) of Oestrogen. The increased levels of oestrogen stimulate proliferation of breast tissue

In liver cirrhosis, androgen is converted to estrogen in adipose tissue. cirrhosis liver cannot break down estrogen, so gynecomastia happens. I already told my alcoholic uncle.

Liver failure means impaired biotransformation so estradiol accumulates in tissues, stimulating proliferation of breast tissue. Aromatase is still functional in the gonads so testosterone is converted to estradiol, amplifying the pool.
