Lily Nichols is a Registered Dietitian/Nutritionist, Certified Diabetes Educator, researcher, and author with a passion for evidence-based prenatal nutrition and exercise. Drawing from the current scientific literature and the wisdom of traditional cultures, her work is known for being research-focused, thorough, and sensible.
Lily's bestselling book, 'Real Food for Gestational Diabetes' (and online course of the same name), presents a revolutionary nutrient-dense, lower carb diet for managing gestational diabetes. Her unique approach has not only helped tens of thousands of women manage their gestational diabetes (most without the need for blood sugar-lowering medication), but has also influenced nutrition policies internationally.
Lily’s second book, 'Real Food for Pregnancy', is an evidence-based look at the gap between conventional prenatal nutrition guidelines and what’s optimal for mother and baby. With over 930 citations, this is the most comprehensive text on prenatal nutrition to date.
Lily is also creator of the popular blog, www.LilyNicholsRDN.com, which explores a variety of topics related to real food, mindful eating, and pregnancy nutrition.

After start a keto diet for at least 5 weeks, finally I got pregnant after 2.5 years trying at 36 years old. I wasn't expecting this "side effect" 😁 but thanks god happened. I continued this keto diet but my doctors were very concerned about it when ketones in my urine appeared. As a result of my urine test I was told to eat a "healthy" diet, full of carbs... the results: gestational diabetes. I wouldn't say another factors contributed to the development of GD, but the way I managed the blood sugars was by doing mostly keto. They (doctors) really scary you about ketones and I was forced to eat carbs but it is an inconsistency what they tell you. I was a whole mess between what doctors told me to do and the results I was having with keto while pregnat. I decided to follow keto diet (once in a while I ate carbs) and I have a healthy baby who was 7pounds and 7ounces as a new born. My point is that many doctors don't even know the benefits of low carb and we are supposed to trust them.

Low carb dietitian from Poland here 👍 If I said keto was good for pregnant women and spread that (good!) news among dietitians and doctors in my homeland I would not only be completely ostracised but possibly also jailed. Polish nutritionists and doctors on the sound of the word "lchf" get so mad they act as if possessed. It is extremely hard to work when you know you're right and others only keep making more and more people (including children) hyperinsulinemic, often not knowingly. I intend to persist because I know low carb works beautifully hoping my people will eventually wake up when diabetes will be so widespread that healthcare professionals will have to deal with the conseqences of the mad high carb diets they recommend.

I got pregnant while doing keto. Blood sugars were perfect. Because I had previous history of diabetes my dr sent me to a diabetes clinic. They told me I had to eat more carbs and when I told them I didn’t want to they got upset. So being dumb I gave into their demands. My blood sugar kept increasing as they pushed more and more carbs. Then they started insulin and if I had too much they’d counteract with more carbs. Eventually they maxed me out on insulin but continued to push carbs. I begged them and told them that if I ate less carbs I’d need less insulin. They wouldn’t listen. My blood pressure became out of control. It was a very hard pregnancy and birth. My son had issues when born because of how high my sugars were even in insulin.I am now pregnant again and doing keto. I will not let the clinic push carbs this time. I know what’s best for my body and it can not handle loads of carbs and be healthy.
@hamdimahamed6427
@hamdimahamed6427 Жыл бұрын
Wow, that’s crazy that the clinic pushed carbs more and more when clearly, your body didn’t need them to stay healthy. I’m so sorry they did that to you. I’ve been on keto for 5 months and thinking about starting a family soon and definitely want to continue keto throughout pregnancy. I feel so much better throughout the day and don’t have blood sugar crashes or cravings at all. Protein and fat are essential nutrients for our bodies and our babies and not carbs. It’s ridiculous how the medical field is so ill-informed in terms of nutrition. Congrats on your pregnancy btw. Hope you’re doing better than last time.

That conventional diabetes diet plan didn't help my mom. I had her off carbs or extremely low carb plus no starchy foods. That resolved her diabetes. No more meds. She could eat what she wanted once in a while as she continues a healthy lifestyle. The dieticians said that we shouldn't do that and I said, "Your diet plan and education didn't help at all. What we tried helped 100%. So why should we follow your directions to go back to your diet plan?" Back on topic, I keep track of my glucose and blood pressure. I normally take in a low carb diet. So my glucose levels are normal. I noticed my sugar levels were high due to the diet plan my OB doc gave me, which contains lots of carbs and sugar. I didn't pass my initial GB test and had to do the 3hr fasting test to confirm. I decided to eliminate sugar and most carbs and that helped regulate my sugars again so I wasn't diagnosed with GB. So, my conclusion is that not all diet plans are actually good for you. Do you research, testing, and validation.

Thank you for this interesting presentation. Many facets of low carb nutrition have been covered over the years, but the place for low carb eating within the context of pregnancy is not something that has been covered in depth. I appreciate you for covering a topic that has seldom been given exposure. As with many misgivings that the public and medical profession have with low carb, whether pregnant women should adopt this eating pattern can best be assessed - in the absence of clinical data - by looking at it through a historical or evolutionary lens. As Ms. Nichols points out here, if having a low carbohydrate diet were that pernicious to the development of the fetus, humanity would never have made it to this point, because, for most of human history, rich sources of carbohydrate were in scarce supply. Given the dearth of plentiful carbohydrates throughout much of human existence, we must have developed adaptations for us to run on fat/ketones for fuels. It logically follows that the fetus also has these adaptations. Otherwise, every successive generation of human beings, prior to the advent of agriculture, should have shown increasing levels of mental impairment and physical disability. Modern day hunter/gatherer population should also be experiencing a rampant spread of mental retardation and physical handicaps. On a side note, it is interesting that epidemiologists will jump to making causal connections between associated phenomenon when it favors the establishment narrative (i.e. high carb/low fat is good for you), but then don't make such leaps in logic when associations work against that same narrative. After all, most studies of western dietary patterns show an increase in carbohydrate consumption has occurred in the West for the past few decades, with a concomitant reduction in overall fat intake. We have also seen increased levels of autism, ADHD, and food allergies in children. Now logically we should never make a causal connection between increased carbohydrate consumption and any one of these maladies, based simply on observational evidence of an increase in these two things. We would be guilty of the same error that we criticize epidemiologists for making. So I would never advocate for such a non sequitur. However, following the same illogical thinking of epidemiologists, if one phenomenon is on the rise (i.e. increased carb intake) and another phenomenon was on the rise (ADHD, autism, and allergies), why don't these same epidemiologists, who like to denigrate low carb, say that the former causes the latter? Why won't epidemiologists erroneously jump to this conclusion? Could it be their funding?

Always follow the money..
@KR-jg7gc
@KR-jg7gc 3 жыл бұрын
Because that the money as mentioned 🤣🤣.. the medical / health space / pharma would be DEAD if 90% of the world was healthy. But my main comment was to say they won't do that. Because cognitive dissonance is very real. I also have come to realise since I got interested in the carnivore diet that there's actually NO naturally occurring High grain field anywhere in the world. So no way on earth we ever ate high carb diet. Second thing is fruits and nuts are also a fallacy. No where in nature can you walk and find the so called rainbow colours of "healthy fruits and veg". None. You won't find a single field of 10 different fruits and veggies all in season, 365 days a year. Doesn't exist. Yet we're told the more colour the better. But again, that's where the money is at 😏

No seed oils, no dressing. I was finally able to keep food down which never happened to me in any pregnancy. Towards the last few weeks of pregnancy it always goes away. Because of that I got off keto and I feel like trash. I’m tired, exhausted, I’m getting headaches and having trouble sleeping. Yet you mention it and I hear also KenDBerryMD mention it. Could you elaborate on that please? In any case thank you so much for this video - it is so aggravating to find out that regulations and guidelines are based on no or not solid evidence, and that when one tries to do something that's evolutionarily healthy, the health system tells them they are wrong. This video needs to be shared more and more. I hope more research appears soon...

Can I just add that the dangers of a macrosomic baby can be catastrophic for the mother and infant as increases chance of a traumatic painful and often difficult labour/c section. This not only puts the baby at risk of many undesirable outcomes from hypoxia and birth damage but also for the traumatised and often physically damaged mother. Slow post partum healing, life long morbidity, postpartum depression.

@@winning3329 you are very ignorant and very uneducated. If you do keto while pregnant and happen to lose weight, it doesn’t mean your baby is losing brain cells. You can still give all the nourishments to your baby and lose fat. Even if you are fasting, your body won’t take nourishment from your baby. Your body will take nourishment from your stored fats, and eventually start to take from your organs (if you go into starvation mode) your stored fats will help nourish you and your baby. you can obviously tell that your not malnourished doing keto or even fasting. It is 100% healthy to lose weight while pregnant if you are overweight, because you are losing the fat from your fat storage. Even people at a healthy weight have a large fat storage. And yes it’s healthy to have some fat storage, but most people have too much fat and it causes a wide range of problems.

I'm just learning about the keto diet, still doing more research about it. I am 22 weeks pregnant. I'm thinking to start this diet but I don't know about if it will effect the baby since it's going to be a big change between glucose to ketosis. But if I'm going to start I feel is best to start slowly move away from carbs and more to fats to be safe. I'm afraid if my body goes into shock if just start eating fat all in one day bc i know i eat more carbs than fats. But I just don't know if it's safe to start this keto diet late on my pregnancy.

Hmmmmm, they did a low carb pregnancy study more recently. Published in January 2018: "To assess the association between carbohydrate intake and NTDs, we analyzed data from the National Birth Defects Prevention Study from 1,740 mothers of infants, stillbirths, and terminations with anencephaly or spina bifida (cases), and 9,545 mothers of live born infants without a birth defect (controls) conceived between 1998 and 2011".

@kayzar293 Жыл бұрын
@@flores-rc1uy Background: Folic acid fortification significantly reduced the prevalence of neural tube defects (NTDs) in the United States. The popularity of "low carb" diets raises concern that women who intentionally avoid carbohydrates, thereby consuming fewer fortified foods, may not have adequate dietary intake of folic acid. Methods: To assess the association between carbohydrate intake and NTDs, we analyzed data from the National Birth Defects Prevention Study from 1,740 mothers of infants, stillbirths, and terminations with anencephaly or spina bifida (cases), and 9,545 mothers of live born infants without a birth defect (controls) conceived between 1998 and 2011. Carbohydrate and folic acid intake before conception were estimated from food frequency questionnaire responses. Restricted carbohydrate intake was defined as ≤5th percentile among controls. Odds ratios were estimated with logistic regression and adjusted for maternal race/ethnicity, education, alcohol use, folic acid supplement use, study center, and caloric intake. Results: Mean dietary intake of folic acid among women with restricted carbohydrate intake was less than half that of other women (p < .01), and women with restricted carbohydrate intake were slightly more likely to have an infant with an NTD (AOR = 1.30, 95% CI: 1.02, 1.67). Conclusions: This is the first study to examine the association between carbohydrate intake and NTDs among pregnancies conceived postfortification. We found that women with restricted carbohydrate intake were 30% more likely to have an infant with anencephaly or spina bifida. However, more research is needed to understand the pathways by which restricted carbohydrate intake might increase the risk of NTDs.

You should do your own research before commenting like that. Pregnant women get into ketosis much quicker because pregnant women NEED and become naturally insulin resistant in order to feed their baby (fetus) with nutrients so it's a natural process. You can ask about that to any doctor. Therefore, if a pregnant woman eats 80 gr of carbs a day it's like if she'd be eating 30-40 gr of carbohydrates. She has another life to feed.

the longest living population (very thin) from Okinawa Japan ate almost all their calories from rice and potato. the diet has been extensively studied - 85% calories from carbs, 10% protein, 5% from fat. at the reverse, the Inuit diet in the arctic leads to horrible health outcomes. in all the large scale epidemiology (the chine study, Adventist health studies, etc) the thinnest healthiest longest living diet groups eat plant based which tends to be very high carb, low fat. the FACT is that the average person on a high fat diet is obese and the average person on a high carb diet is thin and healthy. the TRICK is ditching all the high fat foods in the process. THE PROBLEM then is most people in the west currently don’t want to stop eating animal products. So they were never on a low fat/ high carb diet. You cant remove all the fat from animal products. THERE IS a thin healthy population among us in the west. The whole food plant based (high carb low fat) group. Good image search “vegan body mass index” and look at the graphs. I would not deny some people cant have some success on high fat with calorie restriction. The question is what is optimal for health in the long term? And the answer and evidence is so overwhelmingly on the side of whole food plant based (high carb/ low fat) Get a clue people...

So this woman's lower carb advocacy is rooted on hunter gatherer diets. She's a poorly informed dietitian in ignoring that - there was no obesity, diabetes, and macrosomia epidemics until the 1980s. - the longest lived traditional cultures have high carb diets. - hunter gatherer communities do not have long average lifespans, due to succumbing to food insecurity, and irrational superstitious primitive beliefs. - higher carb diets have been the norm in civilized nations for thousands of years...the reason being grains provide superior food security. Once again, we have an unprofessional health professional deluded by confirmation bias.

helicart or perhaps the research needing to be done can’t because of prevailing beliefs and it being unethical to do so just in case of risk, not even following healthy women and providing guidelines on foods that would meet micronutrient needs and seeing what women choose to eat and following health outcomes based on self selected carb intakes. But never mind that both side could be perpetrating confirmation bias
