What if you do just the karate chops altering left and right hand? Is there a difference in the final knot?

First of all thank you for sharing clearly how to do it. In my opinion it is always helpful to have a few tie techniques in the sleeve. Unfortunately this hand tie method is a huge waste of material suture in oral surgery especially when you are in private practice. The method is used mainly in general and orthopaedic surgery where you don’t have deep and small cavity like the oral cavity. Leaving a long one end unused which happens with the hand tie it’s a financial burden. If you use a needle holder the suture material waste is minimal. I am proficient with both and I use needle holders

If you're holding the suture needle/driver in the left hand, the right hand is doing all the work. Make sure to keep the tension on the suture with the left hand as you continue to form your knots. Hopefully this helps! Keep at it as it takes some practice. All the best!

No worries. This is all done outside the mouth and when you make the tie, you slide the suture down with a finger which will then enter the mouth to tighten the knot. All the best!

If you look closely, you are creating a series of slip knots rather than a series of square knots. If you pinch these finished knots and pull, they will slide down supply/needle side of the suture. Not a secure suture especially with monofilament suture.

Can confirm needs cross hands counter clockwise for second knot to lock in square knot, hes stacking girth hitches on a single line with this method which will slip off when pulled

You should hold the needle driver in your dominant hand while you practice ties. It's better training. Still a nice video though 👌.

Do you find you use more suture material using this method? (Because your tail seems to be longer)

The tail is longer, so yes I do use more. There is typically more than enough in a package for what’s needed for most procedures though. If you want to use less suture use the needle end of the suture to pull on and tighten things down instead of pulling on the tail. All the best!
